EE was doing maintenance at the finish plant screens. EE bent down to pick up a piece of angle iron and felt a pinch in back and shooting pain down the leg.

EE was diagnosed with Carpal tunnel syndrome resulting from repetition of tasks over numerous years

EE was shoveling snow off of some steel plates at the Denver Quarry and felt stiffness in neck resulting in restrictions making them off work.

Operator was welding lugs on crusher foot slipped and right knee struck edge of sheet metal

The employee was struck on the left arm by a piece of steel while hammering on a loader bucket tooth with a steel hammer during maintenance.

Employee was removing a return roller when EE's left middle finger got cut requiring several sutures.

Employee was using a pocket knife to remove an o-ring seal and the blade closed on EE's finger causing a laceration requiring sutures.

Employee was stemming finished bore hole and struck a piece of frozen stemming. Employee felt a pop in right forearm.

As the EE was traveling from his car to the shop he slipped on ice, spraining/straining his lower back. This was not reported until now because EE didn't seek medical attention until 2/4/2013.

While performing maintenance EE knelt down on catwalk and knelt on a stone that was lodged in the expanded metal, causing a bruise on the L knee.

Employee experienced a standard treshold shift (hearing loss).

Employee tripped while ascending the screen tower steps. When he caught himself he felt pain in lower back. The employee did not seek medical attention until 03/18/2009 and that is why the later entry. The accident was reported but at the time of the report was not MSHA reportable.

As the employee was using a hammer to loosen a tooth from a loader bucket, a sliver of metal from the tooth chipped off and became imbedded into the employee's hand.

AS THE EMPLOYEE WAS PUTTING A SCREEN PATCH ON THE TOP DECK FINISH SCREEN AT THE SECONDARY PLANT, THE EE LOOKED UPWARD TO TIGHTEN BOLTS WHEN STONE CHIPS FELL & STRUCK HIS LEFT EYE.

While preparing to assist in removing chunks of ice from a flat conveyor discharge chute, the employee climbed three steps to the base of a conveyor catwalk, turned around, lost his footing and fell face-forward to the ground. He subsequently jammed his shoulder when he put his right arm out to break his fall.

THE RIGHT REAR SIDE OF THE EMPLOYEE'S HAUL TRUCK HAD SUNK IN A SOFT SPOT ON THE UPPER ROADWAY. WHILE EMPLOYEE WAS DISMOUNTING THE ACCESS LADDER ON THE FRONT OF THE VEHICLE, HE MISJUDGED THE E XCESSIVE DISTANCE BETWEEN THE BOTTOM RUNG AND THE GROUND, CAUSING HIM TO LOSE HIS BALANCE AND FALL ONTO THE SIDE OF HIS LEG.

WHILE CHECKING THE AMOUNT OF MATERIAL IN HIS BIN TRUCK, THE EE SLIPPED OFF OF THE DUMP LADDER AND STRUCK THE TRUCK'S BATTERY BOX.
