EE was replacing a relay for the fuel pump truck. EE tried to drill out the new electrical terminal to fit on the existing stud using the drill press. The bit grabbed and spun the terminal, pulling EE's hand into the bit, causing a fracture and a laceration on the tip of EE's left index finger. EE received 12 sutures.

EE was installing U-bolt in a bin truck. The jack shifted and struck the ee in the forehead, causing a laceration that needed 11 sutures.
EE's were replacing blow bars inside of a crusher. When the EE cut the steel bands on the skid, the top blow bars slid off and landed on EE's left foot. Two other EE's helped lift the bar off. EE got an X-ray and had no broken bones, only bruising.

EE was doing maintenance at the finish plant screens. EE bent down to pick up a piece of angle iron and felt a pinch in back and shooting pain down the leg.

Employee tripped while ascending the screen tower steps. When he caught himself he felt pain in lower back. The employee did not seek medical attention until 03/18/2009 and that is why the later entry. The accident was reported but at the time of the report was not MSHA reportable.

While preparing to assist in removing chunks of ice from a flat conveyor discharge chute, the employee climbed three steps to the base of a conveyor catwalk, turned around, lost his footing and fell face-forward to the ground. He subsequently jammed his shoulder when he put his right arm out to break his fall.

THE RIGHT REAR SIDE OF THE EMPLOYEE'S HAUL TRUCK HAD SUNK IN A SOFT SPOT ON THE UPPER ROADWAY. WHILE EMPLOYEE WAS DISMOUNTING THE ACCESS LADDER ON THE FRONT OF THE VEHICLE, HE MISJUDGED THE E XCESSIVE DISTANCE BETWEEN THE BOTTOM RUNG AND THE GROUND, CAUSING HIM TO LOSE HIS BALANCE AND FALL ONTO THE SIDE OF HIS LEG.
WHILE CHECKING THE AMOUNT OF MATERIAL IN HIS BIN TRUCK, THE EE SLIPPED OFF OF THE DUMP LADDER AND STRUCK THE TRUCK'S BATTERY BOX.
