An unplanned roof fall occurred on 12/26/25 at 11:59 am at 34 3/4 crosscut on 5 north #1 entry. The fall length was undetermined at the time of the fall. The fall was dangered off and will be cleaned up and bolted.

Employee was assisting with removing shuttle car tire when EE had EE's hand behind the tire, the tire shifted pinching EE's finger. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Employee was standing near the LW headgate watching a job, when a rock fell from between straps and struck the back of their ankle. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Employee was helping work on belt pulley when EE said EE was carrying chain fall and felt pain in EE's right chest muscle. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

While walking on the beltline, a rock fell from the top and struck the back of the employees neck and then landed on EE's hand.

Employee was working on locomotive when lid lock let off and pinched arm between motor and cover. PPE was not an issue, mining systems was not an issue, compliance with rules and regulations was not an issue.

Employee was helping unload short sticks of rail at shanty when one rail rolled and pinched finger in between another short stick. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Employee was loading beams onto crib scoop when beam rolled off and fell on a finger. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Employee was bolting when bolt came down out of the hole and plate struck their lip. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

At approximately 3:02 the Blake Ridge elevator went down due to governor switch had grease from the elevator ropes that set the governor switch off. One surface electrician came to clean the switch and the elevator ran as normal. The elevator was up and running at 3:34 pm.

An unplanned roof fall occurred on 9/29/2025 at 10:19am at 15 1/2 crosscut in 6 North 3 East #2 entry. The roof fall measured 16' wide by 8' high and approximately 14' long. The fall was dangered off and will not be cleaned up.

Employee was changing rollers when EE came into contact with a piece of roof screen on the middle of EE's back. PPE was not an issue, mining systems was not a factor, compliance with rules and regulations was not a factor.

Employee was center bolting when EE went to remove steels from roof, one of the steels came out of roof and hit finger. PPE was not a factor, mining systems was not a factor, compliance was not a factor, hand placement was a factor.

Employee was climbing down off of 777 dump truck, when they slipped off of bottom rung on ladder. They felt a pop in knee. Employee worked after the injury until March 4th 2026, when they began missing time for surgery.

An unplanned roof fall on 8/27/25 at 3:25pm on Mainline 12 track 14xc. The roof fall measured 16' wide by 30' long and 10' high. The fall was dangered off and will be cleaned up.

An un-planned roof fall occurred on 8/24/2025 at 12:48am on Mainline 15 track 13xc. The roof fall measured approximately 16' wide by 50' long and 10' high. The fall was dangered off on both sides and will be cleaned up.

Employee was removing crib from under the pan. Pan came off of scoop and struck their left hand, crushing their fingers

A 4'x4'x 1/4'' steel plate guarding fell (old piece being removed) pinching the employees finger between the plate and backhoe slope mucker handrail.

Employee was z drilling and went to bring the bolt up, when the spin got caught. The drill head and cylinder snapped off and hit their right hand.

Employee was hanging miner cable and water line onto roller hanger when they felt pop in the middle of their back. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Employee was attempting to assist to move power center cars. Employee was walking into the track spur and slipped, tripped and fell over a piece of pipe. At approx 2:57am on 7/23/25 an unplanned roof fall occurred at 15-17xc on the haulage. The roof fall measured approx 16'w x 10'h x 40'l. The fall was dangered off on all side and will be cleaned up.

Employee was attempting to lower pipe from the rib line. The EE let the pipe down to fast and injured EE's left ribs. The cause of this incident was improper body mechanics. PPE was not a factor, mining systems was not a factor, compliance with rules and regs was not a factor, miner proficiency was a factor

Employee was unloading a beam from the scoop. When they went to set the beam down, the rib rolled and large pieces of material struck them in the back, neck and back of their head.

Employee was working at the mainline # 13 belt drive when they were injured. Employee was attempting to lift a vulcanizing plate into position when they experienced pain in left shoulder.

Employee was driving a scoop, parked it, got out and started carrying beams. They then proceeded to walk up the face with a beam on their shoulder, when taking the beam off their shoulder the person carrying the other end of the beam spun the beam when setting it down, which caused the injury.

On 7/2/2025 an unplanned roof fall occurred at 13-37xc track to neutral entry. The fall was approx 16'w x 50'L x 10'H. The fall was cribbed off and won't be cleaned at this time.

Employee was assisting with lifting chain barrel onto car when they cut their hand on metal ring at bottom of barrel. Mining system was not an issue, compliance with rules and regulations was not an issue. PPE was an issue.

An unplanned roof fall occurred on 6/28/2025 at 3-17xc. The fall was 10'-15' wide 20'long, and height was undetermined. The fall was flagged off on the out by side and will be cleaned up.

An unplanned roof fall occurred on 6/26/2025 at 3-19 xc. The fall was 10' wide by 10' high and 40' long. Both sides of the fall were flagged off and fall will be cleaned up.

Employee was greasing head gate sprocket when rib roll pushed EE into side of headgate causing EE to cut EE's hand. PPE was not a factor, mining system was not a factor, compliance with rules and regulations was not a factor.

An unplanned roof fall occurred on 6/24/25 at 3-18 xc on the track. The fall was 20'-25' long by 16'wide and 10'-15' high. Both sides of the fall were flagged off and fall will be cleaned up.

An unplanned roof fall occurred on 6/10/2025 at 14-22 crosscut track to belt. The fall was 20' long by 16' wide 8-10' high. 14-22 was flagged off on the track side and will be cleaned up.

Employee was setting a jack when they swung sled hammer to tighten, they pinched a finger between the handle and the jack. Mining system was not an issue, compliance with rules and regulations was not an issue. PPE was an issue.

Employee was dragging dust hose on belt line when it snagged, EE slipped and fell injuring EE's shoulder. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Employee moving air drill to bolt when EE felt pop in EE's lower back. PPE was not a factor, mining system was not a factor, compliance with rules and regulations was not a factor.

Employee was installing cable bolt when EE lifted bolt into the hole EE felt pop in shoulder. PPE was not a factor, mining system was not a factor, compliance with rules and regulations was not a factor.

Employee was center bolting when EE's bolt wrench broke and hit EE on EE's forearm. PPE was not a factor, mining system was not a factor, compliance with rules and regulations was not a factor.

Employee reports that they were packing monorail on the belt line in 6N3 39 1/2 X-Cut. The monorail EE was carrying hit the belt; they grabbed it and went to reposition it on their shoulder. When it hit the existing monorail, it left the piece EE was holding hanging and shoved it down onto their shoulder. Employee then felt a pop in mid-back and sharp pain.
