Operating a scraper in a fill, tire went into depression in ground, air ride seat bottomed out jarring operator.

Operating a scraper, ran over a rock in cut area causing air ride seat to bottom out.

A contract production driller moved drill to continue drilling a series of uppers at 1-5204. Driller noticed some loose material starting to dribble from right rib at far end of drill. Driller notified the shift supervisor & was advised to stay out of heading until further notice. The shifter arrived & noted that the right rib had failed beyond the drill.

While pressing the cab mounts pin on an R1600 the bar they were using with the press slipped out while under pressure. Bar slipped out striking EE in left side of mouth. Causing a split in upper and lower lip. 2 Stitches required in upper lip.

Contractor employee was walking on site and had to cross the 2-step bridge over cabling on the ground to eliminate a trip hazard. The area was icy, covered by snow. Employee slipped coming into contact with the sharp edge of the step tearing EE's leather work glove and causing a laceration to the left thumb requiring sutures to close.

Miner was holding ANFO hose while blowing it out. Hose slipped out of hands striking miner in the face and spraying ANFO onto face and eyes. Right eye irritated by ANFO dust.

Employee had opened the front hydraulic tank cover on loader. EE then proceeded to open the next cover back. As EE reached the fully open position EE was not watching and EE's finger was pinched between the 2 covers. This smashed the end of the little finger on left hand almost removing the nail and splitting it. Resulted in 4 stitches and a broken finger bone.

While showering at the end of shift miner felt debris wash into eye. Later during the off hours the eye became irritated and the miner went to the health care professional. The health care professional removed a small piece of metal from the eye and gave miner a prescription for antibiotic drops.

Employee was manipulating control arm from the right to left position on the Stinger Longhole drill. This action resulted in a pulled muscle in right shoulder.

On November 23, 2016, at the start of day shift an underground miner entered the 2-5551 S to retrieve a can of paint. As EE was doing the work place inspection while entering this area, EE noticed a fall of ground and called the supervisor.

Employee was working on fixing the oil dip stick that attaches to the oil pan when the engine cover fell down and struck the employee on the right hand and caused a laceration to the palm portion under the right thumb. This laceration received stitches.

HCN gas alarm activated in acacia reactor in mill building; HCN gas detected outside acacia reactor room with hand-held gas meter. Mill building evacuated and ventilated. Lime water applied to overflow from acacia brought pH up and eliminated HCN generation.

Employee was moving a size 200-oxygen bottle to the cascade pump at the mine rescue station. When EE lifted the oxygen bottle, EE felt a pain in EE's left arm above the elbow. Diagnosed with partially torn tendon.

While adding vent bag dust blew into face and eyes, irritating right eye.

While placing a bolt in the hole miner stumbled and wrist contacted a bolt plate resulting in a laceration.

Employee was leaning over frame to fix an O-ring on bottom of hoist pump and had to stretch to reach the lower bolts. He heard a pop in lower rib, left side and felt a sharp pain.

Mechanic was pulling out the boom safety lock pin (pin was stuck) on an LH204 Loader, after repairing a hydraulic hose. He pinched his right middle finger between the pin and a headlight bracket on the boom.

Employee slipped while holding Jack-Leg drill and the machine fell and smashed his left index finger against the rib.

Miner was pulling a stuck jackleg steel out of the rib. When steel came loose the miner fell backwards striking the edge of a bolt plate with his right elbow, resulting in a laceration.

Miner slipped and fell while turning around. Extended his left arm to break his fall and his hand struck a sharp rock on the low rib, resulting in a laceration to the outside edge of his left hand.

On October 6, 2014 a miner was working in the 5 Alimak cut 5. A six foot drill steel fell out of a skip bucket and impaled his right forearm.

After shoveling loose material mill employee reported that his shoulder hurt and that he had aggravated a pre-existing, non work related injury. Employee placed on light duty pending additional testing.

While taking two nips apart at the 7-4550 load center, in a squatting position with the nips on the ground the employee felt a sharp pain in his right knee.

Employee was drilling an entry round to a stope from inside the #4 Raise-South Side. While drilling, a 3" x 12" x 5' piece of lagen fell from a higher point of the raise and struck him on top of the left foot. The result of the impact was two broken bones on the metatarsal part of the left foot.

Employee was retrieving sectional long hole steel from a drill hole and a section was stuck and when he tried to rattle it loose with the drill hammer, a small shard of steel broke off the collar and embedded in his left upper thigh/groin area. Restricted duty release on 12/21/13.

Employee was adding a rod while drilling uppers and the rod slipped out of his hand and hit his left thumb causing a fracture just below the thumb nail at the joint.

Employee was working in a hot heading and suffered from heat exhaustion. He was taken to the hospital for evaluation and was given fluids and was released to full duty.

A ground fall breaking beyond the anchorage involving the foot wall rib and a portion of the back occurred in the 6-5530 stope. No one was in the area when the failure occurred.

Employee was removing a man basket when it shifted and pinched his right index finger resulting in a small fracture of the tip of the finger and sutures.

Employee was diagnosed with carpal tunnel syndrome and had carpal tunnel procedure done on 5/29/13.

Employee was diagnosed with carpal tunnel syndrome and had carpal tunnel surgery on 03/18/2013.

Employee was changing out a header and was loosening the nuts on a Vic clamp and felt a pop in his left shoulder, which resulted in a sprain of the shoulder.

While driving a haul truck in the ramp the employee hit a rough spot in the roadway that jarred him upward and backward in the seat causing pain between his shoulders.

I was notified at about 1:30 pm that there had been a rib and roof failure in the 4-5728 stope that was found after a blast. The failure broke beyond the anchorage of the bolts.

A 15 foot drop raise was drilled and shot. When it was blasted there was damage to the nose pillar, which in turn eroded into the back of the intersection beyond the anchorage of the ground support, which was noticed on 11/27/12, which was 3 days after the blasting of the drop raise. Upon noticing the failure it was reported to an MSHA inspector on site for a regular inspection.

Employee was in the process of loading explosives and lifted a 50 lb bag of ANFO and felt something pop in his right lower back.

Employee was working on a barge at the tailings pond and a pipe fell on his left hand causing a contusion at the knuckles.

Employee exited the back of the administration building to board the bus to town. He slipped on a thin sheet of ice that had formed in a small puddle on the cement pad outside the building and fell on his right side injuring his right knee.

Employee was dismounting from the back of the water truck and stepped on a small rock in the road and rolled his right ankle causing swelling and a bad sprain.

Employee was bolting with a jack leg when a rock came down from about 5' and struck him on the top of his left foot. At first was thought to be just a bruise and he was given a full release, but after the x-rays were examined by a radiologist 2 days later it was determined there was a minimally displaced fracture of the navicular and a restricted duty release was issued.

During the weekly inspection of the secondary hoist a hydraulic valve, which supplies hydraulics for the raise and lower of the hoist became stuck rendering the hoist inoperable. The valve was repaired and the hoist was returned to operation.

While moving jackleg it fell pinching his thumb between machine and leg.

Employee diagnosed with carpal tunnel syndrome and had surgery 05/09/2012.

When employee stepped on loader to check for hyd leak he twisted knee. He was placed on restricted duty on 5/9/12

Employee got his right thumb caught in the door of a 4-yd loader. His thumb nail was torn off and he received a laceration, which required 2 sutures.

Employee was attempting to unload Drisco pipe from the rack of a tractor. He had his rap wrench on the end of the pipe and the wrench slipped off the pipe and he lost his footing and tried to brace the fall with his left hand resulting in a slight fracture on the distal end of the radius.

Pulled on hose-reel at lub-bay and strained abdomen

Employee was drilling blast holes when a rock came loose and bounced down the wall and hit employees foot at edge of metatarsal guard causing injury to left foot.

While jackleg drilling, air hose came loose and hit him in the face and eye.

Employee was at the surface shop getting his loader ready to go underground. He realized the seat needed to be adjusted and when he lifted the adjustment handle the seat slid back and pinched his left middle finger in the track causing a small laceration and slight tuft fracture of the end of the finger.

Employee was pulling jack leg out of a hole and felt a pop in his left elbow.

Changing a tire on haul truck, employee lowered jack and caught his finger between tire and truck frame. He lost the tip of his middle finger of his left hand.

Employee was hanging pipe in a sill and lost his footing and fell to the ground and sprained his left wrist and dislocated his right little finger when he tried to break the fall.

The operator of a service truck was driving up the ramp in Spiral #4 and ran out of fuel and the Getman truck rolled backward and he turned it into the rib and when he did he hit his head on a light mounted on the canopy which caused a laceration on his forehead requiring 4 staples.

While in the process of removing the quill rod from the rod string and lying the quill rod on the deck, the driller helper's right ring finger became caught between the quill rod and the drill swivel base.

While pulling a jack leg drill steel out of the hole, the drill pinched his hand against the rib.

Employee was collaring a hole with a jack leg drill and lost control of the drill and got his right thumb pinched in the chuck tender, which required sutures.

Employee was bolting and a bolt fell from the back and hit her nose and lacerated her lip, which required sutures.

Employee was re-positioning a jack leg drill during set-up and the drill got away from him and pinched his left middle finger between the drill and the rib causing a laceration on the under side of his finger requiring sutures.

Employee was putting a hydraulic hose back into the hose reel and it went too fast and as he tried to stop it he twisted his back causing muscle spasms in lower right side of back.

The injury result of the leg on a jackleg drill kicking out which caused the leg handle to strike the drill operator in the knee. The drill operator cont. to work through shift and the following shift at the end of which he was limping and said knee was getting stiff. It was recom. that he stay off his feet and see a Dr. the following Monday if no improvement.

Two employees were in the process of moving a long hole drill. The victim got his left index finger pinched between the back of the 2-yd loader and the tow bar. This avulsed the very tip of the finger and fingernail and fractured the tip of the finger.

Employee was unloading bags of cement in the 1-4550. After about 1 1/2 hours he felt a burning sensation on his forearms. He showered and applied cream. On Tuesday the 10th his right arm was irritated upon touch. Right arm was treated for infection by physician.

Miner stepped off bolter facing out and slipped injuring his back. The injury did not become reportable until 4/19/11 when he was placed on restricted duty.

Removed radiator cap and hot cooling fluid sprayed arm. resulting in restricted duty.

Employee was drilling in the 3-4450 S when a rock fell through the wire and struck him in the mouth, chipping his tooth.

Employee was moving utilities. When he was moving vent bag, he tripped and twisted his knee.

Miner was bolting with a jackleg when his wrist started hurting. He handed machine off to his partner. He bolted back and rib after a break and finished his shift. His arm was stiff at the end of the shift when he asked to have it looked at.

The miner observed that water/steam was coming out of the top of the Radiator from under the Radiator cap as well as the overflow on LHD Company #61. The Miner then placed his right hand on top of the cap to tighten it. The cap came off scalding the Miner's right/ left hand and right arm. The miner received 1st & 2nd degree burns to include restricted duty as of 01/10/2011.

Employee is a Mechanic Foreman who reported having pain in his hands, elbows, and shoulders. He was seen by a Physician and diagnosed as having Carpal Tunnel Syndrome as of 12/15/2010.

Employee stated that he was going up the steps leading to the mill control room. The employee stated that he had three point contact when his foot slipped on the first step causing his right foot to come into contact with the side of the stair case itself.

Employee was installing ground support with a jackleg. He was drilling in the springline collaring a hole. He bent down to reposition his leg and when he looked up a rock rattled loose and slid down the drill steel striking him on the right shoulder.

Miner was drilling with a stope drill and was struck in the palm of his right hand by a piece of metal that broke off of the string of drill rods.

While fueling tractor @ fuel bay, EE was walking around to the back of the tractor and tripped on containment berm. He fell into the bumper of the tractor causing a cut on his right forearm.

While Bolting the jack-leg was turned on and the mat hit his arm cutting it. Two staples.

The Mechanic hit his right ring finger with a hammer while installing a Tie Rod end on PU 117.

Jib line caught on top head causing claim shell to elevate up and catch onto the employee's harness lifting employee up and swinging him into side of derrick.

While bolting up heading in 6-5530- material fell from face hitting EE on left side of his head and shoulder.

While pulling rods, emplyee dropped a rod and pinched his finger between the rod and the centralizer. The tip of the left pinky finger was fractured.

The miner received a thermal burn to the right forearm when the packer broke spraying him with hot water. (Over 130 degrees). The miner was tighting the valve when the packer broke sending hot water up the sleeve of his slicker jacket and under the rubber glove.

While cutting string with a utility knife, employee cut his left pinky finger. The finger required sutures.

Employee was bolting the back with a jackleg drill when a small rock fell through the wire and struck his left upper arm. The wound required several sutures and the employee was released to full duty.

While working on a 2 yrd loader, employee bumped the engine cover knocking the engine cover rod out of position, which allowed the cover to fall forcing his head down and he split his lip. Several sutures were needed to repair the laceration.

While cutting open a box with a knife, employee's hand slipped and cut the palm of his other hand. Sutures were required to close the wound and he was released for full duty.

In process of moving the drill back on line the helper stepped forward to get a better look and placed his finger between the dewee dag plate and the footclamp plate on the drill. At the moment the helper did this the drill moved again pinching his finger between the dewee dag plate and the footclamp plate.

Employee bumped his head getting into the van after his shift was over. Several sutures were used to close the wound. He returned to work with a full duty release.

While getting into the back of a mantrip, employee fell landing on his back striking his head against the ground. Employee needed several sutures to close the wound on his head and was placed on restricted duty.

While unloading 6' rockbolts at the 6-5230 laydown, employee lost his balance in trying to steady himself he reached out to the rib and cut his left arm on a rock. Several sutures were used to close the wound.

Employee lost his balance while dismounting his dozer and when he reached over to the rib to regain his balance he cut his wrist on a mat used for ground support on the rib. Several sutures were used to repair the wound.

Employee was working on the hydraulic steering pump on the loader, the compartment cover pin became dislodged and the cover came down pinching employees hand.

Employee was using a scaling bar to free up a clevis on the back of a loader when it slipped causing him to fall to the ground fracturing his leg. Employee is currently off work.

While installing ground support, employee had used a scaling bar to push the wire up to the back and as he drove the bolt in to secure the wire, the scaling bar fell striking him under his right eye. Several sutures were used to close the wound.

While driving a split set bolt with a jackleg drill, a small rock fell from the back striking his upper lip chipping his tooth.

While bolting the back with a jackleg drill, a rock came through the wire and struck employee's forearm slicing it open. The wound required several stables to repair. Employee returned to work full duty.

Employee collapsed while starting a fan that had gone down. Employee had several non occupational medical issues, but it has been determined this could have been related to heat and dehydration. He was released to full duty work.

Employee sliced his left thumb with a knife. The wound was closed with sutures.

A rock fall occured at approxiamately 3:30 am in the Main decline approximately 300 feet from portal. The fall was estimated at 8' height by 8 foot wide by 25 feet long from the rib and back. The size of the fall has continued to grow as miners work on installing additional support required by MSHA to access the fall area.

While rattling a drill steel on long hole drill 102 a metal shard broke off and penetrated operator's forearm.

Employee was bolting with a jackleg drill, when a bolt struck the side of his face when it slipped out of his hand causing a laceration. The wound was repaired with sutures.

Driller completed his run and went to take the quill rod off the rods grabbing it with both hands. Driller then went to set the quill rod on the drill frame but didn't get his right thumb out of the way. Thumb was pinned between the quill rod and the frame causing the skin to break below the first knuckle.

While attempting to remove a stick steel from the striker bar by hitting the steel with a hammer, the striker bar was hit and a small piece of metal struck the employee's thigh. The employee was taken to a medical facility where the metal was removed and he was returned to work full duty.

Two miners were preparing to start bolting the 1-5451 N sill when they noticed the left rib behind them dribbling. When the rib began to shift they pulled out of the area and evenually a slab approximately 3'W x 8'H x 10'L came down. The ground appeared to have failed to a slip.

Suburban Propane Service Technician was repairing a propane dispenser tank. He started to remove meter when pressure in the line caused meter to blow off mount. This caused employee to crush left hand ring finger against tank support.

While barring down unsupported ground, employee was struck in the mouth chipping a tooth when a small rock fell through the wire mesh over his head.

Driller was removing vent and a rock fell through wire and struck him in face. Laceration to nose.

Employee was supplying heading for bolting when he slipped on the wire on the sil and cut his forarm. Sutures were required to repair the wound.

While cutting a piece of metal with a stationary chop saw, the clamp holding the metal in place broke and the employee's hand was lacerated when the piece kicked back. Sutures were needed to repair the injury.

Employee slammed his finger in the door of his loader after opening the door to activate a pull cord. The tip of the middle finger, of the right hand, was fractured.

While working in an electrical switch box, employee tried closing the door, which was held open by a benita clip. The door sprang shut, causing a laceration on his left eye brow, which required several sutures to repair.

While attempting to check the transmision oil level of Loader ULD61, the engine cover slipped out of his hand and struck his left hand, causing a laceration. The wound needed several sutures.

While taking in NT rod apart from HT sub that was attached to a HT rod, the rods separted and the HT rod with sub fell approxmately 4 inches smashing the tip of the right finger between the sub and feed frame of the drill.

While walking to dumster in the reagent area, employee slipped near the floc pumps, twisting his knee.

While mixing cement for back fill, employee developed a rash on his arms and legs. Over time the condition worsened and the doctor placed him on restricted duty on 4/29/08.

Employee twisted his ankle, when he stepped on a rock, while opening the door to his truck. He sought medical attention and was released for work, but he continued to have trouble with the ankle, so he went back and was taken off work.

While driving a bolt in the left arch, employee felt something strike his left forearm and when he looked he had a laceration. Several sutures were needed to close the wound and he was released full duty.

While exiting the mine as the passenger of a utility vehicle, a rock fell from the spring line striking employee's hat hat pushing the hat into his glasses, fracturing his nose and also creating a hairline fracture of the C-1 vertebrae. Employee is currently off work.

While mixing copper sulfate, some of the solution splashed onto employee's face and under his safety glasses into his left eye. He was released full duty. In reviewing the file it was recogonized he was given presciption medication for comfort which makes this reportable.

At the 5-5200 south bench, a fall of ground from the back fell approximate size 10' by 10' by 20' long. The area was not open to personnel only remote equipment. The fall was approximately 178 feet from where personnel had access. A miner thought the rib had failed and reported it. The next morning the mine foreman went to look it over and determined the failure was from the back.

Employee was carrying a bundle of bolts, when he turned, he slipped and turned his knee. Initially released full duty, however he is now scheduled for surgery and has missed days.

While driving a bolt in the back with a jackleg drill, bolt slipped off driver and bolt fell out of hole striking employee's upper lip. 4 sutures were used to repair.

While unloading a mounted 26.5R25 Tire from service vehicle to spare rack, tire & wheel assembly hit the spare rack and came off Bead Hook being used to lift assembly. It fell to ground pushing injured into side of service truck. Resulting in contusion to posterior portion of left shoulder/rib area and fractured ribs.

Two mechanics were traveling on the surface haul road in a kubota buggy. When the operator attemtped to stop, the buggy began to slide in the loose gravel; the passenger reacted by putting his leg out to keep the buggy upright and received a fractured lower leg.

Unloading rods from tractor, rod rolled and smashed finger. Broken right index finger.

Employee was walking down a slope of a mountain and tripped over a piece of sagebrush causing him to trip. Resulting in a sprained left ankle.

Additional ground support was being installed in an area that was in need of it. Two 8' split set bolts had been installed and more holes being drilled. Small material started "dribbling" at the brow so they backed off a safe distance to observe it. After about five minutes the brow & more material fell exposing two 8' swellex bolts that appeard to have been installed at an angle.

Employee was tightening the water swival onto a 10' NT drill rod when his 24" pipe wrench slipped, causing him to hit the drill deck with his right hand, spraining his wrist.

The EE was carrying two roof bolt plated in his hand and received a cut from a piece of metal protruding from one of the plates.

The EE was drilling a hole with a jackleg, when a rock came out of the back through the 4"X4" wire mesh. The small rock hit the EE's hand/finger that he was holding the jackleg with.

The EE was hanging electrical cable and a pull bottle with a cable hanger. The scissor deck truck he was standing in moved, causing the cable to be pulled out of his hand. When this happened, the cable hanger (attached to the cable and pull bottle) cut the palm of his hand.

The worker was moving & dumping precip from a cart in the refinery around 7 a.m. The precip cart (which has 2 full pans on it weighs around 1300 pds) was being moved when the worker pulling on the cart felt something snap in his shoulder.

While helping electrician cut tape off of a pipe on a jumbo, the box knife slipped off the tape & sliced into his finger as he was holding the cable.

On 2/15/2007 an employee reported an incident occurring back in August of 2006 where he hyper-extended his knee and then two more incidents at work and one at home. Employee continued to have problems with knee and eventually had surgery to repair a ligament. Employee lost 4 days and had 9 restricted days.

Employee was jacklegging in the 4-4650 when his safety glasses became dirty. He then took then off to clean. While cleaning them, a rock fell hitting the muffler, which made the rock bounce back striking him in the eye. At the clinc he was prescribed a cream and placed on light duty.

Driller slipped and caught his (r) ankle in gap on deck. (r) ankle sprain.

Employee was attempting to attach a chain around a refuge chamber door to a forklift fork. As the fork was being boomed out his right arm was inadvertantly pinched between the fork & the door.

Employee was checking coolant on SV12. As he jumped down his left foot landed on a rock and twisted his ankle.

EE was collarting a hole with a jackleg. A rock slid down the steel and hit him in the chest causing a laceration that needed three stitches.

EE and two other ees were riding in a pick up when a rock came thru the window of the pick up. Striking ee on the cheek.

The jumbo operator leaned a 5' x 10' wire up against the rib to paint it for bolting. He had the wire too vertical and when he walked away to get another wire, the wire fell forward and struck him on the arm causing laceration/puncture to his left arm elbow.

The employee leaned a cume-along against the rib. He thought it was secure, When he walked away, the cume-along fell and struck him on the right shoulder.

The EE was in a hurry and was stepping back into his six yard loader he was operating. His muddy boot slipped off the step and he hit his knee on the edge of the cab.

Employee was walking to his tool box and stepped on some grease. He slipped, caught himself and twisted his back. He was taken to the doctor and was placed on restricted duty.

While scaling his heading a rock fell off the rib and struck the scaling bar causing the bar to hit his leg just above the knee.

Employee was operating a 30 ton haul truck. He hit a bump in the road which caused his cap lamp to come off the bracket on his hard hat. When his cap lamp came off it swung down in front of his face. It struck a front tooth chipping the back side.

The employee was drilling a hole with a jackleg. The steel abruptly stopped turning causing the jackleg & hoses to rotate as the machine (Jackleg)rotated the employee was still holding on to it. This caused him to strain his shoulder.

7/11/06 employee reported to her supervisor that her knee had been hurting for several days. She beleived pain was result of twising & turning at work. The next day, 7/12/06, she went to the doctor and was placed on restricted duty.

A fall of ground occurred in the 4-5 450 heading where roof bolts were in use. This was above the anchorage point of the bolts. It is not know when this occurred, however, upon discovery it was immediately reported to MSHA and a 103k order was issued.

Miner was walking down 5 north when he felt a shootingpain in his lower back. The miner had picked up a prill pot earlier in the day and thought his back was reacting to that. He was taken to the doctor adn was placed on restricted duty.

Ee was removing a bolt from the ware plate on the bucket off the 988 loader. The bolt started to spin, so he tried to hold it with his hand. The bolt soun cutting through his glove and into his hand. He was taken to the medical facility, received stitches and released to full duty.

EE was operating a Getman utility truck. EE was taking supplies to a heading down off of 3 haulage. The driver tried to avoid hitting 2 bumps in the road. The truck bounced up and down, EE had his seat belt on, and pulled the muscle in his lower back.

While mounting a 950X20 tire, picked up tire to rotate and install on equipment, lifted to mount Right Frt tire, felt a pop in back, and could not bend over afterward.

EE has been diagnosed with silicosis. A conclusive diagnosis was given on 4/10/2006 by Dr. Cahill at University of Utah, pulminary clinic.

While fueling loader, he stepped off and slipped on the frame of the loader, causing his ring and pinky fingers on his right hand to bend backwards. It was not reported until the next day. He was taken to the doctor and placed on restricted duty.

HELPER WAS PUTTING SOME CORE AWAY, HAD TURNED AROUND & WHILE HIS BACK WAS TURNED SOME RODS THAT WERE STACKED UP FELL, STRIKING HIM IN HIS RIGHT RIB CAGE.

Employee was bolting in the 4-4850. Employee used a scaling bar with a bearing plate to hold up the side of the wire in order to drill a hole. The scaling bar kicked loose and the end with the bearing plate hit his forearm.

While working on drill around 9:30 pm I felt a pain in my back (upper left side). I continued to work but after awhile I could not lift even an empty tube. The pain had started to go down my left arm & halfway up my neck. I informed driller & we went to hospital.

Employee was going to dump a bucket of muck into the muck bay with a 2 yard LHD. The forward pedal stuck causing the mucker to keep going forward. The employee struck his elbow on the cab. The mining company did not know of any light duty from the employee until 1-26-06.

Employee was drilling a hole with a jackleg drill. When pulling the drill steel and machine out, the jackleg came off the drill steel which made the drill pivot down and smashed the employee's finger.

EE slipped on a muddy rock, to stop his fall he put his hand out to brace himself against the rib. The EE jammed his finger in the action of stopping his fall. The EE did not report the accident until 1-16-2006 then he sought care after that.

EMPLOYEE WAS HOLDING A PIPE WRENCH WITH HIS LEFT HAND PLACED ON A D-D NUT IN AN ATTEMPT TO LOOSEN IT BY SWINGING A SLEDGE HAMMER WITH HIS RT.HAND. ON HIS 3RD ATTEMPT HIS SWING MISSED THE PIPE WRENCH, HITTING HIS LEFT THUMB.

THE EMPLOYEE HAD JUST FINISHED CLEANING THE CONVEYOR BELT & STARTED TO STEP BACKDOWN TO CATWALK WHEN HE LOST HIS BALANCE AND TWISTED HIS ANKLE. HE WAS TAKEN TO THE DOCTOR AND PLACED ON RESTRICTED DUTY.

The ee was operating the 2 yard lhd in reverse. The 2 yard lhd hit a hole in the road which caused the lhd to bounce. He bounced striking his head on the remote box that was mounted in the operator's compatment.

THE EMPLOYEE THOUGHT THERE WAS AN EYELASH IN HIS LEFT EYE. HE RUBBED IT. THE IRRITATION DID NOT GO AWAY. HE WENT TO THE DOCTOR TO HAVE IT LOOKED AT AND A PIECE OF METAL WAS REMOVED. HE WAS GIVEN A PRESCRIPTION FOR ANTI-BIOTICS AND RELEASED TO FULL DUTY.

EMPLOYEE WAS IN THE PROCESS OF MAPPING A BLAST AGAINST BACKFILL IN THE 1-49545. EE CHECKING DRILL HOLES & HE GOT DUST PARTICLES IN HIS RT. EYE.

Employee was back dragging in spiral 3-4750 sill. He was finishing ramping the sill when boulders from top of the muck pile rolled underneath his bucket which caused the mucker to bounce up and sideways. Employee's head hit the top of the FOPS/ROPS or the remote box causing pain in his neck.

Employee was rehanging some ventilation in 5 haulage. His partner lifted the vent bag and some dust fell off the bag and went into his eye. The employee was wearing prescription safety glasses while working on the vent bag.

On 10/14/05 the supervisor was made aware that there had been a ground failure in teh 4-5630 S heading.He and mine engineer went to heading to determine what had happened. Material in the unsupported sill had fallen and coked off at the brow. Unable to determine if it was abouve the anchorage point of the bolts until it could be mucked out to expose the brow.

Employee was lifting a 40 lbs bag of anfo to dump into a prell pot. He was on his 4th bag when he felt a constant pressure and discomfort in his abdomen.

Employee was assisting another miner to unload prell pot out of the back of a pick up. The employee was getting out of the back of the truck, squated down and lost his balance. Employee tried to get his balance back by putting his foot out in the process twisted his foot wrong.

Employee was cleaning up oil pads from under a 4 yard LHD, foot slipped when he was standing up - causing him to twist back and catching himself.

Employee was helping to move some old core boxes off of the top shelf, he took a step back & lost his balance. His right foot rolled underneath his right ankle. He returned to work the following day & the client sent him home because they did not have light duty work for him. Client said that he would need a doctor's release for full duty. Drug test results: Negative.

EMPLOYER WAS A DRILL HELPER ON OUR U8 CORE RIG. THEY WERE LOOKING FOR THE GRINDER & EMPLOYEE STEPPED OFF OF THE BACK OF THE MAN BASKET ON THE SKY TRACKER. WHEN HE DID, HE CAUGHT HIS CLOTHING ON METAL CAUSING A LACERATION.

EE WAS TIGHTENING A ROD WITH A 36" PIPE WRENCH. HE SOMEHOW HIT THE ROTATION LEVER IETHER FELL INTO IT OR HIT IT WITH THE PIPE WRENCH CAUSING HIS HAND TO GET PINNED BETWEEN THE WRENCH AND THE DRILL HEAD MOTOER

EMPLOYEE WAS OPERATING A HAUL TRUCK UNDERGROUND. THE TRUCK HIT A HOLE IN THE ROAD CAUSING THE TRUCK TO BOUNCE. THE OPERATOR OF THE TRUCK STRUCK THE RIGHT RIB AFTER THE TRUCK BOUNCED OUT OF THE HOLE.

EE was rock bolting on a ramped muckpile ee started to walk down ramp, slipped and twisted his knee.

Employee turned off air and water valve and stepped back onto roadway and caught a rock with the heel of his boot, slipped and twisted his right ankle.

Employee was working in the 6-5430 Sill, operating a 4 yard 006 scoop tram. The scoop tram began to overheat. The employee proceeded to an intersection. At some point he began to cool the unit down with water. The radiator cap blew off, dispencing hot antifreeze.

EMPLOYEE WAS TRAMMING LHD TO HIS WORK PLACE WHEN THE OVERHEAT LIGHT CAME ON. HE PULLED THE LHD INTO A CROSS CUT TO COOL OFF. HE PARKED THE LHD AND SHUT THE ENGINE OFF. THE RADIATOR CAP BLEW OFF CAUSING ANTIFREEZE TO COME OUT OF THE RADIATOR. HE THREW UP HIS HAND TO BLOCK HIS FACE FROM GETTING SPLASHED WITH ANTIFREEZE.

Employee had been re-blasting some miss-fired holes for about 4 hours. Heading conditions were hot; he had been drinking from his water jug the entire time. After completing his task, he was sent to setup and rock bolt the 4-5580. He was repairing air hoses to do this task when he felt dizzy. While pulling on the air hose and he fell to the ground striking his right cheek.

Driller's right middle finger pinched between centralizer and guide shell while connecting drill rods.

EE WAS OPERATING A 936 CATERPILLAR LOADER WITH FORKLIFT ATTACHMENTS, UNLOADING 2 UNITS OF GALVANIZED WIRE MATS OFF OF A DELIVERY TRUCK. EE STATED HE REVERSED TO CLEAR THE TRAILER. HE HIT A RUT OR BUMP, CAUSING THE REAR OF THE FORKLIFT TO COME UP IN THE BACK AND THE FORKS DOWN. EE STATED THAT THE SEAT BELT CAME UNDONE.

Operator was removing brick from a furnace. On raising his faceshield particles fell across his forehead, bounced off glasses into right eye. Eye was flushed & operator transported to town. Seen by doctor. Eyedrops prescribed for scratch on the sclera. Temporary irritation. Miner returned next shift.

EE injured his back while attempting to move a fender that had been knocked off a truck. He continued to have pain and was taken to medical clinic on 4/4/05. He was diagnosed with back spasms and was given prescription muscle relaxers and was teaken off work for 2 days.

Miner was unloading wire mat off flatbed. A wire caught on the bundle and when he released it, the wire slid off the truck and landed on his foot. He received a puncture wound. He was given antibiotics by the doctor the next day and was given a full duty work release for his next shift (3/25/05).

While replacing a seat on a 6 yard loader, the base caught on some hoses forcing the seat down with employee's thumb under it. The mechanic was seen at the clinic and was given restricted duty dut to a sprained thumb.

Employee was in the act of removing an o-ring from a check valve with an o-ring pick. The pick slipped & punctured his rt. middle finger tip. Treated w/first aid on 3/14/05. EE went to the Dr.'s office & was treated for an infection on his rt. hand, middle finger tip. He received a prescription for antibiotics.

While fabricating a roof for structure, the frame for the roof slipped. It caught the index finger pulling off the nail. First aid administered and he was taken to town. No fracture but antibiotics prescribed.

Employee was rock bolting using a jacklet. During the course of the day, he noticed pain in his stomach that continued to worsen. He had been diagnosed in August of 2004 with a hernia but felt that it was not work-related. He had postponed corrective surgery until after the first of the year. He had surgery 2/1/05 and is off work.

Stepped off drill deck sprained rt ankle.

EE dismounted loader for getting service maintenance. as he was walking across heading, his left ankle gave out and he fell. he continued working remainder of shift. the following day the ankle swelled up and he went in for medical evaluation. He was placed on restricted duty. he previously injured ankle 1/19/05 by.

Employee stepped on a short piece (less than one foot) of nine wire while walking in sill to his mucker. The nine wire perforated his work boot and punctured his left foot. The wound was flushed on site and he was transported to town where he received a tetanus shot and was given a one week perscription of antibiotics. He received a full duty release.

WHILE PLACING/DRILLING GROUND SUPPORT BOLTS AND WIRE THE INJURED PULLED THE LEG OF HIS JACKLEG DRILL BACKWARD AND CAUGHT HIS LEFT ARM ON A PIECE OF THE GROUND SUPPORT WIRE. THE INJURED RECEIVED A LACERATION THAT REQUIRED TEN SUTURES TO CLOSE THE WOUND.

THE CRUSHER (CONE) PLUGGED. AFTER L.O.T.O. THE OPERATOR WAS USING A BAR TO DISLODGE THE PLUG. HE NOTICED THAT HIS HAND WAS STINGING. HE TOOK OFF THE GLOVE AND SAW THE INJURY. DOESN'T KNOW HOW IT HAPPENED. (HIT BY ROCK OR THE BAR.) TAKEN TO TOWN FOR FIRST AID. X-RAY'S FOUND FINGER TO BE FRACTURED.

At some points during shift, the employee was bitten by an unknown insect. He noticed it at approx. 10:50 but continued to work. At around 1:00 pm, he noticed the burning and swelling had spread up and down his arm so he reported it. He was evaluated and transported into town, given antibiotics and placed on restricted work duty until swelling went down and reevaluted.

EE WAS USING PST TO CLEAN SHOWER STALL. WHEN SCRUBBING, A SMALL AMOUNT FLEW INTO HIS EYE. (EE WAS NOT WEARING SAFETY GLASSES.)

Ee was attempting to remove brake cover from 40 ton truck. He removed all bolts but 2. He supported cover on his knee then removed last 2 bolts. He put his hand on inner side of ring & moved his knee. When he moved his knee the ring fell thus smashing his left hand between ring & spindle. Resulting in broken middle finger & ring finger. Middle finger also required 8 stitches.

Claims nose and throat burning after being exposed to dust from blast.
