Injured employee was removing bolts from a belt frame on a crusher screen. He had his arm underneath the frame piece when the final bolt was removed the frame piece fell pinching his arm between the piece and the box. He went to the emergency room and was given a prescription and released back to full duty.

Cleaning sand from under conveyors w/ a telehandler. Employee turned to dump out the material in a pile. His right front dropped into a soft spot and the telehandler flipped over onto its side.

WHILE VISITING WITH THE SUPERVISOR, THE INJURED BELT FAINT, AND BECAME DISORIENTATED SUPERVISOR LAID HIM DOWN ON THE GROUND, LOOSENED HIS CLOTHING & COVERED HIM AMBULANCE WAS CALLED JUST TO BE SAFE. EE TAKEN TO HOSPITAL STAY OVERNIGHT- RELEASED THE FOLLOWING DAY FULL WORK STATUS RESTORED AS THEY FOUND NOTHING WRONG
