EMPL THOUGHT HE WAS DRINKING WATER BUT HAD PICKED UP JUG OF BRAKE CONDITIONING FLUID-METHANOL INSTEAD.THOUGHT WATER TASTED STRANGE BUT TOOK A SECOND DRINK BEFORE LOOKING AT JUG SEEING IT MARKE D AS POISONOUS.
WHILE SPLICING A NEW SECTION IN THE CONVEYOR BELT THE KNIFE SLIPPED CUTTING IN TO MY LEG
IN SHOP WELDING GOT DUST AND FLASH BURNS TO EYE

OPERATOR WAS ADVANCING LOADER INTO BROKEN ROCK PILE TO LOAD BUCKET BUCKET CAUGHT ON ROCK IMPEDING LOADERS MOVMENT AND CAUSING OPERATORS HEAD TO JERK FORWARD.
CLEANING BOWLS SLIPPED AND HIT HEAD AND TWISTED KNEE.
WHEEL OF SCRAPER STRUCK ROCK IN ROAD WHILE HAULING AND THREW OPERATOR UP AND SIDEWAYS IN CAB. OPERATOR WAS WEARING SEAT BELT AT THE TIME

EMP WAS MOVING 5 LOADER ON ROAD BEHIND THE MILL. HE STARTED AROUND TURN GEARED DOWN FROM 3RD GEAR TO 2ND GEAR WHEN GOING DOWNHILL. THE ENGINE DIED AND OPERATOR LOST STEERING ABILITY. DROPPED B BUCKET AND SLID3LOADER DOWN BVANK BANGE DHIS HEAD ON CAB FRAME AND STRUCK LOADER
HE WAS USING A 7ORGUE WRENCH TO TAKE THE BOLTS OFF THE BELLY PAN OF A D-9 DOZER. HE WAS WORKING APPARENTLY WITH HIS HANDS AND ARMS NOT IN THE CLEAR. HE HAD JUST STARTED LOOSENING THE LAST BOL T, WHEN THE PAN3BROKE FREE AND BY FALLING, PINNED HIS LEFT HAND. COME-A-LONG AND OR BLOCKING WERE NO USED.

TWISTED MY LEFT KNEE DUE TO A HYDRAULIC LEAK WHILE GETTING OFF BACKHOES. OIL WAS ALONG THE FRAME, I WAS REFILLING HYDRAULIC TANK IN FRAME.
