While cleaning rock off of the miner employee pulled a rock off and the end EE was holding went to the roof pinching the tip of EE's left pointer finger resulting in one suture.

While bending roof bolts employee felt a pop in EE's shoulder. Employee worked up till EE's surgery on 10/10/2025

Employee had a rock stuck in the arms of the bolter when EE lowered the head the rock sprung out hitting the employee in the lip and shoulder causing lacerations that required sutures.

While pulling belt employee's channel locks slipped off of the belt causing their elbow to hit the roof resulting in a laceration that required 4 sutures.

A small piece of rock fell from the roof and hit employee on the forearm causing a laceration that required sutures.

While removing a drill steel from the roof one came out and pinched employee's finger causing a laceration that required sutures.

While exiting the caterpillar 926M forklift employee missed the top step and fell to the ground on shoulder.

Employee was servicing a set of scoop batteries when the lid fell on their hand resulting in a laceration to the top of hand requiring 5 stitches.

A roof fall above anchorage occurred at the face in the belt entry in the C-5B section. The roof fall was approximately 25' long, 18' wide and 8' high. There were no visible features in the roof in this area.

While building a wall a block fell off the top row and landed on employee's finger resulting in 6 stitches.

While adjusting the bolter head employee got EE's finger pinched between the longarm and the canopy post. Employee received medical treatment 6/12/2024 receiving stitches making this a reportable accident.

While employee was pinning the belt tail EE's glove got caught in the hilti drill bit causing 7 stitches.

While trying to straighten a bolt that was bent in the roof, employee felt a pain in the shoulder. Employee worked till 5/3/2024.

While throwing a roll of rubber onto the top of the bolter employee cut EE's hand on a T3 channel that was bolted to the roof. Employee received 4 stitches.

A roof fall was found in the return #2 entry above the anchorage point. The length of the fall was approximately 80 feet by 18 feet and approximately 8 feet high. There was one geological feature present in the area.

While pulling out of the shop employee hit a supply car with employee's lil mac buggy. When hitting the supply car the steering wheel spun throwing employee's hand off of the wheel and cutting it.

While crawling past the bolter employee cut their left ear on a sign that was hanging off of the bolter resulting in 6 stitches.

While bolting employee pinched their hand in between their pointer finger and thumb between two bolter steels receiving eleven stitches.

Employee felt a pop in knee while crawling. Employee received surgery on 6/30/2023

Jacking up the dolly employee thought the jack was engaged. When letting go of the handle it hit them in the cheek causing 5 stitches and a lost tooth.

Our employee is a truck driver. Employee had mud on boots and slipped on the ground and fell while stepping down out of truck. Employee fractured wrist.

While exiting the bolter employee cut EE's arm on a roof strap that was hanging off of the bolter receiving 6 stitches.

There was a roof fall above anchorage at the face area of C4butt section. The fall likely occurred due to localized condition in this area and appeared to be made up of layers of laminated shale. The fall was approximately 135 feet long by 18 to 20 feet wide and 4-8 feet above the roof line.

While hanging canvas employee cut their hand on a roof bolt plate. Resulting in 8 stitches.

While loading the bolter a bundle of cable bolt plates fell and hit employee on the hand. Resulting in 8 stitches on pinky finger.

The injured employee was riding on a diesel mantrip facing the out by direction when the operator of the mantrip turned out of a crosscut too hard and accidently came into contact with a bolt car. The part of the bolt car came inside the mantrip and pinched employee's toes between the support post on the mantrip resulting in the fractured toes and sutures.

Employee was cleaning rock off of the miner. When they slid one off it hit their leg causing a laceration. They received 11 stitches.

There was a roof fall above anchorage point in the belt line of 22E at wall 3 to 4. There was one geological feature present in the area. The length of the fall was approximately 80 feet by 20 feet. It was around 10 feet above normal roof.

Employee was loading the bolter with supplies when EE cut EE's arm on a roof strap that was hanging off the side of the bolter. Employee received 8 stitches

Employee was trying to stand a scoop tire up by hand when employee lost footing hitting face against the rim. Resulting in 3 stitches inside bottom lip and 1 outside.

While cleaning up around the pit area employee cut left hand on a piece of sheet metal. Resulting in 6 stitches.

Employee has a sore knee from crawling. Has not returned back to work since 5/4/2022

While belting up employee got EE's middle finger pinched between a low structure rail and a low structure stand. Resulting in 4 stitches

Normal working section. Injured was crawling through a man door when the injured cut a hand on a sharp edge of the frame. Injured was wearing gloves at the time and received 13 stitches.

While running MBC EE twisted to check the cables straining neck. Was diagnosed with degenerative disc disorder. Worked till April 22nd. Received surgery April 30.

Knee was swollen from continuous crawling. Received surgery on 9/2/2020

Employee was moving structure, when they threw a piece, arm came in contact with a sharp rock. This resulted in 7 stitches.

While going to hook up slings EE slipped and fell. While falling EE put arms out to break the fall resulting in a fractured right wrist. No work missed.

While loading belt structure onto a skid. EE pinched finger in-between the structure skid and the structure EE was loading. EE received 13 stitches. No work missed.

While cleaning rock from the top of the miner. The rock the victim was removing broke and cut EE's left forearm. EE did receive 4 stitches. No work missed.

While lifting a stack of roof mats from the scoop to the bolter EE felt a pull in right shoulder. EE received surgery on 2/27/2020 making the accident reportable.

While trying to put a main tail on site using a railroad the tail slid causing the drill steel handle to kick up and strike employee above left eye. EE received 4 stitches. No work missed.

Employee was crawling under a bolter cable and the snub came loose causing the cable to hit EE's face. EE received 11 stitches. No work lost

It was found that there was a geological feature just above the installed ten foot cable bolts that caused the fall.

While walking thru the shop victim bumped into the scrap steel dumpster cutting right forearm with a piece of cut steel. Resulting in seven staples. No work missed.

Roof fall was found on a pre-shift examination. Area was cribbed and dangered off

There was a 4' strap installed on the roof that was bent down. Victim cut hand trying to bend the strap back. EE received 4 stitches

Normal working section. Miner was helping load bolter when they turned, they cut their arm on a monster mat that was hanging off of the side of the bolter. They received 11 stitches, no time lost.

normal working section. Victim was drilling a hole when the drill steel broke and struck EE in the chin causing 7 stitches. No work missed

Victim was tightening a bolt on the bolter, the wrench slipped off the head of the bolt, causing their hand to hit the bolter, cutting the top of their hand. Employee did receive 4 stitches. No work lost.

Normal working section - employee pulled a strap from the back of the bolter and cut arm. Employee did receive 7 stitches. *NO WORK MISSED

Knee swollen from repetitive crawling. Normal working section. EE worked until ee surgery on 10/9/17

When putting top rollers in a scoop bucket, one stand kicked up and hit EE's eye. EE received 4 stitches in eyebrow. No work missed.

Victim crawled under miner tail and when EE thought EE was clear EE started the miner to swing and raise the tail EE hit the wrong function and set the tail down on EE's leg.

Employee was climbing ladder onto equipment. EE lost EE's handle and fell to the ground.

While loading his scoop with rock dust he felt something pop in his lower back.

Trying to hook supply car to a scoop, pinched his left middle finger. - No work missed. - Received 11 stitches.

While he was cutting a phone line the knife slipped and cut his hand. Approx. 12 stitches. *No work will be missed.

A piece of steel fell and cut a small piece off the tip of hid pinky finger. No work missed.

While relaying belt structure for section belt up. Victim attempted to grab a piece of structure and was hit on right hand by another piece that was thrown by another miner.

Area was very muddy. Section was about to start moving to set of rooms outby. Haulage system was being disassembled. Bridge conveyor was removed from RHS and dolly and set on a crib block on the bottom. Mechanic stepped between bridge and RHS when bridge slid toward RHS catching victim's left ankle and fracturing same.

Injured stated that as he hooked up the bulk dust bag to the IT Loader Fork, he turned around and his right foot fell into a hole in the pallet causing his right knee to twist. He felt a sharp pain. Employee continued working his regular job, but was later diagnosed with a tear of the medial and lateral meniscus. He had surgery on 9/29/14.

A Roof Bolter Operator Trainee, while being trained, got his fingers caught between the drill steels as he was putting the drill steels together to drill a hole. Employee was wearing the proper metacarpal gloves at the time of the accident. Laceration to his finger required stitches.

Injured stated that when pulling a piece of rock from the top of his continuous miner, he felt a pain in his abdomen. He continued to work his regular job and refused to go to a doctor. On 05/13/2014, he was sent to a doctor who diagnosed a left inguinal hernia.

Employee was walking along the RHS when his foot slipped, stuck in the mud and he felt pain in his right knee. He continued to work his regular job until and MRI revealed a torn miniscus.

Employee was operating a scoop tractor using a 1/2-inch grade 80 steel chain to tow a disabled MBM when the chain broke he was struck by a link causing a laceration requiring sutures.

Roof fall occurred in an intersection in the R-3 entry (Intake) of the 12-Butt off 2-Right Continuous Haulage Section approximately 310-feet outby the working face. The area had been supported using Jenmar 6-foot "Suprtwist" roof bolts with 8-inch x 8" plates on 4-foot x 4-foot centers. No roof defects noted.

Roof Bolter Operator - While attempting to remove a damaged strap by loosening the nut on a super twist roof bolt, the strap rotated suddenly around lacerating the left side of his face requiring three sutures.

Notified of Reportable Hearing Loss on this employee

While bolting in the cross cut R1-R2. After inserting the bolt into the chuck. The operator accidentally bumped the rotation lever. This action caught the glove on his right hand to be caught in the rotation. This caused a fractured left 5th proximal phalanx.

Injured claims that when tightening a bolt on the cable tender, the 8-inch crescent wrench slipped causing him to lose his balance falling backward and twisting his left knee. Initially diagnosed as a strain and he was released to regular duty. A recent MRI shows a left meniscus tear requiring surgery. He became a LTA on March 1, 2013.

Operating Scoop Tractor - Pulling belt with scoop, when the belt caught in the rack, the belt grab slipped off the belt causing the scoop to lunge forward causing the employee to strike the back of his head on a supertwist bolt causing a laceration requiring sutures. Scoop is not equipped with a canopy due to the low mine height of 37-inches.

Roof fall discovered by mine examiner on the 1-Right Beltline extending from just outby SS#628 to SS#835 (540' x 19' x 4' to 12' high).

Injured was repairing a broken coal chain on the miner using a chain brake tool, chisel, and a hammer. He struck the chisel with a hammer causing the chisel to fly up and strike him in the head causing a laceration. The wound was closed with sutures.

A recently mined 30-foot cut taken through crosscut Belt to L1 Entry and cutting into the fully-supported L1 Entry experienced roof working and falling in the unsupported crosscut to a height of 24 that extended into the L1 Entry causing an unintentional roof fall in the supported L1 (4-foot FG Resin bolts) to a height of 42. Cave dimensions = 19 wide x 19 long x 42 high.

Employee started having problems a few weeks ago with a "water sack" (prepatellar bursitis) on his right knee and has been treated for it as he works. His Doctor now says he must have surgery and wants him off work until he has the procedure.

Employee states that his work related activities have resulted in a repetitive illness defined as left carpal tunnel syndrome. He continued to work his regular duties until his surgery on 01/2 5/2012.

Employee was positioning drill head to install last row of roof bolts when a piece of draw rock fell striking his finger of his hand that was resting on the drill head causing a slight fracture and a laceration requiring sutures.

Employee was shoveling around belt tailpiece when a cable tender chain broke and struck him on the right side of his body in the rib area.

Injured releasing grease take-up to replace CBM Foot Shaft when the grease gun fitting he was using broke allowing pressurized grease to strike him in the face. Employee has been working his regular job, but had surgery on 9/15/2011 to remove imbedded grease from under the skin on his face. He has not missed any work as a result of the surgery and continues to do his regular job.

Employee stated he was removing a wheel unit from a shuttle car when the ratchet he was using slipped and struck him on the right side of his face causing a fracture and lacerations.

An unintentional roof fall occurred in the 3 Left Section in the Belt, X-Cut Belt to L1, and the L1 entry at spad 8401. Roof fall was 70' long x 20' wide x approximately 12' high. The area had been supported with 6-foot suprtwist bolts.

Employee was installing a 4-foot fully-grouted resin bolt. As he was pushing the bent bolt into the hole, a small piece of roof rock broke free from the roof when the bend in the bolt contacted the roof. The rock struck his left forearm causing a laceration requiring stitches.

Roof initially bolted with 4' FG Resin Bolts. Cutter started working along left rib of L-2 Entry on 4/5/11. Supplemental supports added at that time 6' Supertwist TT Bolts and 12' cable bolts. A 20 foot unbolted cut fell in and the entry outby was center cribbed. Roof fall extended another 20' bolted with 4' FG Resin bolts. (40'L x 19.5'W x 4.5'H).

While walking in surface screen area, employee slipped on the ice, landing on his right ankle. The bones above the ankle were fractured from the FSL.

Employee was placing timber on the forks of the front-end loader, when a nail in the timber caught his finger, causing a laceration that required sutures to close.

Employee claims to have right shoulder pain that has developed from his roof bolting job. The employee went to the doctor on 09/24/10. Employee continued to work until on 10/05/10 at a subsequent doctor visit, the employee was determined to have bursitis and restricted from working.

Roof fall found by mine examiner during pre-shift of section. Roof fall measured 40-ft long X 20-ft wide X 6+ feet high, located between two intersections on the beltline. Clay vein crossed heading just inby cave discovered initial mining and roof supported with 6-ft Super Twist Bolts (torque tension) with six bolts across on three foot advance with 10-ft cable bolts.

Injured and co-workers were installing a guard on the #1 CBM when the guard slipped pinching his thumb causing a simple fracture.

Injured and co-worker replacing a CBM guard when it dropped, pinching his left finger between the guard and RHS, causing an avulsion to his finger.

Employee was helping pull a broken conveyor belt together with a pulley and wire rope. As they were pulling, the pulley came loose and struck a post, dislodging the post which struck the employee.

Employee claims that as he was getting out of his scoop tractor, he slipped and felt pain in his lower back on the right side. Employee continued to work until April 21.

Injured stated that as he was operating his Johnson Mantrip the front wheel went into a hole in the runway causing the steering wheel to spin out of his grasp. The steering knob struck the back of his hand causing a fracture to one of his fingers.

Crosscut and partial intersections between the L2 and L3 entries one crosscut outby the LOX in the 3 Butt off 2 Right working section experienced a roof fall. The L3 entry was supported with 6' x 3/4" fully grouted resin bolts. The crosscut and L2 entry supported with 4'x 5/8" FG resin. The L3 entry had bed separation at 40' and water running out test holes & vertical cracks.

Mine examiner encountered a roof fall in the return entries (L3 & L2) just inby SS#5612 in 2-East off 1-Right section at appx 5:10 AM. The L2 entry was supported with 6' super twist bolts with T-5 channels and 12' cable bolts on 8' centers. The L3 entry was with 4' resin bolts and then center cribbed on 5' centers. The area was at the bottom of a dip in the coal seam.

Employee was advancing lifeline on the R1 Entry adjacent to a Fletcher Roof Bolter, when the bolter was started and trammed forward running over his right foot and toes, causing a simple fracture to the 4th metatarsal bone in his right foot. Employee is working his regular occupation.

While installing belt structure, the rib fell, about 12' long, 3 to 7 inches wide and 10 inches thick at the thickest point, hitting his hat and partially covering him with rock.

Roof fall found by safety man and federal mine inspector while traveling through an abandoned worked out area that is not normally traveled or examined. Roof fall 110' x 18' x unknown. Area supported by 4-foot fully-grouted resin bolts on 4-foot x 4-foot centers.

Unintentional roof fall occurred in 4-Right (001) Section while mine was idle. Fall found by pre-shift examiner at approximately 8:45 PM - called outside at 9:05 PM. Fall from x-cut R-2 entry to L-1 entry (175 feet x 19 feet x at least 8 feet in height). Area supported with 4-foot fully grouted bolts, except belt entry supported with 6-foot super-twist & T-5 Channel.

Employee states that as he was walking from the parking lot to the portal building prior to the start of his regular work shift, he stepped in a hole at the top of the landing, causing him to twist his ankle.

Roof fall found by mine examiner during pre-shift of section. Roof fall measured 60-ft long X 20-ft wide X 9.5-ft high located between two intersections on the beltline. Clay veins discovered initial mining. Roof supported with 6-ft Super Twist Bolts (torque tension) with six bolts across on three foot advance with cable bolts located at the edges of the intersections.

Employee stated he was bolting first cut in a x-cut. As he installed the drill steel into the chuck, a rock fell and struck him on the left hand, causing a laceration to his left index finger

Employee was installing roof bolt and guiding it into the roof bolter drill chuck when he bumped the rotation lever to seat the bolt his glove caught onto the bolt wrapping his hand around the bolt causing a "mallet finger". Employee continued working regular job until 04/28/09 when he had minor surgery on the finger.

Employee states that he was pulling miner cable when he felt a pain in his right groin. Initially thought to be a muscle strain was later diagnosed as a hernia requiring surgery on January 5, 2009.

Employee stated, that while he was not involved in any type of accident, his left knee became irritated and swollen. Originally diagnosed as cellulitis and not work related, a specialist diagonosis on 9/24/2008 stated that this condition could be work related.

Employee was operating a personnel carrier, when it hit a rut in the runway, causing the steering wheel to spin. The steering knob struck his hand, causing a bruise and cut, that required one suture to close the wound.

Injured was operating Johnson Man-trip down the 1st Right Beltline. When turning from 1 Right to Main B, the fron wheel of the man-trip contacted a beam leg causing the steering wheel to spin suddenly causing the steering know to strike his right thumb.

Employee was splicing a #2 - 3 conductor GGC charger cable. As he twisted the cable, his left hand came into contact with the open blade of his utility knife causing a laceration requiring 4 sutures

Employee developed pain and numbness in his left wrist, hand, and fingers over a long period of time. Employees first day off was 5/29/08.

Employee stated that as he was operating the continuous miner, he turned and felt his knee pop and that he continued to experience pain in his knee as the shift went on.

Employee was surveying undergound in the mains section and he turned around to pick up hand drill. When employee went to place right knee onto mine floor. He heard and felt something pop. He finished shifted and sought medical evaluation on 10/15/07.

Roof fall found by examiner in worked out area of 1 Right just outby spad 14340. Fall measured 60? x 19.5? x 6 to 7? high. Roof supported with 4? fully grouted resin bolts (6 bolts across) with 8? x 8? plates. Probable continuation of cave located 80? inby.

A roof fall was found in the intersection of the L-2 entry of the RT 11 off 1st Right Butt 160 feet outby spad 2388. Cave measured 45' x 19' x 4' high. Roof supported with 4' fully grouted resin bolts (4' x 4' centers) with 8" x 8" plates. No known defects in the shale roof.

Employee was lifting a 50 pound bag of rockdust when he felt a slight pain in his lower abdomen. This pain was diagnosed as a right inguinal hernia on August 3, 2007.

Employee was removing a rub-rail from the side of the tail bridge when he lost his grip on the rail. His left hand was caught between the rail and the RHS causing a laceration and fracture to his finger.

Injured was kneeling beside roof bolting machine bending roof bolts, when a piece of draw rock fell from the roof, striking injured on his right side.

While operating Continuous Miner from kneeling position, injured was struck on the side of his right foot by a piece of draw rock.

Employee was unloading belt structure off a skid; as he was dragging a top stand off the skid, a bottom stand fell over pinching his little finger on his left hand between the two stands causing a laceration and fracture to the little finger.

Employee was withdrawing drill steels from roof when the steels suddenly came loose falling out of the hole and striking employee on his left hip causing a small cut and bruise. The accident was mentioned to his foreman on 10/2/06, however medical treatment was not obtained until 10/6/06.

Employees left knee became irritated and sore due to repetitive crawling.

Roof fall occurred in the 1st Right Haulage Section approximately 2,000 feet outby the loading point in the R-2 entry spad 1147. Fall is approx. 60' x 20' x 5'. Ventilation and travel were not affected. A shiny material noted in the rock strata may have been a contributing factor.

Roof Fall intersection of Beltline at 6,697 feet from B-Main adjacent to SS 1438. The approx. dimensions are 20' x 90' x 8' high. The area bolted with 4' fully grouted resin bolts. Evidence of a cutter and water after the fall.

Employee using a come-a-long to pull a pin out of the trunion on #1 shuttle car while pushing on the handle he slipped and the handle struck him in the mouth

Injured was making a splice on a shuttle car cable. While turning the cable, his left hand came into contact with the open blade of the utility knife causing a laceration to his left hand little finger requiring 3 stitches.

Employee was walking along the RFM in the beltline to check the haulage dolly for blockage when he either slipped and fell onto the moving belt or stepped onto the moving belt (the employee is unsure). He proceeded down the belt and became jammed between the dolly and the belt

Intersection of L-1 Entry in the Continuous Haulage Section 1st Right caved extending through the crosscut toward the belt entry. The cave is approx. 60' long x 20' wide x 4' to 5' high. The area was bolted with 5' resin bolts.

Employee was kneeling in the R1 entry of the 1st Rt. haulage section completing his survey calculations & was struck with the bucket end of the scoop that was loaded with roof bolting supplies. EE was struck in the hip/back area causing his legs to be pushed underneath the bucket. EE was then pushed inby approx. 10' before the scoop operator heard his screams to stop.

Injured was drilling a 1" hole when the drill steels stuck in the roof. As he attempted to line up the drill steel and the drill chuck using his right hand, he caught his fingers between the chuck and the roof causing lacerations that required stitches.

Employee was splicing a shuttle car cable when he cut his left thumb with a razor knife.

Employee was lifting a bag of b-bond when he felt a pain in his right groin in the same area where he had had a hernia repaired in April of 2005. Doctor reports have not been sent from the doctor seeing him after this incident on 3/13/06 as to the extent of the injury.

Injured employee was lifting a piece of 42" belt using a belt rail when he felt a pain in his back.

The cave 480' outby the face of the B-Main Section started in the intersection of the belt entry, running through the crosscut belt through the L-1 entry and stopping approximately 25 feet from reaching the L-2 entry. The caved area is approximately 20 feet wide and 95 feet long. Four foot bolts were used in the affected area, and the cave is approximately 4 feet high.

Injured was installing an 8 foot bolt into the hole when he accidently bumped the rotation lever causing the bolt to suddenly bend and rotate causing him to hyper-extend his left arm. Originally diagnosed as a strain. New diagonosis is rotator cuff tear requiring surgery.

Injured was operating scoop tractor to clean up bottom coal when a piece of rock fell from the rib striking injured on his right shoulder and arm causing contusions.

Employee was climbing ladder to get into front-end loader when he slipped on a rung hyperextending his left knee. It was raining when the accident occurred.

Employee's left knee started to swell due to repetitive crawling "miner's knee". No incident of an accident was reported.

At approximately 7:00P.M., a mine examiner making his weekly return run found a caved area in L-2 entry, 60 feet inby spad #463. (Spad located in L-1 entry) Roof fall started in the intersection of L-2 entry and traveled part way through the crosscut between L-2 and L-3 entry. Caved area is approximately 20 feet wide,45 feet long, and 5 feet high.

Employee was installing roof bolt. The bolt got stuck in the hole. As he was pulling the bolt out of the hole, he strained his back.

Employee having pain and swelling in his right knee. Diagnosed as "miner's knee", bursitis.

Employee was in a kneeling position roof Bolting the corner of an intersection. He had drilled to the depth of the first drill steel. As he dropped the drill head, it came in contact with his right leg causing a bruise just above the right knee.

Employee was twisting around in an attempt to hang a roof bolter cable across the travelway when he heard a "pop" from his right shoulder. Employee had dislocated his right shoulder.

Injured had just returned to work from a 2 1/2 month absence due to a home related injury to his right shoulder. On his first shift back to work, employee stated that when lifting and throwing a piece of belt structure he felt a pop in his right shoulder.

Employee was lifting and dragging a piece of 42" belt structure off of a skid to place it along the rib for an upcoming belt advance when he lost his grip on the structure and it fell striking him on the back of his right lower leg.

Employee was installing a roof bolt when a slab of roof rock fell from between the last permanent row of bolts and the ATRS striking him on his back and right side ribs causing a contusion and pain.

Employee was carrying a bottom belt idler when he tripped on a cable lying on the bottom causing him to fall. The weight of the bottom roller struck the top knuckle of his middle finger on his left hand causing a fracture.

Injured was operating shuttle car when he hit a bump on the roadway causing the battery of his cap lamp to jam into his side causing sharp pain and a contusion.

Employee stated that he was lifting a continuous miner cable in order to hang it up over an intersection when he felt something pull in his groin area.

Walking into shower room

Employee was tightening the bolt on the Joy 14 Continuous Miner gathering arm using a wrench and a cheater pipe. The wrench slipped off of the bolt head causing the employee to fall forward striking his finger on the gathering arm.

Injured was setting a wooden post (5" x 6" x 60") under a "C" channel approximately 40 feet outby the face area. As the employee reached behind the post and started to move it under the channel, a piece of rock 1" x 6" x 12" fell off the top of the "C" channel striking the employee on his left wrist.

Employee and a co-worker were tramming scoop tractors out the main intake of the mine in close proximity to the drift mouth. The scoop tractors became stuck in soft bottom. The weather that day was extremely cold temperatures. The worker spent too much time in the cold and did not have on insulated footwear.

Injuried was assisting co-workers in cribbing up high spot over beam sets when one of the 32' x 5' x 7" crib blocks fell striking injured on his right foot causing a slight fracture.

AFTER COMPLETION OF INTERSECTION BEING CUT THROUGH AREA WAS COMPLETLY SUPPORTED. SOME ADDITIONAL (EXTRA) LEGS HAD BEEN SET UNDER I-BEAM IN CENTER OF ENTRY. WHEN THESE LEGS WERE REMOVED ROOF STARTED TO DRIBBLE AND END RESULT CAVED.

THE CAVE WAS IN INTERSECTION OF THE 2ND XCUT IN MAIN BELT ENTRY WITH KING RAIL INSTALLED ON BELT ENTRY SIDE. THE MINER TOOK OUT APPROX. A 4' LIFT FROM RETURN SIDE TOWARD BELT TO CUT XCUT THROUGH. DURING CLEANUP OPERATOR OBSERVED ROCK DRIBBING. HE BACKED MINER UP THEN THE INTERSECTION FELL.

Victim was loading roof bolter, victim lifted a box of resin to put on bolter and experienced back pain and trouble breathing.

THE MAIN BELT ENTRY WAS IN APPROX 35' FROM EDGE OF HIGH WALL AND WAS SUPPORTED TO WITHIN 3' OF FACE. THEN MINER WAS TAKEN IN TO TAKE AN ADDITIONAL 6' OUT OF LIFT. ON CLEAN UP ROOF CAME ON BRING 3 PERMENT C CHANNELS WITH IT. ROOF THAT CAME WAS MUDDY AND BROKEN UP.

WHEN ATTEMPTING TO LOAD AN I BEAM ON LOADER FORKS THE BEAM ROLLED OVER ON ANOTHER BEAM CATCHING VICTIMS LEFT POINTER FINGER BETWEEN BEAMS SMASHING IT.

On initial start up 8' lift was taken and channels and bolts were installed to within 3' of face rain was contributing factor to fall erosion on high wall this fall was in belt entry of main heading.
