Employee was cutting the strap off of a pallet of roof bolts when the strap came back and hit his right arm causing a laceration.

An unplanned roof fall was discovered in the #3 entry approximately 9,300 ft. from #1 section. No equipment or miners were involved. The fall measured 20' X 20' X 7' high.

A roof fall wis discovered by the mine examiner at spad #6455 approximately 2600' outby the faces. Fall measured 25' long x 19' wide x 5' high. No personnel or equipment were involved. Area will not be rehabilitated.

Member was cutting a piece of belt with a utility knife when the knife slipped and cut his left hand.

An unplanned roof fall occurred in the 3rd southeast sub mains located 160' from spad #6181 and 11,680' from the section face.

Member was operating the offside of the right roof bolter in 5 right. He went to remove his drill and a piece of draw rock fell. The rock hit him on his right arm, and bounced and hit him in the chin causing a laceration.

Member was standing in the #7 entry talking to the miner operator when a small piece of draw rock fell and hit him. Note: The Dr. did not take member off from work until 11/6/08

Member was operating a stinger when the right side tire hit a rock causing the steering knob to spin and hit the members right hand.

An unplanned roof fall above anchorage in the 1st southeast mains #2 entry at spad #4788 3500' from the face.

An unplanned roof fall above anchorage 1600ft outby the face between #2 and #3 entry at spad #6250.

Member was squaring up the belt to make a belt splice, when the utility knife that he was using slipped and cut his right thigh.

Unplanned roof fall above anchorage on the #1 section #4 entry at spad #6269.

The crew had loaded the mantrip and started outside. The mantrip hit a jack that had been unloaded beside the track. The jack came around and hit employee in the left forearm causing a laceration.

An unplanned roof fall was discovered app. 1,200 ft. from the active section 1 x-cut inby S.S.#6088. The fall measured app. 32' in length X 19' in width X6' in height. The area was bolted with 6' torque tension bolts. The area is dangered off and will not be rehabilitated. MSHA inspector investigated the fall.

Member was getting out of a scoop and had his right hand on the corner of the canopy, when a piece of draw rock fell, stiking him on the right hand.

An unplanned roof fall occurred in the # 4 entry at survey station #5797 appr. 3,000 from the active section. The fall measured 22' in length X 20' in width X 6' high. The area was bolted with 6' torque tension bolts and has been dangered off. The area will be rehabilitated.

On 4-18-08 EE ceased work & went to ER w/back pain. Referred to chiropractor, he was dx'd w/a simple spasm & released to return to work. EE was placed in a different position soon after. He later went to a chiropractor & was sent for MRI on 5-4-08. Results were interpreted as degenerative. He worked till 6-9-08. At that point he had a dr slip stating unable to perform job

EE stated that a piece of rock fell from the rib and hit his right leg in the calf area causing a contusion and a sprain to his ankle. He took off and has not called or provided any dr. reports as to a diongnosis. This report is being filed as a reportable injury with no days lost till a determination can be made as to the reason for injured being absent.

Member stated that while bolting top he thought he got something in his eyes. He finished his shift and went to the eye doctor on Monday, 3/24/08. The doctor washed his eyes out and prescribed him anti-biotic drops.

A unplanned roof fall occurred in the # 4 entry on the 001 section at S.S. # 5694. The fall was 1250 ft. from the face. The fall will be rehabilitated and the size was 30ft in length X 19ft. in width and app. 10 ft. in height. The area was inspected by M.S.H.A inspectors and State.

EE was installing a jack on a roof bolter and while taking off a hydraulic fitting his hand slipped and he stated he hit something sharp causing a laceration to his right index finger. He recieved 4 stiches.

A roof fall measuring 60' in length X 17' in width X 4' high was discovered on a routine weekly exam. 4,400 ft. outby the working section. This fall will be rehabilitated and has been dangered off.

Employee got his hand pinched between a rocker arm and boom while changing a pin on a roof bolter. He received sutures.

Employee was cutting a piece of CO line with utility knife and slipped causing him to cut the top of his left hand. He received 4 stitches.

EE contacted mine management stating he was going to the Dr to have his hand checked. He stated he thought he had gotten a splinter in his hand a few days before. EE reported to management on 11/9/07 that he was in the hospital and had a foreign object removed and was staying in the hospital overnight for observation on which meds work best on injury.

EE was installing a boom lift jack and mashed his finger.

Employee was assisting maintenance foreman wire a belt drive. While using a utility knife to cut the cable he slipped and cut his left hand. He received 4 stitches.

An unplanned roof fall at and above anchorage of original 48" fully grouted roof bolts. The fall occurred at the No.1 Belt at brk 11, S.S. #3921. This fall was reported immediately to State and MSHA agencies.

Employee stated he was picking up a can of oil and a small piece of rock fell from the roof striking him on the right hand.

EE was installing a roof bolt and strap when a piece of draw rock fell striking him on the head and back.

EE stated he was bending a roof bolt and felt a severe pain in his lower abdoman.

Employee stated that he was pulling himself up in the seat of the lo-trac and hit the tram pedal and got his hand caught between the tire and frame of the machine.

Member was welding on a bit lug on a continuous mining machine. Resulting in a burn to both his eye. was not reported until 3/15/07

Employee stated he was using the boom of the bolter to push bolt into a hole and his thumb got caught between the bolt head and the boom causing a crushing injury to his left thumb.

Employee was removing a guard from a Joy continuous miner and while he was laying the guard on the ground his glove got caught in the handle causing him to fracture his right ring finger and middle finger.

Employee was on his knees in a scoop bucket throwing crib blocks from the scoop to the back of the feeder. Employee ceased work and requested an ambulance.

Member was operating a continuous miner in the 4 left crosscut when a rib roll measuring 9' long x 42" wide x 1" to 5" thick struck him resulting in a fractured right foot.

Member was getting on mantrip on the surface and triped over the track switch that was covered with water injuring his left knee. 1st day away from work 11/2/06.

An unplanned roof fall was discovered at 34 cross cuts inby the portal on the #1 belt. This mine is in non producing status. Time of fall is unknown. Fall measured approximately 25' long x 30' wide x 10' high.

Employee was making a belt move up. The scoop operator was pulling the tailpiece up. One of the chains slipped causing the tailpiece to go side-ways catching employee between the rib and the tailpiece.

He was installing a roof bolt when a piece of rock fell striking his left arm resulting in a laceration requiring stitches. Employee continued working until 8/16/06 and was taken off work.

EE was operating a shuttle car traveling toward the miner. The shuttle car struck a hole in roadway causing his right arm to hit the hand hold on the canopy injuring his right elbow.

Was loading 8" I beams. I beam slipped mashing member's finger between two of the beams cutting his left index finger.

While loading supplies, employee stated he felt pain in his lower back. He didn't start missing work until 8/16/2006.

Member was adjusting the cat pads on the miner when a chain rachet slipped off of shim and end of chain rachet struck him on the left cheek.

The intersection fell due to ryder seam just above it. 20' wide, 20' long, 6' thick.

A ROOF FALL OCCURRED AT LOCATION #3 ENTRY 1 BREAK INBY SPAD 3772 WITH AN INACTIVE FACE AREA. THE FALL WAS 65' X 19' X 8' TO 10' IN SIZE. SUPPORT WAS 4' RESIN BOLTS IN MIXED STRATA WITH NO RIDER SEAM. THERE WAS NO PERSONAL INJURY OR MATERIAL DAMAGE. THIS AREA WILL BE BREAKERED OFF AND NOT REHABILITATED.

A roof fall occurred at the above noted mine at locationg-1 break inby spad 2375. Face is inactive. no personal injuries occurred. the fall measured 18'x 18' x 7' in size with torque tension bolts as support in shale. there is rider seam of approx. 24" x this area will not see rehab.

A roof fall occurred at the above mentioned mine. exact time is unknown. location was spad 2386-face inactive. no personal injuries. the size of the fall was approx. 25' x 18' x 7" supported by torque tension bolts, strata being wet shale, and also has a 2 ft rider scam, this area will see rehab.

A roof fall occurred at Diamond Energy possibly the day of 5/17/2004 or before. The time is unknown. This fall occurred in the area of spad 2385 in an entry between headings 3 and 4. and measured 16' x 18' x 6'. and was originally secured with 8' cable bolts. This area is being rehabilitated as part of it is in the track entry area.

A ROOF FALL OCCURRED AT THE ABOVE MENTIONED MINE AT ENTRY #2 1 BRK INBY SPAD 2329. THE SIZE OF THE FALL WAS 70 FT X 18 FT X 8 FT. SUPPORT CONSISTS OF RESIN BOLTS, SHALE ROOF WITH A 14" RIDER SEAM ABOVE AT 6 FT. THIS AREA WILL NOT BE REHABILITATED.

A ROOF FALL HAS OCCURRED AT THE ABOVE MENTIONED MINE LOCATION OUTBY IN ENTRY #2 (SPAD # 3830 - 1 BRK INBY) AT 1600 FT FROM THE FACE. THE SIZE OF THE FALL WAS 18' X 19' X 7 FT. RESIN BOLTS WERE USED IN MINED STRATA WITH (NO) RIDER SEAM. THIS AREA WILL SEE NO REHAB.

EE WAS HELPING AN ASSOCIATE PULL HIGH LINE TO GO TO #12 TRANSORMER. WHILE PULLING HIGHLINE, HE SLIPPED INT HE MUD AND FELL ON THE RING FINGER OF HIS RIGHT HAND.

ROOF FALL AT SPAD #2308 IN THE BELT/TRACK ENTRY. NO ONE WAS INJURED. THE SIZE OF THE FALL IS 40' X 20' X 6'. 6' TORQUE TENSION BOLTS WERE USED IN SHALE STRATA WITH A RIDER SEAM 8' ABOVE WORKIN G SEAM. THIS AREA MUST BE REHABILITATED.

EE WAS MOVING A SCOOP CHARGER & A CABLE. HE PUT IT ACROSS THE BELT (ON THE BELT) INSTEAD OF HANGING IT ACROSS (OVER) THE BELT WHICH HE SHOULD HAVE DONE. THE CREW STARTED THE BELT. THIS ACTION PULLED THE CABLE & PULLED HIM TO THE GROUND ON HIS RIGHT ELBOW RUPTURING HIS BURSA IN THE ELBOW.

INJURED WAS IN PROCESS OF MAKING A VULCANIZED SPLICE IN A CONVEYOR BELT. HE WAS STANDING ON A PIECE OF PIPE, WHICH WAS LAYING ALONG THE BELTLINE, WHILE TRYING TO CUT THE BELT WITH A UTILITY KN IFE. INJURED SLIPPED OFF OF THE PIPE & STRUCK HIMSELF IN THE LEG WITH THE UTILITY KNIFE.

EE WAS IN THE PROCESS OF LIFTING A BELT ROLLER. HIS INTENTIONS WERE TOGET IT TO THE OTHER SIDE OF THE BELT WHERE THE TRACK IS SO THAT IT COULD BE TRANSPORTED ON A FLAT CAR. HE LIFTED THE ROLLE R TO HIS WAIST LEVEL AND EXPERIENCED LOWER BACK PAIN. HE WORKED 3 MORE SHIFTS IN THE WEEK AND BEGAN MISSING WORK THE FOLLOWING WEEK. THE SAFETY DEPT WAS NOT INFORMED OF THIS UNTIL 11-07-02.

EE HAD INSTALLED THE FIRST BOLT IN THE "NEXT TO LAST ROW" OF BOLTS. HE SWUNG HIS BOLTER IN TO INSTALL THE INSIDE BOLT WHEN A ROCK FELL AND HIT EE ON THE FOOT. THE ROCK MEASURED 40" BY 40" BY 3 " THICK. THIS REPORT IS LATE DUE TO AN OVERSIGHTIN THE SAFETY DEPT.

AT #6 BELT HEAD LOCATION, EE WAS INSTALLING AN ELBOW IN A 4" WATER LINE. AS HE WAS DOING SO, HE STOOD ON THE LINE. HE SLIPPED OFF THE LINE, FELL ON BOTH OF HIS KNEES AND HAS NOT RETURNED TO WO RK AS OF THIS DAY BEING 9-11.

EE WAS TRAMMING MINER. HE ALLEGED HIS FOOT SLI0PED ON MINE FLOOR AND STRAINED HIS BACK.

A ROOF FALL OCCURRED AT THE ABOVE MENTIONED MINE WITH NO PERSONAL INJURY AT THE SPECIFIC LOCATION LISTED, BEING #4 ENTRY SPAD 2834. THE FALL OCCURRED OBSTRUCTION THE TRACK ENTRY AND THE INTAKE , AND WAS ABOUT 8' HIGH. THIS AREA IS TO BE REHABILITATED, REASSESSED FOR SAFE PASSAGE.

EMPLOYEE APPLIED FOR PNEUMOCONIOSIS BENEFITS AND HAS BEEN APPROVED BASED ON EMPLOYEMENT HISTORY. HE IS TO BE REFERRED TO THE OCCUPATIONAL PNEMOCONIOSIS BOARD FOR EVALUATION. IN THE EVENT THAT HE DOES NOT RECEIVE ANY BENEFITS DUE TO INELIGIBILITY, AN AMENDMENT WILL BE SUBMITTED. (NOTE: RECEIVED IN THE SAFETY DEPT. 9/19/02).

A ROOF FALL OCCURRED AT THE DIAMOND MINE 5400 FT FROM THE FACE, AT #6 ENTRY, SPAD 2802, SECT 001. THE SIZE OF THE FALL WAS 60' X 19' X 7', AND THE ROOF STRATA CINSISTS OF SHALE WITH NO RIDER S EAM. THIS AREA WILL NOT BE REHABILITATED.

EE WAS MAKING A PERMINATE SPLICE IN A ROOF BOLTER "CABLE" WHEN HIS UTILITY KNIFE SLIPPED & CUT THE LEFT THUMB. HE RECEIVED STITCHES FOR HIS LACERATION.

EE WAS INSTALLING THE OUTSIDE BOLT IN THE LAST ROW OF BOLTS. AT THIS TIME A PIECE OF ROCK FELL, HITTING THE CANOPY, SLIDING FROM OFF THE CANOPY.HITTING EE IN THE RIGHT SHOULDER AND NECK. THE ROOF BOLTER WAS EQUIPPED WITH MASSEY ENERGY "IRONING BOARD" FLAPS. THE ROCK MEASURED 2' BY 2' BY 3".

EE TRIPPED OVER A METAL STAKE IN THE GROUND, AND HE FELL ON A PIECE OF BELT STRUCTURE, CUTTING HIS LEFT ARM. HE RECEIVED STAPLES FOR A LACERATION.

EE WAS GOING TO SECTION FEEDER IN A LOADED SHUTTLE CAR WHEN HE HIT SOME BAD BOTTOM IN MINE FLOOR. HE SAID IT CAUSED HIM TO BE EJECTED OUT OF THE 21SC ONTO MINE FLOOR.

A ROOF FALL OCCURRED AT THE ABOVE MENTIONED SITE 120' FROM THE FACE AREA ON SECTION. ENTRY #4 SPAD #3170. THE SIZE OF THE FALL WAS 30' X 20' X ?. THIS FALL WAS ABANDONED AND DANGERED OFF (NOT REHABILITATED).

THE CONTINUOUS MINER WAS DOWN WITH NO WATER AT THIS TIME. A PIECE OF ROCK MEASURING 18"X 12"X 1 1/2" FELL FROM BETWEEN ROOF BOLTS. IT STRUCK EE ON THE SMALL OF HIS BACK. EE IS PRESENTLY STI LL NOT WORKING SINCE THE DAY OF THE ACCIDENT.

A ROOF FALL LOCATED AT #2 BELT, #4 ENTRY, BLOCK #26, 6500' FROM ACTIVE FACE. SIZE 20' X 20' X 6 1/2'. TYPE OF STRATA IS SHALE WITH A 40" RIDER SEAM PRESENT. THE AREA WILL NOT BE REHABILITATED.

EE WAS DRAGGING WATER LINE FROM THE BELT ENTRY OVER TO THE SCOOP. AS HE WAS DOING SO HE EXPERIENCED BACK PAIN. HE IS CURRENTLY NOT WORKING AND WE HAVE NOT BEEN INFORMED AS TO HIS CONDITION OF THIS TIME.

EE WAS OPERATING A ROOF BOLTER. HE HAD PULLED A BOLT OUT OF THE HOLE WHEN HE HAD BENT IT, AND THE PLATE HIT HIM IN THE RIGHT ARM. HE RECEIVED STITCHES IN HIS ARM. THIS REPORT IS BEING SENT BECAUSE THE SAFETY DEPT. WAS JUST INFORMED THAT HE RECEIVED STITCHES.

EE WAS INSERTING A PIN BACK INTO THE RUB RAIL OF THE CONTINUOUS MINER. THE REMOTE CORD WAS BEHIND THE RAIL, SO A CO-WORKER MOVED THE RAIL TO REMOVE THE CORD. THE RAIL FELL ON HIS LEFT FOOT AND BROKE HIS TOE.

ROOF FALL OCCURRED IN THE #1 ENTRY SIZE OF FALL WAS 50'LONG, 20'WIDE AND 6'HIGH.

ROOF FALL OCCURRED IN THE #2 ENTRY. SIZE OF FALLWAS 60'LONG, 20'WIDE AND 6'HIGH.

A ROOF FALL OCCURRED ONE CROSS CUT OUT BY SPAD 2343 AT THE ABOVE MENTIONED MINE. FALL MEASURED 70' BY 20' BY 6'. ROOF WAS LAMINATED SANDSTONE. PRIMARY ROOF SUPPORT WAS 6' TORQUE TENSION. RIDER SEAM 18" TO 20" PRESENT 5-6' ABOVE MINING SEAM. AREA WILL BE REHABED.

EMPLOYEE & A CO-WORKER WERE ATEMPTING TO INSTALL A SHIELD COVERING A SCRUBBER MOTOR ON A C/M. A BOLT FELL BEHIND THE SHIELD & VICTIM TRIED TO RETRIEVE IT WHILE AT THE SAME TIME HIS CO-WORKER PUSHED DOWN ON THE SHIELD & IT FELL ON VICTIM'S RIGHT INDEX & MIDDLE FINGERS. (NOTE: THE SAFETY DEPT. WAS NOT INFORMED UNTIL 2/27/02 THAT HIS INDEX FINGER HAD BEEN BROKEN).

EE WAS PULLING ON CONVEYOR CHAIN, ATTEMPTING TO PIECE IT TOGETHER AND IN PULLING, HE EXPERIENCED SHARP LOWER BACK PAIN. HE SOUGHT MEDICAL TREATMENT AND WAS ORDERED TO STAY OFF WORK UNTIL FURT HER NOTICE.

EE ALONG WITH A CO-MEMBER (ELECTRICIAN TRAINEE) WAS LOADING A "HOLD DOWN" ROLLER, WHEN THE ROLLER SLIPPED ON THE MAN TRIP JERKING HIM DOWNWARDS, AS HE WAS ALREADY IN A "BENT OVER" POSITION.

EMPLOYEE WAS HELPING TO WORK IN THE PANEL OF A CONTINUOUS MINER WHEN A RIB ROLL OCCURRED AND ROLLED ON TO HIS BACK, PUSHING HIM TO THE GROUND FLOOR. THE ROCK WAS REMOVED IMMEDIATELY. THE ROCK SIZE WAS 2 BY 3 BY 10 FT LONG.

EE WAS OPERATING A MANBUS TRAVELING INSIDE AN UNOCCUPIED MANBUS COMING FROM SECT.#2 HIT THE FIRST BUS HEAD ON. THE PARK BRAKE OF THE 2ND BUSHAD BLED OFF, ALLOWING IT TO ROLL AWAY AT A DISTANCE OF APPROX. 5,440FT., HURTING HIS L.KNEEAND LOWER LEG. EE IS ON RESTRICTED DUTY AT THE PRESENT TIME.

CLAIMANT HAS BEEN GRANTED A 5% AWARD FOR OCCUPATIONAL PNEUMOCONIOSIS. THIS TRANSACTION TOOK PLACE ON 4/9/02.

EE WAS BUILDING A STOPPING. WHILE BUSTING A CAP BLOCK, A CEMENT BLOCK FELL OUT OF THE PILE ONTO THE BUCK OF HIS HAND. HIS HAND WAS BRUISED THROUGH THE METACARPAL GLOVES HE WAS WEARING.

EE STATED HE WAS LOADING A TOP ROLLER OF BELT STRUCTURE AND FELT PAIN IN HIS LOWER BACK.

EE REPORTED HE WAS HANGING MINER CABLE WHEN HE FELT PAIN IN THE GROIN AREA. HE CONTINUED TO WORK UNTIL OCT 30. HE WENT TO A DR WHO ADVISED TWO DAYS REST. HE RETURNED TO WORK ON HIS REGULAR JOB NOV 3. THE SAFETY DEPT WAS NOT AWARE OF THIS UNTIL CONTACT WAS MADE WITH MINE. AN ACCIDENT REPORT HAD BEEN FILED ON TIME. SUPERINTENDANT DID NOT KNOW WHY MAN WAS OFF. HE TURNED IT IN ON COMPE

EE WAS WALKING IN BATHHOUSE, SLIPPED ON FLOOR & HIT HIS HEAD ON FLOOR. WENT TO HOSPITAL AND GOT 12 STITCHES.

EE WAS APPLYING B-BOND TO A RETURN OVERCAST. HE STATED A PIECE OF DRAW ROCK FELL FROM BETWEEN ROOF BOLTS AND STRUCK HIM IN CENTER OF BACK.

A ROOF FALL OCCURRED LOCATED AT #2 BELT SPADS 2378-2385, 2160' FROM ACTIVE FACE. SIZE 50' X 20' X 6'TO 7'. RESIN BOLTS/TORQUE TENSION BOLTS WEREUSED. STRATA IS MIXED. AREA BEING REHABILITATED.

EE WAS CHIPPING SLAG OFF METAL THAT HAD BEEN WELDED. SLAG WENT UNDER FRAME OF SAFETY GLASSES INTO LEFT EYE.

EE WAS RUNNING SHUTTLE CAR. HE WAS BACKING DOWN AN INCLINE AND HIT A HOLE, JAMMING HIS HEAD UP AGAINST THE CANOPY OF THE SHUTTLE CAR.

EE WAS MOVING MINER CABLE OUT OF THE ROADWAY. WHEN PULLING, HE PULLED AND STRAINED HIS BACK. NOTE- THE SAFETY DEPT WAS NOT NOTIFIED OF THIS ACCIDENT UNTIL THE 18TH OF APRIL. HIS FIRST DAY AWAY FROM WORK WAS APRIL 16TH. EE IS TO BE ON COMPLETE BED REST FOR THE NEXT TWO WEEKS.

EE WAS WALKING FROM #4 ENTRY TO #3 ENTRY WHEN A PIECE OF ROCK (PART OF THE RIB) ROLLED OFF STRIKING & COVERING EE. EE'S RIGHT LEG FROM THE KNEE DOWN WAS SURGICALLY AMPUTATED FROM JUST ABOVE THE KNEE

EMPLOYEE WAS LIFTING STRUCTURE AND PULLED MUSCLE IN BACK. EMPLOYEE DID NOT INFORM COMPANY OF INJURY UNTIL APRIL 27, 2001.

EE WAS SEPARATING STEEL TRACK TIES AS HE DID THIS, A PIECE OF STEEL JERKED OUT UNEXPECTEDLY AND STRUCK HIM ABOVE THE LEFT EYEBROW.

EE WAS WORKING ON CONVEYOR CHAIN ON #2 CM MINER WHEN THE CHAIN BROKE CAUSING PAIN IN HIS RIGHT SHOULDER. **NOTE: THIS NOT DECLARED REPORTABLE UNTIL HE VISITED THE DOCTOR ON 3-21-01.

EMPLOYEE WAS BOLTING AND MISTAKINGLY ALLOWED THE DRILL POD TO COME DOWN ON HIS LEFT INDEX AND MIDDLE FINGERS, BRUISING AND CAUSING PAIN. HE RECEIVED FIRST AID AND MEDICAL TREATMENT.

EE WAS HELPING MAKE A BELT MOVE. A ROLLER STRUCURE FOULED ONTO THE STRUCTURE BELT RAILS & MASHEDEE'S LEFT INDEX FINGER Y

EE WAS SITTING, OPERATING THE REMOTE FOR THE CONTINUOUS MINER. AS HE MOVED TO STANDING POSITION, HE EXPERIENCED PAIN IN HIS LOWER LUMBAR BACK AREA. NOTE: THIS IS DETERMINED AS AN OCCUPATIONAL ILLNESS.

EMPLOYEE WAS PULLING WATER LINE SLACK TO A PRESSURE PUMP. AS HE WAS PUTTING WATER LINE ON HIS SHOULDER, HIS FINGER HIT THE TOP OF THE OVERHANG, THUS CUTTING HIS HAND.

EE WAS TURNING A CROSSCUT IN #3 ENTRY WHEN A PIECE OF ROCK MEASURING 29"X31"X4"THICK, FELL FROM BETWEEN THE RIB BOLT AND THE MIDDLE BOLT, STRIKING HIM ON THE HEAD. IT WAS BOLTED WITH 4'RESIN B OLTS AND WAS CHECKED BEFORE THE MINER MAN ENTERED. EE'S POSITION WAS THE 3RD ROW OF BOLTS OUTBY THE CROSSCUT.
