The employee completed spraying/cleaning underneath the tail end of the sand conveyor, when they lost their footing, causing them to fall down the embankment.

Employee was standing on stationary barge helping to take slack out of a line, to tie off a barge. The tension built causing the line to come off the timber head striking the employee in the left leg near the knee.

Hydraulic ram used to break back tire bead slipped off rear rim flange and hit technician in head. Knocked off hard hat, broke safety glasses.

Service truck rolled forward and pinned employee's leg against a hopper feeder.

2 employees working, one running skid steer, pulling axle from beneath crusher. Injured person reached beneath axle, and other employee accidentally moved skid steer forward and pinned injured person's hand. Bone between wrist and knuckle on little finder was broken.

Employee getting into water truck, slipped, caught right arm.

Individual sustained fractured left fibula and injured right shoulder after being knocked off a barge into the river. Coworkers state the individual was hit by a cable on the left leg, which knocked EE down into the water between two barges.

Employee was setting a conveyor in place with a crane when his hand was pinched.
