Employee was checking dust curtain, as they were walking, they tripped over a rock and sprained their left ankle.

Employee stepped off of the frame of the shovel and as EE stepped down, EE lost balance and fell as EE felt strain to EE's left leg

While a maintenance employee was installing a spring on a fan belt, a ratchet slipped causing a deep laceration on EE's left forearm.

Employee was exiting the bus when they slipped.

Taping a drill pattern and twisted ankle on uneven ground.

Haul truck operator's foot got stuck in floor liner, tried pulling out foot and heard a pop and felt pain.

On April 22nd, employee reported pain to knee and refused treatment. In July, employee reported to supervisor that knee was still hurting. During appointments in July and August, no work restrictions were prescribed. In September MRI diagnosed the employee with a torn meniscus and surgery was scheduled for October 9. Employee had no work restrictions until date of surgery.

Employee slipped while exiting a commuter bus

While cleaning a window on a dozer, the wind caught the door and the employee tweaked their wrist on the handrail

While picking up a 12" valve, employee heard a popping sound in their chest. Started to hurt a few hours after the pop.

While an employee was cleaning a window they felt a pop in the right rib area

While lifting a section of walkway grating into position the worker's gloved right pinky finger was pinched between the grating and the frame it sits on causing a pinched finger with a minor cut.

Mechanic was removing a drain plug from the differential on 599 lube truck. When the plug came loose the wrench came down quickly causing the left pinky of the employee to strike the concrete floor.

Employee was lifting a box and pulled/tore/ruptured their rt. Biceps muscle.

Employee was checking a sump in acid wash area when EE walked a small containment wall and twisted left ankle.

The employee was cutting the top of a cone off. EE's hand slipped and the knife unlocked causing injury to EE's index finger.

The employee experienced an STS in both ears on the annual test when compared to the 2004 baseline. An audiology retest confirmed the findings for both ears. An audiologist review of all hearing records indicates the STS for the left ear meets the criteria to report. The STS for the right ear does not meet the reportable degree criteria.

While trying to line up a pin on a drill rod support adjuster, the employee's hand slipped and the left middle finger was pinched between the adjuster and the pin boss.

Employee was walking across the tire pad when EE felt a pop in their right ankle.

While removing old lip on 346 shovel, employee was using a slip hammer when a piece of metal hit the employee in the groin.

IP was holding the bolt with vice grips while cutting with a grinder when the grinder kicked and caught the top of EE's thumb. IP went to driller and reported injury. All notifications were made, and IP was transported via crew truck to Battle Mountain for evaluation. The IP received 3 Stitches.

An employee was filling a Haul Truck. When the employee grabbed the DEF nozzle the employee twisted their back

The IP tried to control the falling pipe during the process of placing sounder pipe in well. The pipe made contact with IP left middle finger tip after it slipped out of pipe already lowered in hole, Causing an avulsion and fracture finger tip after falling 6inches.

During shift change, the employee was found unresponsive in equipment. Attending physician pronounced employee deceased on scene.

While performing pre-op inspection of haul truck, employee slipped on ice and fell to ground.

Employee was walking to the bus and slipped on some ice.

While fueling the haul truck during a wind gust, the employee got debris in the eye. The employee did not report the event to supervision until 11/10/21

Driller was fighting a bad hole, mistakenly got hand under bottom stop for drill top head where EE knows not to put them, top head smashed right thumb.

The employee was pulling and pushing on 8 and 12-inch pipe and valves to line up the pipe for fusing. On 10/21/21, the employee reported swelling and discomfort in the lower abdomen. The employee was transported to BMGH diagnosed with a small hernia. On 10/27, the employee requested a follow-up at the clinic.

Employee was moving a shovel cable loop when they felt a pop in right calf.

Employee stood up before the bus came to a complete stop when the bus pulled into the parking ditch the employee slipped and hit right knee, hit the metal part of the seat. The employee continued to work until surgery date with no restrictions. Employee did not receive any medical attention or medications.

Employee was tasked with blend grinding a weld on the bottom of the car body. A foreign body metal shard embedded in the employees left eye.

While the employee was removing nuts from rubber tire dozer cutting edges, the air exhaust from the one-inch impact gun blew dust and other materials towards the employee. The employee felt something in eye but continued to work on 12/19 and 12/20. On 12/21 the eye was red and irritated and the employee reported it to the supervisor.

Metal on metal between 2 haul trucks traveling on haul road. Fatigue was the contributing factor.

Employee slipped off a dozer while exiting the equipment.

Employee was removing haul truck tires using a rad gun. When employee began to remove a nut on haul truck tire, the rad gun was not set to remove causing rad gun to tighten and pinch finger between nut and rad gun.

Employee was moving tables in the ambulance bay and tripped on the ambulance stop blocks.

Employee was helping change GET on 343 shovel. As EE turned to hook tooth to crane on truck, EE twisted left ankle and fell to the ground. Fellow employee helped EE up and called lead man.

While an employee was being loaded underneath the shovel, the employees neck and back were jarred when the material was loaded into the back of the haul truck.

Employee stepped off the Dozer while performing work and twisted their ankle.

Employee was digging in frozen drill cuttings for a sample when experienced pain in neck and back. Reported to supervisor on shift and requested medical evaluation.

Employee was working on engine, slipped on intake pipe and fell

Big rock fell from the shovel bucket and landed on dove tail of haul truck. Bouncing the truck.

Employee bent down to open the door on connex and when employee pulled on the door felt a pain in middle of back.

While placing pipe on a dozer at leach pad, the ground underneath broke through and twisted ankle.

Working on the slope of leach pad walking up and down for about 3 hours. Employee felt dizzy and nauseous.

Employee reported that incident occurred on June 25th, 2018 and supervisor was sent a text message at end of shift while on crew bus going home. Employer was notified on July 31, 2018 with a work limitation from personal physician. While pulling self up drill step, employee felt a pop in right shoulder.

While disconnecting a 2" cam-lock hose connection the employee was having trouble getting the hoses to separate. The employee put one hose between EE's legs and when the hoses came apart the cam-lock hit the employee in the mouth chipping EE's tooth

While walking to access the cab of rubber tired dozer, employee lost footing/slipped on the decking which resulted in the employee falling into the front hand rail of the machine next to the cab. The employee's head and ribs made contact with the front handrail

While attempting to remove the table hammer the employee raised the hydraulic wrench with right hand and reached for the hammer with left hand and the hydraulic wrench slipped out of EE's hand and impacted the left hand.

Large rock struck haul truck bed while being loaded by electric shovel, jarring haul truck operator.

While off loading a trailer a co-worker pulled on a pallet, containing sacks of aprox. 300lbs., with a pallet jack. This caused another pallet to shift resulting in a top sack falling onto the injured workers back.

Injured employee was disassembling metal tracks. While using a hammer to remove metal pins, a small metal piece of metal flew off the pin and hit the employee in the upper lip. The employee was wearing all face shields at the time. Employee did go to the local clinic and received 4 stitches. Employee was able to go back to work that afternoon and was released to full duty.

Employee was lifting a 4x4 board and felt a pop in back

While working at lime silo, employee slipped on ice.

Drill operator slipped while climbing down boarding ladder striking right knee on ground.

Drill operator was pulling samples with sample catcher when the operator felt pain in lower back.

EE was in van incident that occurred on 10-31-17. EE was evaluated and given full release that same day. On 11-2-17, ee was taken for further evaluation and given another full release. On 11-09-17 employee went to chiropractor for hip re-alignment and was again given full release after treatment. Employee reported chiropractor visit to Marigold Mine on 11-20-17

Haul truck struck man van resulting in fractured ankle.

Haul truck struck passenger van resulting in fatalities. Individual admitted self to hospital for further post traumatic evaluation.

While lifting GET for scraper with overhead crane and magnet, the GET slid across hand cutting pinky finger.

After lifting the hammer from the interchange, employee was setting the hammer down to lean on the daybed. Employee slid the hammer down left forearm and a shard of metal off the hammer inserted into left forearm requiring stitches.

The pin on the slot wrench broke while making a connection. The driller asked the helper to move the wrench back into place by hand. After doing so the driller placed hand back on the controls to move the hydraulic ram. Then the helper moved hand back onto the wrench as the ram moved, pinching EE's finger.

Employee complained of pain in employee's left rear during employee's shift operating haul truck.

Employee was conducting pre-shift exam on shovel after maintenance had been performed and employee stepped on a lifting eye located on top of engine compartment, fell hurting right ankle.

Employee was installing an intake boot when they felt a pain in right elbow.

Employee was in the process of changing ground engaging tools on a road grader when employee felt a sting to employee's right forearm.

Employee was operating a scraper when EE felt a pain in the back.

Mechanic was manually aligning a track pin up for installation and experienced a pulling sensation in employee's bicep when the pin was driven into place.

Employee sprained right ankle while pulling cable in pit. Employee sat down in a pickup and appeared to lose consciousness due to pain. Employee was given restricted duty and was placed in a walking boot.

While torquing lug nuts on the left inside dual on a haul truck, mechanic placed left hand, middle fingertip between the wheel and the rad gun which resulted in a crush injury. The fingertip was amputated at the first knuckle.

Employee slipped/fell on ice that was under snow on the ground causing employee to twist lt. ankle.

On December 24, 2016 at beginning of shift, employee slipped and fell. Employee felt they were not injured but later experienced soreness in shoulder. On January 5, 2017 the employee requested to be evaluated by a physician. An x-ray and CT scan was performed and a crack was discovered in the employee's shoulder resulting in restricted work.

While employee was shutting off a valve to change spin filter, the vent cover came off and struck the employee in the left side of EE's chin and on the right cheek. Small lacerations resulted on chin and cheek which required stitches on both wounds.

While grinding overhead on haul truck, employee received a foreign body to the left eye.

Dozer Operator was pushing berm at dump, when operator turned to reverse, brakes locked up forcing operator to be pushed into seat belt pulling muscle in back.

Employee was welding overhead when a foreign object fell into eye.

Drill hammer needed replacement. Driller began to lift bit when they felt a snap in their shoulder.

Employee was walking down stairs when EE's vision was reduced due to bifocal element in lower portion of safety glasses.

While changing Engine Oil Filters the employee experience back spasms.

While installing #3 tire on haul truck 473 the employee had completed hanging the top center wedge first. As EE began to hang the 11:00 position wedge, the employee extended EE's left arm to line up the holes & felt a popping sensation on the outer edge of EE's left arm bicep at the elbow.

While Cutting Wear Iron from the bucket of a shovel, a piece of wear iron struck EE's right hand fracturing the hand.

Employee experienced back pain while loading haul truck. The Operator was moving into the dig face when EE experienced the pain.

Employee was assigned to clean snow from walkway with a rubber tire backhoe. Employee reported feeling faint, dizzy, short of breath, and had begun profusely sweating. Employee was transported to the hospital. The treating hospital has not presented the company with a diagnosis so the company is reporting as potential heart attack due to symptoms and 10 day requirement.

Employee was stepping down from a Haul Truck and sprained EE's ankle.

Employee stepped on a snow covered rock spraining his ankle

Employee was installing a pipe reducer and felt a pop in his left index finger.

Welder picked up an inert gas cylinder and felt a pop in his right shoulder, followed by burning.

While employee was unbolting a 6' pipe, he twisted to reach behind himself for a wrench and strained his lower back.

While pulling an oil sample, mechanic felt spasm in his lower back. Medical treatment was sought and worker was diagnosed with musculoskeletal low back pain. Worker ordered to days away from work.

On October 20, 2015 the electric shovel bucket contacted the side board of the truck bed which jarred the truck and the driver. The haul truck driver sustained a back strain but did not seek medical attention until November 8, 2015. The injury became reportable on November 9, 2015 when the haul truck driver began modified work assignment.

Technician was dismounting a ladder on level ground and sprained his knee when he misjudged the step to ground.

While cross-ripping a 3:1 slope on Cell 15 leach pad, a Caterpillar D8 dozer encountered a rock which caused the dozer to roll over approximately 120 degrees onto the right side of the dozer. The operator was removed from the cab in less that 30 minutes and there was no injury to the miner. The miner was evaluated at the hospital as a precaution and released to full duty.

Mechanic was removing a ripper tooth from a dozer. Employee struck ripper with a hammer to break it loose from the shank. When the hammer struck the ripper tooth, a small piece of metal struck the mechanic in the right leg below the knee, resulting in a foreign body that had to be removed by a doctor. Stitches were required.

Employee was driving a rubber tire dozer across a pit floor in muddy, wet conditions. The employee drove through a pothole which jarred his back, resulting in a herniated disc and modified workdays.

A Driller was attempting to retrieve a sample when he caught his foot in a measuring tape. The Employee fell forward into the cab widow resulting a strained back.

While removing wear iron from loader bucket, employee placed hand on top of bolt while removing nut with impact gun. Bolt spun and cut employee's left ring finger with sharp edge on head of bolt. Laceration required stitches.

Welder was using a dye grinder to clean up a weld when he started feeling something in his left eye. The employee washed out the eye with a saline solution for 5 minutes and still had the same feeling. The Employee stated that he was wearing full PPE, including a face shield.

On December 19, as Oiler was dismounting a D10N dozer, he jumped from the side of the track. His right foot landed on top of a rock, resulting in soft tissue swelling. The Oiler did not seek medical attention until December 21; restricted duty was ordered effective December 22.

Employee reported to shift at 5:00 AM on 12/16/2014 and reported that he experienced eye irritation after he left work the prior evening. The irritation persisted despite attempted flushing. The employee was evaluated at the hospital on 12/16/2014 and a foreign body was removed from his right eye. It is unknown when or how the foreign body got in his eye.

Employee lost balance and when he caught himself with his left hand he complained of shoulder pain. Employee was placed on light duty.

Repetitive motion injury to right wrist

While positioning a choker cable employee allowed cable to slip and strike him in the mouth, resulting in a chipped upper front tooth.

Electrician was cutting insulation with razor knife and accidentally cut his middle finger and ring finger on left hand. A total of seven stitches were required.

Truck driver was set up to get loaded by the electric shovel. When the first bucket was placed in the bed of the truck, a large rock struck the inside of the bed. The truck driver hauled the load and then reported that her neck was strained. Driver received a restricted work release.

While performing bucket repairs on electric shovel 343, welder was grinding in a corner of the inside of the bucket. The grinder kicked back causing the employee to lose his balance and contact his left hand with the grinding wheel. Although the employee was wearing welding gloves, the wheel cut through the glove and into the middle finger knuckle, requiring two stitches.

Employee reported that he had received flash burns, from reflection, while welding on his previous shift. Employee was evaluated at the hospital, given prescription eye drops to treat his eyes, and released to return to work.

Employee was descending staircase on a Hitachi 5000 haul truck. When he reached the landing in the mid-point of the stairs, he slipped and twisted his knee. Employee was evaluated and given restricted duty.

As two mechanics leaned a belly pan from a rubber tire dozer against their service truck, the belly pan fell over and struck the top of employee's left foot. This resulted in a fracture of the metatarsal.

The injured employee was replacing an engine on a drill 121. He was lifting a fuel filter base when he felt pain in his lower back.

Employee slipped on ice while walking outside NE corner of truck shop. Employee fell down and injured left ankle, resulting in fracture.

While pulling fuel hose from reel, to fill haul truck, employee slipped and fell down, resulting in a fracture to left wrist.

On 7/11/13, employee was dismounting dozer when his foot slipped and he caught himself with his left hand. Employee hurt his left shoulder but did not want to seek medical treatment and returned to work. Treatment was later requested on 7/26/13. The injury became reportable when the employee began restricted duty on 8/1/13.

While walking on the drill pattern, employee stepped on a small rock which caused him to roll his left ankle and fall on his left knee. This resulted in a sprain, requiring 2 days of restricted duty.

On 4/21, while performing welding tasks under a drill in windy conditions, debris got into employees right eye. Employee reported discomfort on 4/22 and was seen by a doctor, but no debris was found. On 4/24, employee reported continued discomfort. Employee was taken to the emergency room again. A small piece of metal was found on the corneal limbus. Metal was removed.

While employee was handling a piece of pipe, he contacted sharp metal with finger tip on left ring finger, causing laceration. Cut required 3 stitches.

Employee was changing struts on a tool box lid. Employees right hand slipped which allowed the lid to fall on his left ring finger. X-rays revealed small chip on the tip of the bone (left ring finger tip).

At the end of a welders work shift, he reported that at some point during his shift, wind had blown dust into his eye. His eye was irritated. After the welder traveled home, he requested to see a doctor. A foreign body was removed from his left cornea.

Mechanic was removing hoses underneath a service truck. Dirt fell from overhead, got underneath mechanics safety glasses, and contacted right eye. Eye was still irritated after flushing out in eyewash station. Mechanic was taken to emergency room where a foreign body was removed from the cornea area.

Mechanic was descending hydraulic shovel using manlift/basket and the locking mechanism stuck. The mechanic exited and attempted free the basket. When the mechanic pulled, the basket released causing the basket to jerk away from him. The mechanic hyper extended his right elbow. Mechanic was seen at the emergency room and given two days of light duty.

Employee used plasma cutter to notch out some tubing. Employee did not use adequate lens for task, resulting in flash burn to his eyes. Employee did not realize he had received flash burn until later that evening. Employee missed one shift (10/7/12) as a result.

While installing protection chains to the right front wheel position on a rubber tired wheel loader the injured slipped and landed on his right knee and elbow causing a contusion to his knee. The injured completed his shift and went to the Dr. the next morning.

While cutting out the equalizer pin bar, the left side track frame on # 542 D10R dozer came down crushing the right leg of the affected employee. Investigation in process.

While picking up chock blocks strained lower back.

Worker was helping 2 vendor mechanics disassemble tooling used to install a hard bar pin in a CAT D10R Dozer (Unit #6013). The tooling was "bound," & each of the individuals began taking turns using a small sledge hammer to free the tooling. EE on his 2nd swing freed the tooling, but a piece of it (most likely a socket) hit him above his rt eyebrow requiring 12 stitches.

Employee injured eye when putting on safety glasses. He poked eye with eye glass temple

Injured Employee was loading trucks when a large rock broke loose on dig face. Employee tried to catch rock with loader bucket as it fell jarring the loader. The employee strained his back due to the incident. The employee was wearing a seatbelt.

While bending over in front of a portable heater to pick up a tool, metal grindings that were on his jacket blew into his eyes.

After incident investigation it was found that the injury arose from a door shutting on her finger.

Employee climbed a leachpad containment berm and slipped on liner and broke right ankle resulting in a loss time injury.

Working in the pulverizing room using air hose to clean out bowl, rock came up underneath safety glasses and struck the left upper eyeball.

While changing Sawzall blade, the operator did not remove battery and cut left index finger.

Worker was shoveling material out of the anchor trench when she felt a pain in her lower back. The worker informed her supervisor, but declined any treatment. On July 11th, the worker asked to see a physician where a micro fracture was diagnosed. No additional treatment necessary at this time.

Assay Lab employee had pain in right elbow. Initial ER evaluation was negative and returned to full duty. EE reevaluated on 9-12-2011 and was perscribed PT for tendinitis and remained on full duty. Rcvd first PT on 9-30-11. There were no lost or restricted work days.

Sprained back while twisting to throw air filters in maintenance dumpster.

Helper was getting choker cable off of casing. When he lifted up on the choker, the choker came unwrapped from the casing and hit him in the mouth. In doing so, it chipped one of his front teeth.

Employee picked up mud tub to load into truck and felt pain in his back. Treatment first provided on March 3rd.

The accident was caused from working on a slick surface, the surface was slick from the weather conditions that we were having on the site. The worker sprained his left knee when he slipped and had his left leg caught in-between an inverted rail and rebar.

Grease was injected into the abdomen of the injured employee while helping a mechinic repair the autolube system on a DML Drill. Injured employee treated at hospital and released.

While stepping down from a portion of the lube truck that had been removed for repairs, the injured employee mis-stepped onto pallet of unstable 10X10 cribbing and twisted his knee. He was treated on the local ER and placed on modified work for two scheduled shifts.

Injured employee (Truck Driver) got debris in left eye. EE doesn't know how it happened. Operated truck with windows and doors closed for entire shift before injury. EE taken to the local ER for treatment.Modified duty for remainder of the shift and next scheduled shift.EE released to full duty. One day of modified work. No lost work days

Employee was tightening a bolt on the cutting edges on the Rubber Tire Dozer when a piece of slag broke loose flying under his glasses and into his right eye. The injured employee was placed on modified work for the remainder of his shift and released to work in the afternoon of the following day. There was one day of modified work and no lost days

Employee was air arcing and grinding and got foreign material in his right eye.

Employee was loading pipe onto truck when his feet slipped on the icy pipe causing him to fall and injure his back.

Employee was walking back to pickup after using restroom and slipped and fell in park ditch. Area was dry with no ice or snow.

Mobile Equipment Operator was dismounting Rubber Tire Dozer at end of shift, while stepping from the last step, felt a pop in the left knee. Employee didn't initially require medical treatment. Employee visited ER after shift was over and was placed on modified work after scheduled days off beginning on 11/21/09.

Maintenance Mechanic was dismounting D10R Dozer on right side and slipped on bottom push bar step resulting in injury to left bicep area. Employee treated at local emergency room and returned to work on light duty/modified work. He remains on light/modified duty. There have been no lost work days on this injury as of the date of this report.

Mobile Maintenance Mechanic was changing GET on loader using steel hammer. A piece of steel broke off of the GET or hammer and hit the mechanic in the leg. Injured employee received medial aid treatment (stitches) at local ER. No lost days or restricted work. Employee released to full duty same day.

EE was opening engine access cover of haul truck and strained lower back. Incident occured on last scheduled work day. Employee seen for follow up on 4/17/2009 and was perscribed light duty. Employee competed three days of light duty and was released to full duty on 4/23/2009. EE missed no schedule work days

Employee was lifting one end of a compressed gas cylinder with the assistance of another employee and strained his right shoulder. He evaluated at local Emergency Room and perscribed light duty for the next three shifts.

Injured employee stepped off of back of blasting truck and twisted right ankle.

Employee was vacuuming chairs and had pain in lower back. Employee didnt report injury to supervisor/mine until 1/22/2009. Employee evaluated by MD on 1/22/09 and returned to work without restrictions/full duty. Employee received medical treatment (First session of physical therapy) on 1/30/2009. No lost time or restricted work days.

Injured Employee knelt to pick up gold bar in refinery. Received a small laceration to the left knee that required two staples to treat. Injured employee treated and immediately released to full duty by local emergency room on same day. Employee returned to work. There were no lost days and no days of restricted or modified work. This was a medical aid/treatment injury only.

Employee was dumping a wheel barrow into a dumpster and strained back.

Employee walking across pit and was struck by a small rock falling from the high wall. Employee initially returned to work without restrictios. Employee began to experience pain in shoulders and neck approximately 4 weeks later. Employee started physical therapy on 10/23/2008.

Employee experienced numbness in left fingers after hammering on wear iron on hydraulic shovel reported on 3/23/2008. Condition was medically diagnosed as bilateral carpal tunnel syndrome on 5/12/2008. Employee released to full work without restrictions.

Assay Lab employee has pain, discomfort and numbness in both hands and wrists possibly attributed to repetitive motion associated the task of spliting and pulverizing samples.

Employee was opening waste rock sample buckets and cut left ring finger with a utility knife. Employee received 4 stitches to treat wound.

Mine Maintenance Mechanic was exiting field service truck when he sneezed and felt a sharp in the center of his back. Employee asked to see physician on Feb 22, 2008. Employee placed on light duty for two days.

Employee states she was unloading a propane tank with her supervisor and it fell onto her foot.

Crew was tripping drill pipe out of hole and stacking pipe against Roligon. Helper's right index finger was pinched between ladder on Roilgon and pipe. Result was fracture to tip of finger. Helper received a finger splint and returned to work at rig with light duty release.

On 8/20 he was pulling a liner over a berm on cell #13 twisted his ankle. Did not want to seek medical on 8/20 date of initial injury.

Employee was walking across drill pattern and twisted right knee. Employee had diagnostic procedure on7/16/2007 with positive findings and received subsequent medical treatment. Employee not placed on light duty or didnt miss any work.

Employee complained of sore right forearm, referred to MD for subsequent evaluation. Employee diagnosed with repetitive motion type illness and referred to physical therapy on 5/15/07. Believe that this is a recurring injury/illness. Associated with an illness reported in Oct 2005. No identifiable event related to this incident.

Employee stepped out of truck and fell approximately 12 feet to the ground.

Employee twisted ankle after stepping off of Service Truck

Employee was rebuilding a drill hammer. He pinched his right index finger when he was putting the cylinder back inside the tube. He received a laceration that required 5 stitches.

Employee was opening paper towels and got dust in right eye.

Mechanic was washing drill and twisted left ankle on hose bundle on deck.

After further investigation and medical review it has been decided that the on site incident did result in an injury. The injury occurred while the employee was pushing an 18" pipe up a berm and began to feel pain in left knee.

Ground lead fell and hit employee in lower neck/upper back. Initially released to full duty. Employee was placed on light duty on 8/30/06 during follow up visit. Employee placed on no duty on 3 Sept 06 during another ER follow up.

The claimant was riding as a passenger in a work vehicle that was pulling pipes when a haul pack ran over the pipe, causing a sudden stop for the claimant's vehicle. The claimant subsequently strained his neck.

Removing ground and gauging tools. EE stood in bucket and hit the ground and gaugind tools with hammer. A piece of metal broke off and hit employee in upper right thigh.

Employee stepped on edge of 4X6 wood block which rolled and twisted his right knee. Employee place on resticted duty by othopedist after MRI and post incident evaluation on 4-27-2006.

Employee slipped on last rung of ladder and strained knee. Initially the employee was returned to work but was later placed on in no duty status (2/3/06) after an MRI.

Employee went around handrail on 992D loader to wash window and fell from equipment.

Walking stepped on mud clog, uneven frozen ground-twisted ankle.

Employee was changing cutting edges on IT loader and cut his right middle finger. Employee transport to local ER and received 10 stiches. Employee released to full duty and returned to his asssigned job. The was no Lost time.

CUTTING HEATER HOSE WITH RAZOR KNIFE, SLIPPED CUTTING LEFT THUMB. Employee received six stitches to close the wound.

SPLITTING OF LAB SAMPLES.

While lifting a wheel chock to put in the holder, employee thinks it caused him to have a hernia.

Oil filter from Hitachi 5500 shovel was being removed and fell and struck employee's right foot.

While pressure washing trailers, the pressure washer recoiled injuring the claimant's left shoulder.

WHILE ATTEMPTING TO REMOVE LUGNUT INSTALLATION TOOL FROM LUG ON KOMATSU LOADER, INJURED GRASPED "REACTOR BAR" WITH LEFT HAND & ACCIDENTALLY PULLED TRIGGER ON POWER WRENCH PINNING LEFT INDEX FINGER AGAINST LOADER HUB.

EE was climbing off of haul truck and missed bottom step and fell approximately 1 foot landing on butt.

EMPLOYEE WAS DRIVING HAUL TRUCK NOT EQUIPPED WITH AC IN ANTLER PIT AND SUFFERED FROM MILD HEAT EXHAUSTION. EMPLOYEE WAS GIVEN FLUIDS BY SUPERVISOR AND TRANSPORTED TO LOCAL ER. SHE WAS GIVEN IV FLUIDS AND RELEASED WITHOUT ANY RESTRICTIONS. EMPLOYEE RETURNED TO NEXT SCHEDULED SHIFT.

WHILE LIFTING A PIPE WITH TWO OTHER EMPLOYEES, EMPLOYEE TRIPPED AND STRUCK LEFT KNEE ON THE GROUND. DID NOT QUALIFY AS MSHA REPORTABLE UNTIL 9/23/05 WHEN EMPLOYEE WAS PLACED ON LIGHT DUTY.

When cutting through the rock ejector with a torch, it was bound up and came loose hitting employee in left forearm.

Employee was advised to replace segments and track on D10R Dozer. The old track rolled off, new one was being installed and was positioned so that it could be bolted together, when a sling & hook assembly came loose, hitting him in the face.
