EE lifting wood blocking while helping to setup a crushing plant.
EE helping clean large loose rock off primary conveyor belt with fellow EE, rock rolled down conveyor belt along frame and hit EE's left hand.
EE parked truck in designated area, when he exited truck, stepped on uneven ground and rolled his ankle.

After installing patches and lacing on a conveyor belt, EE got up to take break and walk around and felt knee start to swell.
Checking water valve, stone came out of the cone crusher and struck EE on top of hard hat pushing it down causing cut on top of scalp.

While carrying across the quarry employee rolled his ankle.
The hydraulic jacks were used to raise the primary jaw to unpin the stationary legs up front. When the EE's up front attempted to raise the stationary leg manually to pull the pin, the EE in back raised the unit in the back unaware of the teetering effect it had on the unit up front. Resulted in the EE up front getting his fingers pinched.

EE WAS USING A CHISEL TO CLEAN DIRT FROM THE JAW, THE CHISEL SLIPPED RESULTING IN A CUT INJURY.
AS EE RAISED HIS ARMS ABOVE HIS HEAD TO PULL ON THE RATCHET WRENCH TO ADJUST THE JAW, HE FELT PAIN IN HIS RIGHT SHOULDER.
EE STARTED HIS FIRST DAY OPERATING LOADER FEEDING THE PLANT. HE BEGAN TO EXPERIENCE BACK PAIN THE SAME DAY. HE WAS RELEASED TO RETURN TO WORK ON JUNE 23RD, 2003 AND QUIT THE SAME DAY.

A PIECE OF STEEL BROKE OFF INT HE DRILL STEEL COUPLING. EE WAS WARING GLOVES AS HE REMOVED THE COUPLING THE COUPLING SLIPPED, HE TRIED TO CATCH IT AND SUFFERED A LACERATION TO HIS RIGHT RING F INGER EE WHEN ON VACATION 8-5/8-9.
EE ATTACHED ONE LEG OF A SPLITTER STRAP WITH SHACKLE TO THE FEEDER TO ADJUST ONE SIDE TO BOLT ITDOWN. WHEN THE CRANE LIFTED THE FEEDER THE LAG END OF ONE SPLITTER STRAP W/ SHACKLE GOT CAUGHT U NDER THE EDGE OF FEEDER, THEN SPRUNG UP. WHEN ITCAME DOWN IT STRUCK EE IN THE BACK RESULTING IN INJ.
THE BANK OF SAND AND GRAVEL SLOUGHED OFF AND COVERED THE FRONT END OF THE LOADER. IT SMASHED THEWINDSHIELD RESULTING IN INJURY TO THE OPERATORS HAND AND ANKLE.

WHILE OPERATING THE LOADER THE CUCKET HIT AND JUMPED OBER A BOULDER CAUSING HIS NECK TO SNAP.
DURING THE DAY APPRENTLY EE GOT DUSWT IN HIS EYE WHICH REQUIRED MEDICAL ATTENTION.

EE WAS CHANGING THE BELT LACING ON DISCHARGE CONVEYOR WHILE PRYING THE OLD LACE LOOSE HIS HAND SLIPPED RESULTING IN LACERATION ON MIDDLE FINGER
EE WAS WORK AROUND THE PLANT WITH THE SKID STEER WHEN HIS EYES. BEGAN HURTING. BY EARLY EVENING HIS EYES WERE RED AND HURTINGS HE THEN SOUGHT TREATMENT.
EE BENT OVER TO LIFT A SMALL PIECE OF CONVEYOR BELT AND GOT A SHARP PAIN IN HIS LOWER BACK.
