Miner was cutting banding off a pallet with a knife, it slipped and stuck in EE's hand.

Person was on top of dry bulk tank to close lid. Feet slipped.

Operating bag machine. A bag ruptured spraying hot magnetite into left eye.

Standing on electric motor (This motor was locked out but he should not of been standing on it) reached for a hammer slipped off. Only about 12" off floor.

Claimant and ee went to get hose from water truck. Pulling the water hose claimant felt a "pop" in his back.

I stepped out of my truck and was walking around to the back of the truck when I stepped on a piece of scrap metal that was laying on the ground. I twisted my right foot and ankle.

Pulling self up into cab of delivery truck, felt a painin left groin.

Worker lifted 5 gal bucket of liquid soap causing a hernia. Worker visited doctor 4/3/09, refered to surgeon 4/7/09, had surgery 4/15/09.

EE WAS STARTING TO GET IN ENDLOADER, HIS FOOT SLIPPED AND HE FELL ON HIS LOWER BACK. IT HAD JUST STARTED RAINING.
