Employee pinched hand between the head of roof bolter and roof bolt plate. He missed one (1) day due to this injury. This occurred over the Thanksgiving/deer season holiday of which we were idle. We returned back to work on November 28, 2007. His day missed was on Friday, November 23, 2007 only.

Employee twisted lower back while setting fibercribs. Employee did not miss any work however after seeking treatment for ongoing pain the physician placed him on restricted duty effective 12/12/2007 now making this report classified as a medical.

Employee was operating scoop, when he ran over a hump on the roadway, thus causing him to strike his head against a roof bolt and caused a laceration. Employee did not miss any work, however he did receive sutures, making this a medical report.

Employee was servicing his front end loader when he slipped and fell to the ground injuring his lower back and groin area.

Employee was running levels. After doing surveying the employee started to duck walk due to sore shoulders when his right knee popped and partially locked up. Employee continued working until February 28, 2008 at which time surgery was performed to repair a meniscus tear of the right knee now making this a reportable.

An unplanned roof fall occurred in the #1 entry of South Mains 1 x-cut inby survey station #98. Fall was approximately 18' wide, 18' long, and 4'plus high. Area was bolted with 4' fully grouted resin bolts with 5 rows across per the approved Roof Control Plan. Area was timbered and dangered off. This did affect the travelway.

An unplanned roof fall occurred in the #2 entry of 2 Left, 2 x-cuts inby survey station #325. Fall was approximately 19' wide, 24' long, and 4' plus high. Area was bolted with 4' tension re-bar bolts with 5 rows across per the approved Roof Control Plan. Area was timbered and dangered off. This did affect the travelway.

An unplanned roof fall occurred in the #5 entry of 1 Right Mains at survey station #682. Fall was approximately 19' wide, 25' long, 4' plus high. Area was bolted with 4' tension rebar bolts with 6 rows across per our approved Roof Control Plan. Did not affect any travelways, ventilation or equipment. Area was timbered and dangered off.

An unplanned roof fall was found at approx. 9:20AM at survey station #307. Fall was approx. 18'W., 35'L. and 4' plus high.Fall did not affect any ventilation, equi. or travelway. Area was timbered and dangered off. Fall area was bolted with 4' tension re-bar bolts with 5 rows across according to the approved Roof Control Plan.

Employee was operating a Damascus Personnel Carrier. He went to back Personnel Carrier up into a nearby x-cut to get out of the way of an incoming Scoop and while doing so the employee jammed his head against the roof. Employee did not miss any work, however he did seek medical treatment making this a reportable.

An unplanned roof fall occurred in #5 entry of the South Mains, 2 x-cuts inby survey station #227. Fall was approximately 18' wide, 55' long, and 4' plus high. Fall did not affect any ventilation or travelways. Area was timbered and dangered off. Area was bolted with 4' tension rebar bolts with 5 rows across. Fall will not be cleaned up.

An unplanned roof fall occurred in the #4 room of 1 Right Room, 2 X-cuts inby survey station #691. Fall was approximately 19' wide, 30' long, 4'plus high. Fall did not affect ventilation, equipment, but did affect travelway. Fall was timbered and dangered off. Roof fall will not be cleaned up. Fall was discovered at approximately 11:15PM.

An unplanned roof fall occurred in the #3 entry of the 1 Right Room, 1 x-cut inby survey station #504. Fall was approximately 19' wide, 22' long, 4' plus high. Fall did not affect any ventilation, equipment, or travelway. Fall was timbered and dangered off. Roof fall will not be cleaned up. Fall was discovered on 05/30/2007.

An unplanned roof fall occurred in the #3 room of 1 Right Room, 1 x-cut inby survey station #569. Fall was approximately 19' wide, 25' long, and 4' plus high. Fall did not affect any ventilation, equipment or travelway. Fall was timbered and dangered off. Roof fall will not be cleaned up. Fall was discovered on 05/30/2007.

An unplanned roof fall occurred in the #4 entry - 2nd intersection inby the mine opening. Fall was approximately 11 feet high, 19 feet wide and 38 feet long. Area was bolted with 5 foot fully grouted resin bolts along with 6 foot tension re-bar bolts and 10 foot cable bolts. Fall was on intake escapeway so fall was cleaned up.

An unplanned roof fall occurred in the #5 entry of 1 Rt. Rooms 1 x-cut outby survey station #668. Fall was approximately 25' long, 19' wide, 4' plus high. Area was bolted with 4' tension rebar bolts with 6' tension rebar bolts spotted through the area along with straps. Did not affect any equipment, ventilation or travelway. Fall will not be cleaned up.

An unplanned roof fall occurred in the No. 3 entry of 1 Right Room at Survey Station No. 639. Roof fall was approximately 19' wide, 45' long, and 4' plus high. Area was bolted with 4' tension rebar bolts according to the approved Roof Control Plan. Did not affect any equipment, ventilation or travelway. Area was timbered and dangered off.

While employee was performing his normal duties as a Scoop Operator in low coal the employee experienced swelling in his left knee. Employee continued to work until 04/09/2007 at which time he started missing work making this a reportable.

An unplanned roof fall occurred in the 2 Right E Butt section, in the #3 entry (belt entry). The fall was approximately 90 feet long and 4' plus high, between Station Spads #922 and #927. The fall did not affect ventilation. MSHA officials were notified as soon as the fall was discovered, and the place was timbered and dangered off.

An unplanned roof fall occurred in the #4 entry of Second Left inby survey station #403 1 crosscut. Fall was approximately 19' wide, 20' long and 4' plus high. Area was bolted with 4' tension re-bar bolts per the approved Roof Control Plan. Area was timbered and dangered off. The fall changed the normal route of travel. Did not affect any ventilation.

An unplanned roof fall occurred in the 2 left #1 room set-up 1 x-cut inby survey station #288. Fall was approximately 30 feet long, 20 feet wide, 4 feet plus high. Did not affect any ventilation but did change the route of travel in the air course run. Area was timbered and dangered off.

An Unplanned Roof Fall Occurred In The #15 Room Of 2nd Left, 1 X-cut Inby Survey Statiion #388. Fall Was Approximately 5' High, 0-18' Wide, 20' Long. Fall Area Was Timbered And Dangered Off. Travelway Was Re-routed 1 X-cut Outby.

An unplanned roof fall occurred in the #4 entry of the 2 Left Rooms 2 x-cuts inby survey station #420 and passed through x-cut to the #3 entry. Roof fall was approximately 55' long, 18.5' wide, and 4' plus high. Fall did change our course of travel in the return. Area was dangered and timbered off.

A roof fall occurred in the 2 Left Section. It occurred in the No. 4 Entry, 1 X-cut inby Station #254. Fall was approximately 6' - 8' high, 19' wide, 65' long. Fall caught high voltage cable along with the required lifeline. Area was timbered and dangered off. Escapeway was re-routed along with the required lifeline.

A roof fall occurred in the South Mains, No. 2 Entry, approximately 2 X-cuts inby Station #245. Fall was found during the pre-shift examination. It was approximately 6' -8' high, 19' wide, 19' long. Roof fall caught A 30HP Flygt pump. Fall was timbered and dangered off. Will not be cleaned up.

While performing his normal duties as a maintenance supervisor over a period of time the employee experienced pain/numbness in both hands. Employee was later diagnosed with carpal tunnel syndrome. He continued to work until 09/21/2006 at which time he went off to have surgery performed for the right hand and will have the left hand done at a later date.

Employee was helping to belt up - while moving belt structure employee experienced pain in upper back.

Employee was loosening a bolt when a burr off of the bolt struck his eye. Employee did not miss any work however the course of treatment makes this report a medical.

While performing his normal duties as a roof bolter/MBC operator in low coal, employee experienced pain in left knee. employee continued working until 6/7/06 at which time he started missing work. Employee was diagnosed with prepatella bursitis.

Employee was using a die grinder to remove a metal burr and apparently a fine piece of metal blew into right eye. He did not miss any work however the course of treatment makes this report a medical.

EE was riding with crew into section on a long-airdox personnel carrier when he struck a roof bolt with his hard hat wrenching neck.

An unplanned roof fall occurred in the #5 entry of 1 left. Fall was approximately 18' x 60' x 4' plus high. Area was bolted with 48" fully grouted resin bolts with 72" tension rebar bolts spotted throughout this area. Did not affect any equipment or ventilation. Will not be cleaned up.

An unplanned roof fall occurred in 1 West Room in x-cut #3 to #2, 3 x-cuts inby Survey Station #156. Fall was approximately 4' high, 18' wide, 40' long. Area was bolted with 42" fully grouted roof bolts according to the approved roof control plan. Area timbered and dangered off.

While employee was re-positioning roof bolter to start the next inby row of roof bolts he trammed roof bolter over top of left foot.

The employee was positioning himself in order to tear out a bottom structure stand when he suddenly experienced sharp pain in his left knee.

WHILE UNLOADING X-BARS EMPLOYEE CAUGHT FINGERS BETWEEN X-BARS AND SCOOP CAUSING LACERATION. EMPLOYEE DID NOT MISS ANY WORK HOWEVER HE DID RECEIVED SUTURES THEREFORE MAKING THIS REPORT A MEDICAL.

An unplanned roof fall occurred in the #2 entry of south mains (mmu 001-0) at our No. 5 mine. Fall is located 2 x-cuts inby survey station #74. Fall area is approximately 20' wide, 25' long, 9' to 10' high. Area bolted with 42" fully grouted resin bolts according to the approved roof control plan. Fall will be cleaned up. Area posted and dangered off.

Employee was raising the head of the roof bolter to attempt to couple back onto the drill steel that was stuck in the roof when his finger got between the drill head and drill steel.

While emlployee was crawling to the next place to bolt ee experienced pain in right knee. EE returned to work the next scheduled shift, however he started missing work on 3/31/05 making this an LTA.

AN UNPLANNED ROOF FALL OCCURRED IN THE #3 ENTRY (BELT ENTRY) OF SOUTH MAINS, 1 XCUT INBY SURVEY STATION #8. ROOF FALL WAS APPROXIMATELY 10' HIGH, 20' WIDE, 35' LONG. ROOF FALL DID NOT AFFECT ANY EQUIPMENT OR VENTILATION. AREA WAS BOLTED WITH 5' FULLY GROUTED RESIN BOLTS WITH 10' CABLE BOLTS INSTALLED AS SUPPLEMENTAL SUPPORTS ACCORDING TO THE APPROVED ROOF & RIB CONTROL PLAN.
