Miner was installing a small roof over a conveyor belt. A 2x4 piece of wood fell and hit EE in the right hand. EE continued to work this day and the next day, received medical attention on 05/15/2026.

Employee was lifting a metal box with approximately 25 pounds of welding rods inside, upon lifting the box slipped and they tried to catch it. Their finger went inside the opening of the box where you take the rods out of and the metal box sliced finger open. They returned to work after receiving 9 stitches in their finger.

Employee was grinding metal hopper door. Foreign material entered EE's right eye causing a corneal abrasion. EE was wearing safety glasses at the time.

Employee was trying to remove stuck 2 1/4" socket from 3/4" ratchet. Had ratchet in vise, attempted to push in ball of ratchet and pull down on socket. When socket released, employee's left ring finger had come in contact between the socket and work bench, resulting in closed fracture of tuft of distal phalanx of finger.
EE was checking computers and PDU's. When EE bent over, one computer fell on EE's head. EE had PPE on while in the container. EE said the computer rolled off and hit EE's hardhat and neck when it rolled down. The Minerva computer weighs approx. 13kg or approx. 28lbs.
Employee was descending catwalk, slipped upon exiting to the ground, hit right hand on railing. Catwalk was clear of debris and snow and ice. Temperatures 10 degrees, possible residual snow on boots caused slip. Employee went to ER, it was determined no broken bones, but to follow up with orthopedic.
