Employee alleged that he suffered an injury on this date. He claims that he was using a weed trimmer and the string got caught in the conveyor belt and pulled him into the belt. He did not report any injury on this date nor on the next day when the contractor (Green Thumb, Contractor # VZT) terminated him. He sought medical treatment later that day and no injuries were found.

HE WAS INSTALLING A BOARD OVER A LOOSE ROOF PLATE WHEN A PIECE OF ROCK FELL AND STRUCK HIM ON THE NOSE, RESULTING IN A LACERATION THAT REQUIRED SUTURES.

A PIECE OF HIGHWALL FELL OFF PULLING 4 ROOF BOLTS AND CRUSHING THE BELT PORTAL CANOPY AND BELT STRUCTURE. THE ROCK WAS APPROX. 14' X 5'X 12'. IT FELL DURING AN IDLE SHIFT.
A ROOF FALL WAS FOUND DURING A PRESHIFT EXAM THAT MEASURED APPROX. 70' X 20' X 3-4' THICK. IN THE #4 ENTRY, 150' INBY SPAD #54. 4' RESIN BOLTS WERE USED IN THIS AREA. THERE IS A COAL RIDER SEA M THAT RUNGS 12" TO 36" IN THIS AREA.
