Employee stated that after lifting screen, felt pain in right bicep area.

Employee bent over to lift a chain from the ground when ee felt a pull on right side-groin area. EE was diagnosed with a groin sprain.

Employee's right wrist struck a hot knife tool that a fellow employee was holding causing a laceration requiring 3 stitches.

Employee was handling a 5 gallon pail of stone when he felt pain in his right elbow. He later developed an infection in his right arm.

Employee was walking down stairs, He began to fall and reached for hand rail. His falling weight caused him to strain his right triceps.

Employee fell approx 18 inches when he lost his balance when climbing on to equipment. He hit the back of his head on the ground causing a contusion.

Employee stated that on 3/6/12 a rubber Main Frame Liner fell against his left calf. He was sent to a medical facility on 3/12/12 when he developed swelling. The diagnosis was a contusion.

Employee complained of right wrist pain from operating joy stick on 992 loader, on 2/26/12. Diagnosis 5/18/12, Operation 6/6/12. No lost time or restrictions until 6/6/12.

Employee was shovelling stone when he strained his lower back.

Employee was cleaning a shaker chute with a 20 pound chipping hammer, when the hammer broke thru encrusted material, he was pitched foward and felt lower back pain.

Employee was using a utility knife to cut a plastic bottle. The knife slipped and cut employee's right wrist - 3 stitches plus tetanus shot.

Employee was removing bolts on a surge tank cover using an air impact gun. When a bolt broke, the gun fell out of his right hand and struck his left index finger, causing a laceration and contusion.

Employee was removing a broken feeder plate and after the last bolt was removed, the plate released, striking employee on the back of his hard hat.

EE was using a wire wheel grinder. He dropped the tool and the spinning wire brush struck him on his left thigh, causing an abrasion to his skin.

EE states that he had pain on his left elbow for some time from shifting gears in front end loader.

Employee stated he developed pain in his left elbow from operating equipment over time.

Employee was lifting a generator trailer tongue to connect it to a vehicle and he felt a pull in his left elbow.

Employee was cutting brush with a machete, when he struck himself on the right shin with the blade, causing laceration - 3 stitches.

Employee was holding a wrench and turned his body slightly when he felt a pain in his lower back. He was not apply torque to the wrench. He was diagnosed with a thoratcic sprain.

Employee was assisting in the installation of an adjustment ring on #4 crusher. The over-head crane was trolling the piece into position when employee's arm was pinched between the ring and the structural I-Beam.

Employee was lifting shaker screen into position when he strained his lower back.

Employee was shoveling and lifting screens throughout the day. He noticed mild pain in his abdomen and a small bubble near his navel. He was diagnosed to have a hernia.

Employee was unloading a 20' piece of drill casing from the back of a truck when his left index finger was caught between the pipe and a rock on the ground causing a laceration requiring 2 stitches.

EMPLOYEE WAS MOVING A BULL SCREEN INTO POSITION USING A COME-ALONG. THE SCREEN PINCHED HIS RIGHT PINKY BETWEEN I-BEAM AND THE COME-ALONG HOOK.

WHILE ATTEMPTING TO LIFT A 226LB BEARING FROM THE CAB OF THE DELIVERY TRUCK, THE EMPLOYEE STRAINED HIMSELF.

WHILE LOWERING A CORE CHUTE ON TO A CONVEYER BELT, THE CHUTE SLIPPED PINCHING EMPLOYEE'S HAND BETWEEN CHUTE AND CONVEYER SIDE RAIL CAUSING SPRAIN AND BRUISE.

EMPLOYEE WAS PUSHING A SCREEN OUT OF A SHAKER WHEN A PIN ROLLED DOWN THE SCREEN STRIKING EMPLOYEE IN MOUTH.

WHILE WALKING AROUND LOADER DURING PRE-TRIP INSPECTION, EMPLOYEE SLIPPED ON ICE.

EE WAS COMMING DOWN STD. TOWER STEPS AND SLIPPED ON ICE 3 STEPS FROM BOTTOM AND STRUCK HIS TAIL BONE ON REMAINING STEPS.

EMPLOYEE STATED HE TRIPPED ON A ROCK.

WHILE PUTTING ON WORK CLOTHES, EMPLOYEE FELT SOMETHING ENTER EYE.
