Co-worker & EE while on break smoked potpourri otherwise referred to as synthetic marijuana or K.2. After smoking the substance EE passed out resulting in a fall where he struck his head and face on the concrete, 911 was then called. EE was transported to White River Medical Center where he was treated and released to following day.

Miner was trying to remove a rock from the trap on the drum feed belt. The lace on the belt caught the rock as the miner was trying to remove it. This caught his hand between the rock and the trap, resulting in the injury. The injury was a laceration of one finger.
Miner was removing a bearing race from shaft. He had cut the race with a Metabo. After the cut was made, he tried to remove the race by striking with a hammer. At that time a small piece of the race broke off and punctured his lower left forearm. He was wearing a hardhat with safety shield attached at the time.
Employee was replacing a belt on a conveyor when swirling wind blew sand in his eye. Employee was wearing safety glasses at the time. Employee's eye was flushed on-site he was later taken to a local Eye Doctor who removed the sand. Employee was given medicated eye drops to prevent infection.

The employee got off a front-end loader and walked toward the classifier when he slipped on a patch of ice. The employee fell to the ground landing on his back and also hitting the back of his head. The employee was taken to a local hospital where he was treated and released for a possible neck & back sprain.
The employee reported to work on 12-10-10 complaining of a black speck in his right eye.The employee had been performing clean-up activities the day before but stated he did not know how the speck got in his eye.The employee was taken to a local Optometric Physician who removed the foreign material from his right eye and prescripted medicated eye drops to prevent infection.
The employee opened a steel access door/ hatch cover to flip a switch from automatic to manual control on a classifier station. While the employee was flipping the switch the access door/ hatch cover suddenly fell over closing on the employee's left hand. A co-worker helped the employee remove his hand from underneath the door/hatch cover.
EE was assigned to work with a coworker greasing equipment with a grease gun. As the coworker walked by the EE he heard a grunting sound. He turned back & saw the EE face down on the ground & appeared to be having a seizure. EE was bleeding from a cut on his forehead above his right eye from striking the ground that was covered with rocks & gravel.
