Throwing dust collector bags into the rear of the suck truck the employee felt a pop in right shoulder. Employee started treatment with our online Teladoc with no change in pain or discomfort. Employee was advised to seek medical attention with a clinic. Right shoulder strain.

Employee was cleaning material off 117 conveyor belt when they felt a pull in is abdomen/groin they reported this to supervisor and denied care at this time. The employee reported to safety on the 27May2025 they were still experiencing pain and we called care and EE was given a home care plan with no relief they were scheduled a clinic visit for further treatment.

Employee injured right shoulder while install a piece of screw conveyor during the second shift on 1/27/25. EE reported it to the supervisor on 1/28/2025 at 10:30 am. EE called Teladoc and was referred to see a clinical doctor.

Employee got dust in is eye on 1-17-2025 and over the weekend the employee had medical treatment which changed the treatment process to a medical to a lost time incident.

Employee was removing a new fine screw from the rear of a pickup truck into the freight elevator in plant 3 group 2 so they could replace the broken screw, in the process the employee pinched A thumb between the flight of the screw and the frame of the elevator resulting in the right thumb fingernail catching being torn off. the employee was treated at the urgent care.

Employee shoveling frozen stemming stone into loaded bore holes. Felt a pop in shoulder and is experiencing pain.

While greasing the roller in the finish mill the lube tech slipped on loose gravel, they caught themselves, straining their left shoulder/bicep area.

Employee tripped over a parking block and landed awkwardly on EE's right leg straining EE's hamstring.

Employee pinched middle finger between a chain and sprocket resulting in a laceration to the middle finger resulting in stitches and a crush injury to the left ring finger resulting in a fracture to the tip of the finger.

Employee was gouging when employee felt debris in right eye.

Employee was stepping down off the haul truck and foot hit the ground sooner than expected and felt a sharp pain in left lower back, also had numbness in left lower shin and foot.

Employee was moving a tower mounted hoist when employee felt pain in right heel. The employee stopped right away and reported the injury to supervisor, they contacted work care for the initial treatment. Employee was not feeling any better and was advised to have a clinical visit.

Employee was tossing a rake into a pond to clean algae which resulted in a muscle strain or possible tear. The employee is still awaiting MRI and results to have confirmation of type of injury. Employee has not been prescribed any medications but does have restrictions.

Stray hot metal from welding contacted work boot of employee and resulted in a burn injury to EE's left foot.

Employee was shoveling debris in the raw mill when EE twisted improperly and strained EE's lower back.

Employee felt pain in the right arm while jack hammering in plant 3 raw mill.

An employee became trapped in the Finish Mill elevator at approx. 03:00 am while going to the 3rd floor. The elevator stalled between the 1st and 2nd floor. Otis elevator was contacted and assisted in removing the uninjured employee. The third floor door contact was the issue per Otis.

At approx. 730 a contractor became trapped in a elevator. Otis was called and assisted removing the uninjured contractor from the elevator. the Otis tech advised us the first floor door contact was not making good contact with the door sensor causing the elevator to shutdown.

While cutting gaskets for a heat exchanger, the utility knife blade slipped off the gasket, slicing through the glove and causing a laceration across the employee's left ring finger.

Two outside contractors were using the elevator to access the top of the 314 bucket elevator. At 4:45 PM they were taking the "Silver Bullet" elevator down to get some tools and it stopped between floors. Otis repair was called at that time. Otis arrived on site at 6:22 PM and the two men were released at approximately 6:45 PM.

1) The area that the employee was working in was dusty. PPE does not help that. 2) A cover on the back of a small electrical motor was removed, exposing a shaft. That is what caught the employee's pant leg.

The employee was cleaning up spillage from a conveyor. Some of that material penetrated through pants, socks and then onto the skin of EE's ankles, thereby causing irritation.

The incident happened on 6/14/21 and turned into a reportable incident on 6/17/21. The employee was using a torch and cutting above shoulder height above EE when a spark entered EE's right ear.

The employee was assisting on getting the grinding aid pumps flowing and was turning around to walk back to the control room when EE tripped over a short stool that was left in the walkway falling and catching EE's right leg between the stool and the frame to the grinding aid pumps.

The employee attempted to move an empty 55 gallon drum from inside a building to outside by picking it up and carrying it. As employee was carrying it employee felt a pain in their back.

Employee was replacing door on DC when left side fell and smashed EE's left middle finger against handrail causing 6 stitches.

Employee was stooped under a beam to do work. When EE stood up quickly EE struck EE's head on the beam and strained EE's neck.

Employees trapped in aboveground elevator.

A come-a-long was being used to attach a bucket to the structure to prevent movement when it shifted and the hook came off striking the employee in the hand. The impact caused a laceration requiring 5 stitches between the pinky and ring finger on the employee's right hand.

Employee was using a come-a-long and pry bar to get a pin back into place and after pushing and pulling with the pry bar employee felt a pop in lower back.

Dragging 2" discharge hose loading bulk explosives

The employee was removing a silencer off of a dust collector fan with a coworker and as they were moving it out of the work area the employee took 2 steps backwards to the left and hit left leg on the adjacent pipe that was connected to the dust collector motor. The incident took place at 11:30 AM on Monday and was only reported on Tuesday morning to the employee's supervisor.

Employee was removing a drag lid and pinched thumb resulting in stitches. Employee continues to work.

Employee suffered an amputation below the knee while cleaning near the fine screw in the Grinding Facility.

Employee was attempting to unclog a supply line. After turning off air they removed one end allowing material to blow into their eyes. This caused abrasions to the eye.

Fork fell on left big toe while adjusting causing injury.

Employees were using elevator to take tools to top of the 309 silo. Governor switch tripped in control panel and had to be reset by Otis elevator. The car was not able to move while switch was tripped.

Employee came into contact with epoxy resin being used to repair cracks in concrete silo walls. Over a period of time employees' neck developed a rash and eyes became puffy/swollen.

EE received laceration and bruise from jack resulting in NO LOST Days.

Employee had been kneeling and felt a pop in left knee when ee stood up.

Employee was waiting to begin work when the employee suffered a heart attack.

An employee was exiting a work area when the SST hose struck EE in the back of the legs causing EE to fall on left side. The employee injured left ankle.

While opening a silo gate the ee felt a strain in ee's lower back.

Employee was lifting a heavy metal plate and experienced pain in the right side of lower back and down right leg.

EE was cleaning in pack house basement. While EE was dumping material EE tripped causing a strain to EE's left groin area.

Employee was walking on a catwalk and struck a piece of metal, cutting employee's pants and leg.

EE was helping lift a box of floor sweeping compound when EE felt a strain in the right shoulder.

EE left ankle gave out while walking. This resulted in a sprain of the ankle.

Employee twisted knee while servicing truck.

EE was walking from shop to roll press and strained calf.

A choke occurred in a kiln discharge chute. The injured employee was seen walking unexpectedly into the area housing the discharge end of the pipe. The employee later observed the injured employee was covered in dust material. Further evaluation revealed the injured employee had burns on hands, feet, torso and legs. The injured employee was transported to the hospital.

Employee was exiting the finish mill building went through dust curtain, started walking downstairs lost footing and landed on ankle spraining it.

EE was welding in the reject bin when a piece of slag fell behind EE's safety glasses and landed on the inner corner of EE's left eye resulting in a first degree burn. Antibiotics and eye drops were prescribed. No lost time.

EE slipped on ice from storm the previous night resulting in a concussion. Hardhat was being worn.

EE was returning cardox tubes to proper location when EE slipped on floor. This strained EE's left leg.

Employee was removing a bearing from a tire in a raw mill when bearing slipped and smashed left index and middle fingers.

It is believed that a malfunction occurred within the coal mill when a plow broke. This likely promoted ignition of fire due to sparking. No injuries reported, filters and support cages damaged during fire. All safety interlocks functioned correctly as expected.

While EE was wrenching pipe on the air line under 204-205 drags he felt a pain in his back.

EE was walking along the 603 feeder landing while performing an inspection. He was looking at the skirting and not the walkway. There was an open hole (15"x 15") in the grating that he stepped through. EE hit his knee on the side of the grating as he fell as well as his elbow.

Employee was hanging up a 3/4" diameter wire rope choker on to hooks in the truck shop. As he was extending his arms to place the loops over the pegs, he felt a tearing sensation in his RT shoulder.

Employee was cleaning up a spill by the 104 conveyor when a gust of wind kicked up dust into his eyes. Eyes were flushed onsite, but material was still present. EE was transported to clinic for treatment. EE was wearing all required PPE

The employee was walking up the stairs outside the Roll Press building and apparently caught the toe of his boot under one of the steps causing him to fall forward. The employee fell forward landing on his RT elbow.

Employee reported that the Doosan forklift used in the warehouse had a defective horn. They drove the forklift to the tuck-shop and tag it out of service. While walking back to the warehouse, the employee reported that they slipped on the ice near the iron ore ramp.

Employee was working in the clinker basement hooking up several ladders to the hook of the Model 308 Grove Crane. As he was securing the ladders with a rope, he noticed that the rope was tangled in some other equipment. The spotter misinterpreted his movements with his hand as the signal to raise the load, which cause his RT hand to be pinched under the cable.

The employee was unplugging one of the feeders in 117 tunnel using a jackhammer. The employee was positioned on the feeder platform, and was placing the jackhammer bit above the vibrating feeder to break up frozen chunks of rock when he felt a pop in his lower back.

Employee was in the process of climbing up a ladder, when he lost his balance, and fell approximately 6 to 8 feet to the floor level. Employee lost his footing and center of gravity, when climbing up the ladder, possibly due to sharp knee pain experienced, prior to the fall.

Employee was walking on the south side of the cooler building near the IKN fans when his right foot slipped on the snow. The employee twisted his left knee. The employee has a sprained left knee and has been placed on restricted duty.

Employee was directing a tractor-trailer flat bed truck of drainage pipe in the A-Frame building. While directing the truck to move forward, the truck drove over a bump. Apparently there was a loose 4 foot long 2 x 4 on top of a section of the piping which bounced off of the load striking the employee in the left thumb. The employee suffered a broken LT thumb as a result.

Employee was replacing diaphragms on 260 bag house. Employee was kneeling on his left knee using a pipe wrench to loosen diaphragm housing. While loosening one of the diaphragms, he felt a sharp pain on the outside of his RT knee. The diaphragms are located approximately 3 feet from the walking surface.

Employee was working on scale calibration for the 317 and 318 raw feeders. He was carrying 20KG weights and bending under low structures to reach the hooks to hang the weights. The employee complained of back pain after completing his task.

Employee was working on the south shipping load out sock. The employee who was in the basket of a manlift (Model JLG 860SJ) had completed the task and was maneuvering the basket downward. He apparently caught the basket on the load out causing the basket to move upward. He was caught in a pinch point.

Employee was inside the kiln inspecting feed end. While walking, the employee apparently stepped in an area with exposed metal refractory anchors which were welded to the shell. The employee felt something pierce his boot into his foot. Employee was able to walk back to his office and noticed some blood on his sock.

Injured slipped on ice when walking from parking lot to office and sprained his ankle.

Confirmed reportable hearing loss

When the injured climbed onto a forklift she jammed her hard hat against the roll cage and sustained a cervical sprain.

When injured attempted to pull an extension cord out of some mud his hand slipped off the cord and he struck his thumb against a piece of metal structure and sustained a laceration on the back of his thumb.

Injured felt a pop in his left shoulder when he caught a pin press that shifted off of the plank on which it was positioned for work on a bucket elevator.

Three employees were pulling the oil pipe out of the burner pipe. The injured employee was standing on a ladder at the burner pipe, the other 2 employees were pulling out the oil pipe. When the end of the pipe came free of the burner pipe, the injured employee could not hold on to his end and the overexertion caused a torn muscle in his forearm.

Affected employee was involved installing a scaffold base layout on 3 1/3 floor of a preheater. Suddenly without any warning, part of the refractory wall above the work area failed and pieces of the wall began falling. The refractory pieces fell about thirty feet striking employee causing injuries.

While working on a piping system for the delivery of a 19% aqua ammonia to the pre-heater some of the solution unexpectedly squirted from an open pipe and entered the injured's right eye causing chemical burns to his eye that were treated with prescription medication.

The injured slipped on some clinker on a concrete floor and fell against a corner of a partially crushed 55 gallon drum and suffered contusions of his ribs and side.

Injured pinched his left little finger between a hammer handle and wrench when he was attempting to strike the wrench to loosen a nut on a piece of all thread that was held in a vise.

The back of the injured employee's hand was cut when the cutting wheel on the hand grinder he was using to cut a slot in a pipe caught and kicked back and struck his opposite hand. Injuries included skin lacerations on the back of his hand.

Injured felt pain in his shoulder when he was hoisting an 8-inch hose using a rope looped through rope loop overhead to move the hose into position for connection to a hose on the vac truck.

Injured stepped on material in the walkway and felt a pain in his hip when he slipped and regained his balance. Sprain of right hip.

Left middle finger was lacerated when it was caught between the two halves of an elevator door when the injured was closing the door. The straps installed to enable pulling the door closed without exposure the pinch point were not used.

Injured got out of his personal vehicle in the parking lot, lost his footing on ice, fell on his back, and suffered thoracic contusion.

Employee was struck by a guard that he and a coworker were removing from the kiln drive and fell to the catwalk on which he was standing striking his right hip, back, and shoulder.

Employee reported knee pain when he stood and walked after working on his knees to make repairs on a belt scale.

Dust projected under safety glasses and into eyes by the reaction of a piece of corrugated hose that the injured broke apart by hand.

Hammering on the metal skirting that had been altered to fit into the chute area of 108 belt, at this point the employee felt something strike him in the eye causing discomfort. Upon looking into employee's eye the object was no longer visible as it had been.

Noise Induced hearing Loss

Employee fell on catwalk grating injuring right index finger & left knee carrying radio in right hand. Finger was smashed between grate openings causing trauma to fingernail. Hoses for torches & welding leads were on catwalk. Employee thought he was clear of obstructions as he slowly stepped over.

Employee informed supervisor that he had an accident a few hours earlier and was experiencing pain to right side of chest. Employee was working on top of roll press, dumping wheel barrel of waste material down chute and fell backwards down a flight of stairs.

Employee was pulling freight cart w/200-300lbs. of UPS deliveries. Employee lifted her R/leg and turned towards right side pulling the cart and when she put her weight on her left leg, employee felt pain in left knee cap area.

Employee was attempting to unchoke the 3a cyclone on the preheater. The material broke loose upon removing the airlance and flushed out the vessel. The material splased into employee's left boot and burned left ankle area.

Employee was going down the ladder of the haul truck, when he lost his balance on the last rung and fell backwards, landing in an area of hoses/gages that were placed outside the garage from the roller mill gear box. Employee hit his back on some the material on the ground.

Employee was breaking up hard cement with brick hammers around elevator areas. As other employee was in motion coming down with the hammer, IE reached for one of the lumps and was hit with the hammer on his finger.

EE was working in the cement cooler area, when he stepped and slipped on some clinker on the floor, twisted his left foot/ankle area.

Employee was working in the storeroom and was putting away an idler and felt a sharp pain in the middle of her back as she was lowering it into the skid.

Employee was attempting to get movement from the north roll press with the t-wrench provided. The wrench slipped off the sheave mounting bolts and he twisted his left wrist.

Employee slipped off a wooden planks in 147 hopper areas and jammed his left ankle.

Employee was assisting in high reach at A-frame area. As high reach was ascending, high reach basket bumped the hand rail and shook the basket. Right knee of employee gave out and was injured.

Employee was working on 260 screw conveyor to remove a gudgeon bolt with his partner. His partner was attempting to hit a driver to remove a gudgeon bolt and he missed the driver with the hammer. The hammer flew from his grip hitting injured in the groin (right side) are.

Employee was lifting ten 8 lb weights on and off weigh bridge 103 belts scale callibration. After removing weights, employee felt discomfort in lower left abdominal area.

Employee was assigned to clean 3rd deck of preheater area. While operating the fork lift, the employee placed left hand between the hopper and the forks which caused the dump part to kick back and severed his index finger (L) below the 1st joint area.

Employee had climbed ladder to enter discharge end of kiln. Ladder footing began to slide on IKN grates and caused employee to lose his balance. Employee fell and rolled onto his right side hitting some material and hurting his ribs, head and finger areas.

Employee slipped on wet cement at the tail end of 627 and twisted his left knee.

EE lifted a tool bag that weighed approx 60lbs. He felt a pull in his lower back.

Employee was using a 3/4 ton come-along to pull two lower sections of the roll press together. He applied two cranks when the tool slipped causing it to strike his right upper lip area.

EE WAS ENTERING THE BLENDING SILO ELEVATOR & TUBE BEING CARRIED BY 2ND EMPLOYEE ENTERING ELEVATOR HIT 1ST EMPLOYEE ON HEAD AS HE WAS TURNING AROUND.

Employee was using company truck to get ladder for cleaning 626 cooler choke. He twisted his right knee getting into the truck.

EE WS WALKING OUT ONTO STAIRWAY FROM THE 3RD FLOOR PREHEATER.SUPERVISOR ABOVE HIM HAD A FLASHLIGHT THAT FELL AND HIT EMPLOYEE'S HARDHAT. EMPLOYEE JERKED HIS LEFT SHOULDER IN A REFLEX ACTION. HE FEELS PAIN IN HIS LEFT SHOULDER.

The truck was equipped with a utility body which had keyless remote locks. An acetylene cylinder (approx 20 lb) had a handle which could be turned by hand instead of needing a wrench. To the best of our ability, we have determined that the acetylene bottle was leaking and the electronic locks ignited the gas.

WIND BLEW DEBRIS IN EYE

WAS LIFTING STARTER ONTO THE 992 LOADER. EE FELT SHARP PAIN IN RIGHT SHOULDER INTO NECK AREA. LUMPED FORMED ON FRONT SIDE OF NECK AREA.

Employee was working on the upper w. level of 228 damper to install plastic sheeting because it was recing employee fell into 36"x36" hole in gating to acced to the next level. Employee injured Left knee and upper torso areas.

EMPLOYEE WAS REPLACING AIR FILTERS ON 401 FAN THAT ARE ABOUT 12 FT HIGH. AFTER COMPLETING THE JOB (30 MINUTES LATER) LEFT SHOULDER STARTED HURTING. SURGERY ON THURSDAY 10/21/04.

HANDLING A BEAM PAN TO PICK UP HATCH: USING A JACKLEG OVER A BEAM

EMPLOYEE WAS REMOVING A PIECE OF 6"X 16'CHANNEL (WEIGHING 168 LBS.) FROM THE STEEL RACK. PIECE WAS APPROX. 5'HIGH ON THE RACK. EE WAS PULLING IT OUT BY HAND WHEN HE FELT A PAIN IN HIS LOWER STOMACH AREA.

EE REACHED UNDER CONVEYOR TO HELP REMOVE & PULL A LARGE PIECE OF CLINKER WHEN PIECE CAME LOOSE & SLID DOWN PINCHING EE'S LEFT MIDDLE FINGER ON LEFT HAND.

EMPLOYEE WAS HAMMERING A PIN OUT OF THE LIFTING ARM OF THE BOBCAT WHEN A PIECE OF THE PIN SHATTERED AND HIT HIM IN THE NECK CAUSING A LACERATION AND REQUIRING STITCHES.

EMPLOYEE WAS PLACING EYELETS OF CABLE INTO A COME-ALONG LOOP WHEN EYELET SPOOLED OFF HOOK & HIT HIM IN THE LEFT EYELID AREA.

EMPLOYEE WAS WALKING THROUGH #1 KILN TO INSPECT BRICK. WHILE PASSING THROUGH THE BURNING ZONE EMPLOYEE STEPPED IN A PILE OF HOT DUST. HOT DUST ENTERED SHOE BURNING THE SKIN ON INSIDE OF HIS LE FT ANKLE. DID NOT SEE DR UNTIL 11/10/03. NOTIFICATION OF 2ND DEGREE BURN ON 11/13/03.

WHILE REPLACING DOGBONE ON 500E BATCH PLANT, EE WAS USING A SOCKET WRENCH, THE WRENCH SLIPPED OFF OF THE NUT AND HIS FINGER GOT PINCHED BETWEEN THE WRENCH AND THE SUPPORTING ANGLE IRON FOR THE GEAR BOX.

EE WAS ASSISTING SHIFT FOREMAN ON 3 FLR OF PREHEATER & TIGHTENING BOLTS. AT THIS POINT EE NOTICED HIS SAFETY BOOT WAS SMOKING. EE WAS STANDING IN A SMALL PILE OF HOT FEED.

EE WAS ATTEMPTING TO ROTATE 460 SCREW ON INPUT AGUDGEON TO REPLACE BOLTS W/O NEED ONE-HE WAS PULLING ONE A 36' PIPE WRNEHC W/A 3' PIPE ATTACHED AS HE WAS PULLING HE FELT A POP IN HIS LEFT SHOU LDER.

EE'S HEARING TEST INDICATED A THRESHOLD SHIFT. EE HAS BEEN RE-TESTED BY A SPECIALIST (AUDIOLOGIST) AS A FOLLOW-UP AND HAS BEEN INFORMED OF HIS RESULTS.

EE WAS REMOVING BAGS FROM 510 D.C., LOWER SECTION. AS HE WAS PRYING BAG OFF WITH A SCREWDRIVER, HE FELT A PAIN IN HIS RT. RIB CAGE. HE WAS USING A 12" SCREWDRIVER.

EE S HEARING TEST INDICATED A THRISHOLD SHIFT. EE HAS BEEN RE-TESTED BY A SPECIALIST AUDIOLOGIST AS A FOLLOW-UP AND HAS BEEN INFORMED OF HIS RESULTS.

EE & FELLOW LABOR WERE ON THE PREHEATER FLR. 1 4" CAP WAS OFF THE A-SIDE FEED. WHEN EE WENT TOPUT CAP BACK ON, IT SLIPPED & HIS LEFT FOREARM CAME INTO CONTACT WITH THE PIPE. EE RECEIVED A BURN TO LEFT FOREARM.

EE REPORTED THAT HE HURT HIS BACK AFTER THE BRICK JOB

REPETITIVE MOTION.

EE WAS STANDING ON THE NORTH SIDE OF THE SCREW. OTHER EE ON SOUTH SIDE USING SHOVEL TO KNOCK DOWN MUD WHEN SPRAY TOWER FLUSHED OUT OF HOLE OF CRACKS AND FROM LIDS.

EE WAS USING A SHOVEL TO KNOCK THE MUD DOWN OFF THE CRATE INTO A HOLE TO DROP ON THE GROUND WHEN THE SPRAY TOUCH FLUSHED CONE OUT OF THE HOLE & CRACKS IT FROM LIDS.

EE WAS WALKING DOWN STEPS AT 103 BELT TO ROAD. STEPPED ON ICE PATCH ON THE ROAD. HE SLIPPED BUTDID NOT FALL. HE FELT SOMETHING PULL IN HIS RIGHT KNEE.

EE WHILE TRYING TO REMOVE A 5' X 5' PLATE OF STEEL FROM ROCK. BOTTOM OF PLATE SLIPPED ON THE FORK OF THE FORKLIFT, CAUSING TOP OF PLATE TO PINCH RIGHT HAND, MIDDLE FINGER AGAINST LIFT RAIL OF FORKLIFT.

EMPLOYEE WAS SENT TO CLEAN 149 BELT WALKWAY, WALKWAY HAD COAL GYPSUM, CLINKER STONES STUCK IN GRATING. EMPLOYEE WAS ON HIS KNEE PICKING STONE OUT OF GRATING WHERE HE WENT TO STAND UP TO MOVE, HE TWISTED HIS KNEE.

EE WAS ASSISTING ANOTHER EE IN REMOVING A DUST COLLECTOR BAG FROM 510 DC ON THE 3RD FLOOR FROM EAST TO WEST. AS EE WAS PULLING TO REMOVE THE INNER SCREEN, HIS RIGHT HAND SLIPPED CAUSING HIS RI GHT ELBOW TO STRIKE THE INNER DOOR LATCH.

EE WAS HITTING A BRICK SHIM BETWEEN 2 BRICKS ANDHIT HIS THUMB WITH THE HAMMER.

EMPLOYEE WAS UNCHOKING CLINKER SCALE BELT. AS HE WALKED DOWN STEPS FROM BELT, HE WAS HIT ON THE SHOULDER WITH CLINKER FALLING FROM HOLE IN CLINKER ELEVATOR. HE LOOKED UP TO SEE WHERE IT FELL A ND SLIPPED ON CLINKER AT THE BOTTOM OF THE STEPS HURTING HIS BACK.

EMPLOYEE WAS PULLING UP SUPER SUCKER HOSE BETWEEN THE LANDING FROM 3RD FLOOR TO OTHER FLOORS AND FELT A PAIN IN HIS LOWER BACK.

TRYING TO HELP FLIP OVER A PIECE OF STEEL EE GOTTIP OF LEFT RING FINGER CAUGHT BETWEEN THE STEELAND THE FRAME.

EMPLOYEE WAS INSTALLING A WATER FILTER ON 109-110 TAIL BELT LINE WATER SPRAY PIPE. EE WAS KNEELING PULLING ON A 14" PIPE WRENCH TO TIGHTEN A 2"X6" NIPPLE, WHEN HE FELT A PAIN IN HIS LOWER BACK . THE NIPPLE WAS APPROX 20" IN HEIGHT ALONE.

EE WAS WORKING ON A GUARD IN THE SHOP. HE WAS TRIMMING A PIECE OF ONE END. WHEN ADJUSTING HIS GRIP HE SLID HIS HAND ACROSS THE SHARP END SLICINGHIS FINGER.

EE WAS SHRINK WRAPPING PALLET OF BOXES. EE BENT OVER & PULLED WRAP AROUND CORNERS OF PALLET & FELT PULLING PAIN IN HIP & LEG AREA

EE WAS STEPPING DOWN FROM LEDGE OF FAN & AS HE STEPPED DOWN, HE STEPPED IN CEMENT WHICH CONTAINED A ROLLER BEARING ABOUT 1" IN DIAMETER CAUSING HIS RIGHT ANKLE TO TWIST.

EE WAS CLEANING BUILUP ON FEED SHELF KILN USING A BAR TO POKE THE BUILDUP ON SHELF. EE EXPERIENCED A SHARP PAIN IN RIGHT SHOULDER.

EE WAS SWING A SLEDGE HAMMER OVERHEAD, IN THE FIRST COMPARTMENT OF THE FMA TRYING TO DRIVE IN FARTHER THE DIVISION BOLTS. AS HE SWUNG THE SLEDGEHAMMER, HE FELT A POP IN HIS RIGHT SHOULDER.

EMPLOYEE WAS WALKING DOWN SLIGHT GRADE FROM TRANSFER TOWER HEADED FOR ELEC ROOM. GROUND HAD SOME ICE COVERING FROM PREVIOUS DAY. SLIPPED AND FELL. EMPLOYEE HAS SORE TAILBONE AND SPRAINED WRIST .

EE WAS TAKING DOWN SCAFFOLDING WHEN THE PLATFORMON WHICH HE WAS STANDING ON FELL THRU. IT CAUSEDHIM TO FALL APROX 5FT ONTO THE GRATING BELOW

EE WAS SENT TO 109 BELT TO CLEAN OUT UNDERNEATH.FROM HEAVY RAIN ON SATURDAY MUD HAD BUILT UP UNDER THE RETURN FEEDERS. MONDAY THE MUD BECAME FROZEN & A JACKHAMMER WAS REQUIRED FOR CLEANUP. MAT ERIAL WAS HEAVY AND EE HAD TO SHOVEL IT.

EE PULLED ARM & SHOULDER WHILE LIFTING A LINER WHICH WEIGHED ABOUT 100 POUNDS.

EE WAS GETTING OFF LOADER AND SLIPPED OR ROCK AND LIFT KNEE.

EMPLOYEE LIFTED A 480 LB DRUM OF OIL WITH THE AID OF A LIFTING BAR ON THE CATWALK ON THE DISCHARGE PILE OF THE MILL.

EE WAS WALKING UP TO TAKE CARE OF PROBLEM AT 108 FEEDER BELT. HE STEPPED IN HOLE OR RAILROAD TRACKS, NOT SURE. TWISTED LEFT KNEE.

IN THE PROCESS OF CHANGING FAN BLADES AND HUB ON 409-1 EMPLOYEE WAS LOOSING HUB BOLT WHEN IT BROKE AND EMPLOYEE FELL BACK KNOCKING HIS HELMET OFF HIS HEAD AND THEN HIS HEAD TOUCHED A HOT TUBE.

EMPLOYEE WAS REMOVING OLD PIECE OF CABLE COILED UP AND THREW IT OVER HIS SHOULDER. THIS CAUSED EMPLOYEE TO TEIST HIS BACK. EMPLOYEE UNDERESTIMATED WEIGHT OF CABLE.

EMPLOYEE WENT TO THE IRON TANK AREA TO SEE WHY DUST WAS LEAKING TO THE GROUND. HE NOTICED A HOLEIN THE SCREW HOUSING WHERE A PIPE HAD FALLEN OFF. SCREW GRABBED AND TURNED THE BELT. EMPLOYEE'S THUMB GOT PINCHED BETWEEN THE GUARD AND SCREW COVER.

EMPLOYEE WAS ATTEMPTING TO ADD OIL TO THE HYDRAULIC RESERVOIR FOR THE COOLER. EMPLOYEE MOVED A FULL 55 GALLON DRUM OF DT25 AWAY FROM RESERVOIR BY LIFTING DRUM ON END AND ROTATING DRUM.

EE WAS MOVING 55 GAL. OF OIL INTO POSITION TO BE PUMPED INTO MILL GEAR HOUSING RES. MIDDLE OF BACK AT THE BASE OF NECK BEGAN TO HURT.

EMPLOYEE CLAIMSFROM REPETITIVE BENDING WHILE PUTTING 40 LB BUCKETS ON 314 ELEVATOR SOMETHING IN HIS LOWER BACK IS CAUSING HIM PAIN.

EMPLOYEE WAS IN THE AREA WHERE ARC WELDING WAS TAKING PLACE. FUMES OUT OF KILN AND DRIFTS FLOWED TOWARD EMPLOYEE. HE FELT SHORTNESS OF BREATH.

EE WENT TO CLEAR OUT 694 BELT DRAG CHAIN. HE HAD TO BEND OR LIE DOWN TO WORK. WHEN HE GOT UP HE FELT PAIN IN HIS LOWER BACK.

EMPLOYEE WAS RIDING IN A PICKUP TRUCK WITH SUPERVISOR WHEN THEY HIT A BUMP IN THE ROAD. EMPLOYEE JARRED HIS NECK.

EMPLOYEE WAS WALKING TO HIS CAR TO GO TO A MEETING AT NAZ 3 AND SLIPPED ON SOME SNOW/ICE AND FELL. IN AN EFFORT TO KEEP HIS BALANCE EMPLOYEE MADE A QUICK MOVEMENT WITH HIS HEAD.

EMPLOYEE WAS CHECKING 410 DC. HE SLIPPED ON KIND OF HYPEREXTENDED KNEE (LEFT).
