Applying wheel chock to truck trailer, felt pain in his right elbow. Employee lifts the wheel chocks 3-4 times per day on average while conducting CDL driver job duties. Employee did not report incident until 10/22/12 to employer and did not seek medical attention until 10/24/12 at which time this became a reportable injury due to medical treatment.

employee was tarping the reliance trailer when the double jointed tarp rod mechanism slipped out of his left hand and spun back in his right hand. This motion caused the rod to get bent out of shape and twisted his right arm into an awkard position causing him to strain/sprain/bruise right elbow.

Diesel mechanic was adjusting the gates underneath the Garwood Trailer when his wrench slipped and hit some mud/dirt that was stuck on trailer knocking it into his face and eyes. Employee was wearing prescription safety glasses at the time of the incident. Medical Treatment, prescription antibiotic given when employee decided to seek treatment 4 days later.

While excavator operator was mining in open pit south curtis wright area he noticed diatomaceous ore movement from his right side he quickly tried to back up machine and get out of way. Classic wedge failure in wall occurred which caused the safety catch bench above excavtor and highwall above it to fail brining down debris and partially covering/trapping excavator.

HIT BY & BACKED OVER WITH A PICKUP TRUCK. SETTING UP TO SERVICE D9R CAT. WASHING D9R WITH WATER TRUCK, WHICH WAS GOING TO DRIVE AROUND TO WASH OTHER SIDE. I WALKED ONTO ROAD TO MOVE OUT OF D9R & WATER TRUCK'S PATHS. THEN I FELT THE PICKUP TRUCK HIT ME.
EE accidently backed pickup truck into and over coworker. Post traumatic stress syndrome.

MINE OPERATOR NOTIFIED OF INJURY 6/22/03 BY EMPLOYEE SEEKING MEDICAL EVALUATION WITH HIS PERSONAL PHYSICIAN. ONCE ADVISED BY EMPLOYEE, SENT TO COMPANY CONTRACTED CLINIC. DIAGNOSIS MADE 8/19/03 OF EPICONDYLITIS, HAD NOW MADE THIS REPORTRABLE WITH MSHA. INJURED EMPLOYEE WAS TARPING A TRUCK WHEN WIND BLEW AND PULLED ARM CAUSING PAIN.

EE WAS OPERATING WATER TRUCK WHEN HE BECAME LIGHT HEADED. HE PULLED OVER, PARKING VEHICLE TO TAKE A BRIEF REST. HEAT EXHAUSTION SYMPTOMS THEN TOOK OVER AND A COWORKER REALIZED HE HAD BEEN PARK ED TOO LONG AND CHECKED ON HIM. EE WAS TRANSPORTED BY COMPANY VEHICLE TO MEDICAL ATTENTION WHICH HE RECEIVED 2 HOURS OF INTRAVENOUS FLUIDS & WAS THEN RELEASED.
