REPLACING WORN METAL UNDER CRUSHER. OTHER EE WASSWINGING SLEDGE HAMMER TO GET METAL LOOSE. EE REACHED IN TO GET DIRT AWAY. OTHER EE SWUNG AND HIT EE ON LEFT HAND LITTLE FINGER.

EE WAS HELPING TO CHANGE BUCKET OFF TRACK HOE. EE WAS HITTING PIN WITH BAR, BUCKET SHIFTED BAR GOT WEDGED. EE WAS HIT IN FRONT LEFT SIDE COLLAR BONE.

WALKING TO COPIER, TRIPPED ON RUG.

EE WAS GREASING HAUL TRUCK. WALKED INTO TAIL END OF TRUCK BED.

REMOVING ROLLERS FROM SHELF, STEPPED BACK TRIPPED OVER ROLLERS ON FLOOR.

REPLACING TEETH ON LOADER BUCKET, USING SLEDGE TO SET PINS. PIECE OF STEEL BROKE OFF AND FLEW IN RIGHT EYE.

EMPLOYEE WAS PICKING ROCK OUT OF JAMMED CRUSHER; ATTEMPTED TO LIFT A LARG ROCK.
