Employee was using a hand held electric grinder with a wire brush to clean parts for a crusher rebuild. The grinder kicked to the side catching glove and wrist. The brush came into contact with the employee's wrist causing a cut that required 5 stitches

Two employees carrying piece of metal 2'x6' weighing approximately 100 to 150 lbs up a set of steps approximately 5' high. Installing metal box. One of the employees complained of a sore back refused medical treatment. On 1/19/08 employee decided did need medical treatment. Was determined had strained muscle.

EE was attempting to move a flop gate to change direction of material flow. The gate got away fro him and caught his right ring finger between the gate and side of the box. The mashing injury removed the skin on the bottom side of the right ring finger from the first joint to the tip of the finger.
Dismounting cat d8n dozer. as ee stepped onto the push arm, his left foot slipped between the push arm and drive case. ee received a contusion to the left thigh.

EE REMOVED HOSE & FITTING FROM AIR STARTER ON HAUL TRUCK USING PIPE WRENCH. HE SAID ARM STARTED HURTING LIKE HE MAY HAVE PULLED A MUSCLE. ON 4/1ARM KEPT HURTING SO HE WENT TO EMERGENCY ROOM. 4/2 INFO ONLY REPORT FILED BY LOCATION TO INVESTIGATE. 4/3 WORKER COMP CONFIRMED INJURY TORN BICEP TENDON NEEDING SURGERY.
