EE was using the drive belts to rotate log washer tubes for maintenance on a cold day, EE's glove caught the drive belt and EE's finger was pinched between the drive belt and the motor sheave. X-rays at med center on 3/4/25 showed a broken left index finger.

Miner was performing maintenance on a hydraulic excavator. A pin was being driven out of the end bearing of a hydraulic cylinder. As the pin was pushed through the cylinder bearing the bar used to push the pin was forced up by the weight of the cylinder and struck the miner in the mouth. Miner sustained injury to lower lip and a tooth.
While removing bolts from a worn cutting edge the employee attempted to keep a bolt head from spinning and the sharp edge of the worn bolt cause the injury. The employee sustained lacerations to two finger on right hand. The employee was taken to a local medical facility and received sutures in one of the two fingers. The other finger was treated using first aid only.

Miner was operating an articulating haul truck when the park brake was applied unexpectedly. The truck came to an abrupt halt. The operator was wearing a safety belt but the sudden stop "jarred" their back.
Employee was servicing a Cat 307 Excavator in the shop. The employee had stepped down from the machine to get a tool and was climbing back up to remove an access cover from the hydraulic tank when hand slipped off the rail and EE fell backwards, injuring left elbow and left shoulder.
Employee was working on the North Processing Plant. Two employees were working together, one was inside the tub of a sand screw replacing wear shoes and the other (the injured employee) was outside the machine assisting when IE bumped head on a low I-beam support.

Haul Truck operator was attempting to service the air conditioning unit in the truck's cab when the middle finger on his left hand contacted the fan blade of the circulating blower. Tip of finger was cut requiring three (3) stitches to close wound.

Operator was cleaning accumulated material near the feeder pan of the processing plant when he pinched his right index finger. The tip of the finger bone was broken and a laceration required stitches to close.

EE WAS CLOSING THE DOOR ON THE CAT 245 WHEN HE LOST HIS BALANCE AND GRABBED FOR THE HANDRAIL BEHIND THE DOOR WHEN HE GOT HOLD OF THE OPEN SIDE PANEL DOOR. HAVING TWO MOVING DOORS, HE LOST HIS BALANCE COMPLETELY AND FELL TO THE GROUND.
EMPLOYEE WAS REMOVING A SCREEN FROM THE TOP OF SAND PLANT WHEN HE PUSHED THE SCREEN OVER HIS HEAD & TURNED AT THE SAME TIME.

OPERATOR WAS ATTEMPTING TO UNROLL CONVEYOR BELTING FROM A SUSPENDED ROLL WHEN THE ROLL CAME UNHOOKED AND FELL ON HIS FOOT. THE ROLL OF BELT HAD AN ELONGATED CENTER HOLE AS IT DID NOT HAVE A CO RE. IT WAS SUSPENDED BY CHAIN AND A BAR THRU THE ROLL. AS THE BELT ROLLED IN SPUN/JUMPED SUDDENLY AND DISLODGED. THE OPERATOR'S FOOT WAS BRUISED BUT NOT BROKEN.

EMPLOYEE WAS ON A LADDER APPROX 3' ABOVE GROUND. MAINTENANCE WORK WAS BEING PERFORMED ON A GRAVEL CHUTE. THE EMPLOYEE REACHED OUT AND THE LADDER SHIFTED. IN THE PROCESS OF STABALIZING THE LADD ER AND HIMSELF THE EMPLOYEE STRAINED HIS LOWER BACK. THE EMPLOYEE COMPLETED HIS SHIFT AND THE BACK STIFFENED UP OVERNIGHT!

TWO MINERS WERE REMOVING A BROKEN CABLE ON A DRAGLINE BUCKET. THE BUCKET AND LINAGE WERE LOCATED ON LEVEL GROUND IN COOL WEATHER CONDITIONS. A CABLE WEDGE WAS BEING DRIVEN OUT OF THE PULL BAIL SOCKET IN WITH A PIN & 10# HAMMER. THE INJURED MINER WAS STRUCK ABOVE THE RIGHT EYE WITH THE HAMMER AS HE LEANED IN TO CHECK THE WEDGE AT THE SAME TIME THE HAMMER GLANCED OFF THE PIN.
