EMPLOYEE WAS HELPING ANOTHER EMPLOYEE CARRY A SCREEN DOWN A RAMP. HE FELT DISCOMFORT IN HIS GROIN AREA LATER THAT EVENING.

EE WAS TOLD TO GO TO THE PRIMARY CRUSHER TO CHEKC ON SOMETHING. HE CAME BACK AND STATED TO HIS SUPERVISORS THAT HIS KNEE WAS SWOLLEN. HE STATED THAT HE DID NOT DO ANYTHING TO CAUSE THIS INJURY . EE HAD KNEE OPERATION 2/28/92.

WHILE TRYING TO STRAIGHTEN SHEET METAL HE SLIPPED & FELL ON H IS BACK PLEASE BE ADVISED THAT CLMT DID NOT REPORT THE ACCIDENT AT THE TIME OF ITS OCCURR ENCT HIS LAST DAY OF WORK WAS 020488 W HEN HE TRIED TO WORK & COULD NOT ON 021288 HE STOPPED IN TO MAKE OUT AN ACCIDENT REPORT WITH ONE OF THE SUPERVISORS HE RETURNED TO WORK ON 022988
