Employee was carrying a ladder and felt a sharp pain in EE's wrist. Employee iced EE's wrist. Near the end of January the employee felt that it wasn't getting any better and decided to have it checked and ultimately have surgery. Had surgery on 1/28/26 and returned to work EE's next scheduled day of work, 1/31/26.

Employee was carrying fire extinguishers to the safety cage for inspection. EE's elbow became sore with discomfort from lifting the extinguishers.

Employee was walking on the rail car, slipped and landed on a knee. 03/18/2025 employee had knee repair surgery

Employee was walking on top of railcars where employee fell and cut leg.

Employee was installing buckets on a belt near the dry plant. Next morning the employee was experiencing back pain.

Snow, blowing snow, freezing temperatures, icy conditions. EE stated EE slipped while working on the sand dryer & caught self, hurting shoulder. No witnesses. Tried physical therapy but progressed to surgery on 4/23/2024.

Status change from a previous non recordable incident which involved a muscle strain which now is upgraded to minor surgery.

Employee was cleaning out the maintenance truck, bent over to pick something up and felt a sharp odd sensation in lower back.

Employee was attempting to unplug an inlet using a bar. The bar slipped causing EE's hand to strike the edge of the inlet opening. The impact caused a cut on EE's left hand middle finger through EE's work gloves. Employee was taken to ER for stitches and returned to work at full duty.

Employee was kneeled down plugging in an air hose. Employee then raised up and turned to leave, running headlong into the frame of a conveyor. Hard hat took the brunt of the impact, but it is suspected that their reaction of jerking their head backwards to try to avoid or lessen the impact caused whiplash to their neck.

After an overnight ice storm, employee was walking over icy ground and slipped and fell, fracturing fibula. The area is not a regular travel route that would have been treated for icy conditions.

Maintenance tech was pulling wire for an extended period of time. Later that evening EE's back became very sore and EE was out for the next 2 days. Employee went to Physical therapy which helped EE's back. Physical therapist believed EE just had a hip out of alignment. EE reported to supervisor but supervisor did not report up to management until November 2.

EE was operating a dingo cleaning around equipment. EE had been reversing & was in the process of turning when EE's hand was smashed between the dingo grab bar & a cement pillar. EE was taken to ER for first aid. ER made an appointment for EE to see EE's primary care physician the next day. The smash caused a fracture of the phalanx of the finger & a laceration.

ee was assisting another employee move a rotap that weighed approximately 200 lbs. EE did not notice any pain while performing the task. Later on that evening ee began to experience pain in right groin. Upon returning to shift on 1/27, ee notified supervisor and was sent to the Dr. where they were diagnosed with a right groin strain and then placed on restriction.

Employee was shoveling sand and started to experienced pain in right side. Diagnosed with strained right latissimus dorsi muscle.

Employee was walking through locker room to access time clock for punching in and collapsed after experiencing breathing problems. Employee was transported to local hospital where the employee's condition worsened and employee eventually passed away. Cause of death listed as respiratory arrest.

EE was splashed with hydraulic fluid in EE's right eye after the hydraulic fluid cap EE was attempting to tighten unexpectedly came off due to a pressure build up in the system. EE received medical treatment for inflammation of the right eye with a prescription NSAID eye drop.

Employee states employee bumped hard hat on a down pipe at approximately 7:00 pm. It did not knock employee down, knock off employee's helmet or cause employee any pain. Approximately 5 hrs later, employee was complaining of a headache & nausea & then briefly passed out so an ambulance was called. Employee was treated for concussion like symptoms and released from hospital.

Employee was working in shop at the mine removing the worn teeth off a bucket on a CAT 335E excavator. The pins and wedges were removed and EE was hitting the tooth with a steel hammer to loosen the debris packed between the tooth mount and the tooth itself. A shard of metal from the tooth came off striking EE in the chin below the lip causing a laceration requiring two stitches.

Employee was operating water operated cannon. While having the water pressure adjusted, the water stream changed striking a metal beam which caused water to splash back onto employee. This startled the employee and ee involuntarily through up arms in response. Employee states pain began in left shoulder after this incident.

While attempting to adjust the forks on a telehandler, employee pinched thumb between the back of the fork and the backrest on the mast. The pinch caused the skin to rupture. Employee was taken to Emergency Room where ee received 6 stitches and was placed on restriction.

Employee was jerking on a chain link fence gate trying to free it from sand that had washed into the area from the previous nights rains. After EE finished, EE felt a discomfort in the shoulder. Employee ended up going to the Dr 2 days later where EE was diagnosed as having a strained bicep in the right arm.

Employee was walking up a flight of steps to the second floor of a facility. As employee was approaching 2nd floor, a pop in the right knee and immediate pain was felt. Employee applied first aid measures but eventually requested a dr's appointment; during which, employee was placed on restriction and referred for further evaluation.

Employee was working outside underneath a conveyor line when ee felt a bug crawling under pant leg. EE tried to kill it but it stung ee before ee could. Having a bee sting allergy and having seen it was a bee, employee administered an emergency epi-pen that ee kept on site.

Employee was assisting another employee pick up a 55 gal barrel that had fell over. As employee was pulling on the top, felt a pop and a sharp pain in left bicep muscle near the elbow. Employee was seen by a Dr. and diagnosed with a torn bicep muscle.

Employee with a known severe allergy to bee/wasp stings reported to supervisor that EE thought EE got bit or stung by something. EE did not see anything but felt a pain in EE's shoulder. After approximately 10-15 minutes, employee began experiencing adverse symptoms so EE decided to self inject EE's epi-pen that EE kept on site. Employee taken to hospital for observation.

Employee was walking backwards talking to another employee in the equipment parking area. As EE turned around, EE turned into a fill pipe on the side of the parked water truck. The pipe struck EE in the right upper rib cage shoulder area causing EE to fall. Employee complained of right shoulder pain after incident.

Employee was operating an industrial vacuum to clear out dirt from around a pipe that needed repaired. As employee was using the machine, EE got the end of the suction line too close to the ground causing it to abruptly suck down to the ground. This abrupt movement caused the employee to be jerked down too. Employee strained lower back as a result.

Employee was lifting a tow rope for transport when pain began occurring in employee's back.

Employee was opening the lid of a rail car. When employee bent over and pulled on the handle, employee felt something occur in lower back.

Employee was attempting to retrieve a piece of equipment from a platform approximately 10' high that requires fall protection to access. The employee did not follow company policy by not wearing a harness. In attempt to maneuver the equipment, the employee became unbalanced, and fell from the platform.

Employee was completing normal work tasks on 12/28/15, at which point he slipped on a patch of ice and fell down. He informed me of the incident, but felt that any stiffness would go away in a few days. On 1/7/16 the employee informed me that his condition did not get better, and he was going to go the chiropractor on 1/8/16.

Team Member WAS CLOSING A RAILCAR LID AND WHEN I DID I FELT A POP IN HIS LOWER BACK AND INSTANTLY HAD PAIN. IT WAS A CLEAR NIGHT, NO RAIN. HE WAS DOING HIS NORMAL JOB AND JUST MOVED WRONG

Employee was working on removing ice as he stepped off the last step on the ground level set of stairs with my left foot and slipped on a flat sheet of ice. Investigation Findings: At the bottom of the stairs on the line 1 diester there was a layer of packed snow and ice covered by water as the ice was melting. There were no signs of ice or snow on the stairs or hand rails.

Employee was getting air filters off the warehouse rack. He was using the 5 foot mobile stairs instead of the 8 foot stairs. when he reached for the filters on the top shelf at a height of 12 feet he lost his balance and fell backwards off the stairs, landed on his right heel. Employee has a broken heel

Employee was taking a hydraulic line off Volvo 220G loader.

Employee was preparing to clean computers in control room when he set the air compressor down his back had a spasm.

Employee was stepping down off rail car with left leg and spraining left ankle.

Cutting outboard bearing on a 2 ton fan on south side of building. Bearing would not slide off and had to cut off with grinder. Outside cuts were made. When working on last inside section, the grinder blade shredded causing pieces of blade to hit top of the employee's left hand. PPE worn: hearing protection, face shield, mechanic gloves, safety shoes, and safety glasses.

Employee was walking on locomotive walkway and slipped on oil causing him to fall onto his RT knee.

Employee was climbing down from D8 dozer, he was facing machine using 3 point contact. He stepped onto the mid rail between the tracks his foot slipped off, he tried to catch himself his arm slipped through the bar and wedged under the fender causing the fracture. This employee's left hand was severely deformed in an accident a few years back and doesn't have a hand to grasp with.

Employee was in a harness on a man lift using a shovel to clear sand off a conveyor belt and felt a pop in his lower back.

Contractor was performing normal work on 3rd floor of 400 building and allegedly hit his head on a low hanging duct.

Employee stepped on a piece of angle iron and rolled his ankle.

Employee was getting out of L220F loader and rolled his ankle on a frozen clod.

Employee climbing over railcar, missed a step and tried to catch himself. Felt pops / snaps in elbow.

Employee was cleaning railcars on a very windy day. He was wearing safety glasses but a foreign object flew in his eye.

Employee was attempting to open a large trough style rail car lid when he felt a sharp pain in his left shoulder.

Employee was drilling holes and a small piece of metal flew out and got by employee's safety glasses and into his eye.

Employee was stepping out of dozer, slipped, and hit his left knee on the dozer track.

Air would not bleed off air compressor line (though both gages read 0 psi) so employees turned attachment and it blew air out and hit employee in the face. Employee sustained ear pressure/discomfort.

Employee opening bottom doors on railcar. One door stuck so employee applied force with bar used to open doors. When door broke free the momentum forced employee and bar to the ground. The 50 lb bar lacerated employee's left ring finger when smashed against railroad track.

Injured man pinched his hand between the manlift basket railing and a metal beam. Operator was looking at wheels of manlift to fit into small opening. Injured man was watching operator rather than observing his environment. Had hand placed outside basket.

Return idler was on the ground in front of the Maintenance shop. Employee was distracted and tripped over the return idler. Stitches in right leg.

EE was cleaning, shoveling sand from conveyor belt and when complete, his elbow was hurting.

Removing frozen material from rail car, strained shoulder climbing out of car.

Employee was standing on a step stool to increase visibility. The step stool was not secured to the floor grating and it slipped through the floor grating. The employee injured his shoulder when he tried to catch himself. Employee did not seek medical attention until 6-5-08. This is when activity became restricted.
