Miner was walking from SS to SCD, slipped and fell striking EE's head on the ground. The miner didn't lose consciousness and EE got up and went to SCD where EE reported the fall. Miner initially refused medical treatment then decided to go to the ER for evaluation. Physician removed miner from duty for 1 shift as part of a mild concussion treatment.

A SCD operator was replacing the drive belt on a conveyor when EE's gloved right hand became entangled in the sheave due to not performing lock out tag out. The miner sustained multiple injuries, including a fracture and multiple lacerations, to EE's right hand.

Upon review determined 7K1 was not filed. EE was moving railcars as Groundman. Inertia generated by failed coupling caused trailing railcar to move creating gap between knuckles.EE adjusted knuckle then walked between knuckles verifying hand brake was set. As EE was walking sideways between the knuckles, the rail car returned to the location of the failed coupling, crushing the EE.

While trying to unplug wet stone the 273 feed bin the employee reach into the bin cone to remove some stone that had the consistency of mud. The bin unclogged and dropped hot stone, freshly delivered from the SCD dryer, onto hand. Hot stone went inside the employee's glove and gave parts of hand 2nd degree burns.
A Quarry Tech was driving an empty haul truck down into the quarry along a haul road. When the front driver's side truck tire hit a ~ 4" deep depression in the road the truck gave the Quarry Tech a jolt which jarred neck causing a strain.

The employee was removing the tin end of a condenser containing a calcium crown when employee's tooling slipped and employee hit employee's left forearm against the adjacent crown causing a laceration. The employee was not wearing forearm cut protection as required by standard work.
The employee was pushing bags of lime briquettes into furnace retorts using an ~ 20' pushing rod. The rod caught on the inside of the retort and stopped suddenly causing a sprain/strain injury to the employee's right shoulder.
The employee was working at SCD 2nd floor cleaning rocks that had fallen from the 115 belt. A rock fell from the belt and struck the back of the Employee's hand. The Employee continued to work but on Sunday had trouble making a fist. The Employee was sent to be examined by a doctor on 8/28/17. The doctor prescribed PT and sent EE back to work with restrictions.
A maintenance employee was rolling up 50 foot sections of an old conveyor belt for disposal. After finishing rolling up 3 sections the employee started another section. EE started rolling in the kneeling position for 1 turn then EE stood up and hurt EE's lower back. EE went to a doctor on 4/3/17 who diagnose a muscle strain and reported the injury to management at that time.

An Employee was attempting to remove a stone clog in the SCD screen number 131. EE used three tools to remove the clog; shovel, hoe and steel pipe. During the process ee reached for the shovel, which had fallen to the floor, and stepped on the blade of the hoe kicking the handle up off the floor into face causing left side eye tooth to puncture upper lip.

The employee was lifting 50 lb bags of lime briquettes into our furnace loading area. Over a two day period the Employee started to have shoulder pains that radiated into his chest. The Employee brought himself to an ER where they determined that he had a rotator cuff issue.
A Calcium Department Employee was vacuum cleaning up lime that had accumulated below the briquetting area. When removing the lid of the vacuum system, lime dust particles were released into his face and into his eyes. As a result of getting lime particles in his eye, the employee received a corneal abrasion.
Miner parked his forklift and dismounted onto an uneven surface and rolled his ankle spraining it. He stepped off the forklift facing away from it instead of facing it using three points of contact as he was trained to do.
The Miner went up the stairway to Mill #6 to find the electrician working up there. After finding the electrician, the Miner and the electrician proceeded down the stairway. At a landing the Miner turned right to continue going down. While turning his right boot caught on the grated landing and the Miner twisted his knee.
While connecting a product off-load hose to a rail car filled with lime, the employee slipped on the snow that was in the area. When he stood up, he felt a pain in the lower back.

Operator was working to unplug 273 feed bin. He was working on the bin for over an hour. When material started to flow, he used a sledge hammer to close the hammer gate. He tapped the gate side to side to close the gate. He lost his footing and caught his fingers between the hammer gate and sledge hammer handle. He sustained a broken left index finger and stitches.
New employee was putting heat shield in place. He went to steady a falling shield when the second employee was hitting the shield into place with a pull pin. The new employee reached in front of the shield and the second employee struck his hand with the pull pin.

Using a ratchet wrench with an extension bar was being used to loosen hold down bolts on the chipper blades. The employee was pushing the wrench and lost his grip. His hand struck against the metal frame around the chipper, causing a laceration and fracture to the 4th finger of his left hand.
The employee pushed a crown from the pusher cradle to the roller conveyor. As the crown slid onto the roller conveyor, the employees' hand was at the lower portion of the crown headed toward the roller conveyor. One of his fingers were pinched between the crown and one of the rollers. The employee fractured the tip of his right ring finger.
Employee pulling on a chain-fall, when weight shifted on chain-fall the pull chain was pulled from employees hand. The employee tried to hold the chain, but it was pulled from his grasp causing injury to his right bicep muscles.

Operator was pushing a charging rod into a retort when he felt a strain in his neck. Operator uses a 15 foot pole to push six 50 pound bags of material into a 12 foot long retort. When the operator was pushing the 4th or 5th bag into the retort he felt a strain in his neck.

Employee took it upon himself to do a task without training for the task. He replaced the chipper chute and caught the 4th and 5th digits of his right hand between chute and chipper while maneuvering chute. Fracturing a bone in each finger.

The employee was placing condensers in Furnace #9. When the condensers would no go in, the employee went to remove it. His left hand was holding the front of the condenser and his right hand was holding the rear, as the condenser was at the end of the retort, he started to lose grip and he went to catch it with his right hand. That is when he left a pain in his right arm.
The employee was using a wheelbarrow to move stone into the electrical blockhouse. Another employee was using a skid loader to dump the stone into the wheelbarrow, when the stone was dumped into the wheelbarrow, the employee was holding onto the handles of the wheelbarrow. When the stone hit the wheelbarrow, the employee felt pain in his right hip.
The employee was trying to unplug the log chipper that is used to chip calcium metal with a rod. After trying to repeatedly unplug it the employee stopped and went downstairs to shut the chipper down, that is when he felt pain in his lower back.
The employee was walking from one building to the next, when he slipped on the ice and injured his left wrist.

Employee was lifting a roll of stretch wrap when his hand slipped, his right ring finger tip was caught between the stretch wrap and concrete wall fracturing his finger tip.
The employee was discharging a furnace when her goggles fogged up, when she went to wipe the goggles off a piece of dust went into her right eye.
Employee was lifting a calcium metal crown weighing approximately 80 pounds. Employee lost his grip on the crown, dropping it on his right foot.
Maitenance employee was using a piece of thin metal flashing as a straight edge. While moving the flashing, the edge lacerated the back of his left thumb.

Employee slipped on slush covered surface and fell striking his knee on the ground.
Employee was shoveling/discharging furnace when an unexpected sudden discharge of heated dust hit his neck and upper chest area. Furnace 10 was being used to dry spent calcium/aluminate briquettes for a trial at the time and was almost empty.
Employee was assisting in changing out the cooling fan drive motor. To get the motor up to the cooling tower a pipe was put through the motor's lifting eyelet. One employee on each side of the 4 foot pipe lifted and started carrying the motor. When they went up an incline the motor slipped down the pipe contacting the employee's finger.
Employee was pushing electrical motors onto a pallet and when he was adjusting one of the motors on the pallet he felt a slight pain in his back. He immediately stopped moving the motors. He then picked up some bolts off the floor and had severe pain in his lower back. (Reportable due to RWDs, which started on 6/11/07.)
While arc welding inside the dryer at the secondary and crusher dryer, EE's eyes were exposed to flashes reflecting off the very shiny stainless steel inside dryer. Flashes came in from back of welding helmt. Reportable due to prescriptiong antibiotics to prevent infection.
Employee was climbing down ladder of tanker truck trailer and at bottom/ground stepped on uneven ground and twisted ankle.
Employee was greasing secondary crusher and dryer(SCD) L-150 D Loader and slipped on snow/ice and landed on right shoulder. Employee decided not to go to see a doctor until 2/17/2007. Reportable due to restricted work duty days starting 2/17/2007.
EE was working in the calcium/furnace building loading calcium crowns into the crown chipper chute & pinched his left index finger between the crown & metal chute. Reportable due to avulsion injury - ripped a number of layers of skin. He was released back to restricted work duty but was already scheduled for vacation from 2/10 thru 2/18. On 2/19 he was released back to full duty.

On 12/21 employee came to his supervisor & told him lower back was hurting. He stated that he hurt his back on 12/20, likely when he was picking up & tossing bucket elevator buckets into a scrap metal hopper. Still under investigation. Restricted work days.
The driver was walking on the north side of the secondary crusher/dryer building to check on his traine. While walking he stepped on a one inch stone that was ont he asphalt pavement and twisted his left ankle.
While charging (carrying bags of briquettes) furnace #10, the employee went to place a bag on the chute, he planted his left foot & then felt a sharp pain in his left knee. Injury under investigation.
Screen stone building - while cutting rubber belt skirting to size, box opener type razor blade knife slipped and cut left pinky finger. Reportable due to stitches and restricted work duty.
Employee was removing on empty condenser from the calcium crown pusher when he caught his left index finger between the condenser and steel frame causing a laceration, contusion and fracture of bone.
EE was shoveling out (removing spent briquettes) furnace #9, when the shovel stuck going into retort, stopping quickly. The handle of the shovel struck EE in the chin. Reportable due to stitches.
Employee was closing the manually operated slide gate under #1 porduct bin with a 2" diameter steel bar when he caught his finger between the bar and a piece of metal. (reportable due to stitches and 1 restricted workday)
EMPLOYEE IN PROCESS OF CHARGING FURNACES & WHILE RODDING A CHARGED RETORT, EE PINCHED HIS FINGER BETWEEN THE CONDENSING ROD & THE CHARGING CHUTE THAT WAS BEING REMOVED FROM THE ADJACENT RETORT. REPORTABLE: FRACTURED FINGER, PLUS PAIN KILLERS & ANTIBIOTICS.
EMPLOYEE CHARGED & DISCHARGED 5 FURNACES DURING THE DAY WEARING GOGGLES. HIS EYES FELT DRY DURING THE DAY. ON HIS DRIVE HOME THE SUNLIGHT WAS IN HIS EYES & HIS EYES BECAME SEVERELY IRRITATED & STUNG. HE WAS TREATED AT AN E.R. & THE FOLLOWING DAY AT OUR OCCUPATIONAL DR.
Employee was descending down ladder next to aluminum chop bin platform in furance building. He had his right hand/finger pinched between bin and ladder rung when forklift backing up removing aluminum chop box hit bin. causing bin to move.
While charging calcium furnace #10 the employee pulled the charging rod too far back allowing it to slip off the end of the chute. The employee grabbed it from the end and leaned back to lift it. When he did, he felt a pain in his back.
Employee was pushing/removing calcium crowns from the metal condensers using the log/crown pusher when empoyee felt a piece of calcium enter into his eye. He flushed eye with water but was still irritated so he went to see a doctor. (reportable-antibiotics for eye rx).
Employee was shoveling/removing spent calcium alumivate briquettes from the furnace and when he went to push cart with shovels to next furnace he got foreign material in his eye. He flushed but still irritated so he went to eye doctor (reportable due to antibiotic prescribed).

WHILE BAGGING HIS 6TH PALLET OF 50 LB. BAGS OF DOLOMITIC LIMESTONE AT RAYMOND MILL BAGGING AREA, EMPLOYEE FELT A POP IN HIS BACK & FELT SIGNIFICANT PAIN.
Employee was shoveling, removing spent calcium aluminate briquettes when he felt a piece of foreign material in his eye. After flushing his eye at an eye wash station unsuccesfully, he went to the doctor, who provided an antibiotic.
Standing at the quarry edge next to the crusher watching blast in quarry with two contractors and the quarry supervisor, blast went bad. We ran, I fell trying to avoid flying rocks and landed in a truck tire rut. Injured back, legs, right arm, shoulder, knee, right eye and left abdominal pain.
WHILE SHOVELING OUT FURNACE #IO (REMOVING CALCIUM ALUMINATE SPENT BRIQUETTES) EMPLOYEE FELT A PIECE OF FOREIGN MATERIAL IN HIS EYE. AFTER ATTEMPTING TO FLUSH IT OUT AT THE EYE WASH STATION UNSUCCESSFULLY, HE WENT TO THE EYE DOCTOR WHO REMOVED THE FOREIGN MATERIAL UNDER HIS EYE LID. (RECORDABLE - DOCTOR PROVIDED AN ANTIBIOTIC).
EMPLOYEE WAS MANUALLY BAGGING CALCIUM OXIDE/LIME & ALUMINUM BRIQUETTES INTO 48 LB. BAGS & PLACING THEM ONTO STORAGE CART (USED TO CHARGE FURNACES) WHEN HE FELT A SHARP PAIN IN HIS LOWER BACK.
EMPLOYEE WAS WALKING DOWN THE STAIRS BETWEEN SCREEN 131 & 132 IN THE SECONDARY CRUSHER & DRYER BUILDING WHEN HE STEPPED ON A ROCK.
Driver was in the process of having the tank trailer loaded by a specialty minerals ee. When complete, the rebar driver started to walk back to the tractor when his hard hat and then shoulder was struck by a piece of metal.
EMPLOYEE WAS TRYING TO MOVE/STRAIGHTEN OUT A FULL CONDENSER OF CALCIUM METAL WHEN HE WAS SLIDING/PUSHING IT FORWARD, THE BACK EDGE CAUGHT THE METAL RACK CAUSING THE CONDENSER TO SLAM DOWN TO THE RACK CATCHING HIS RT. THUMB BETWEEN THE CONDENSER & METAL RACK.
Electrician was pulling on wires to connect to new motor for #1 mill when he felt pain in his lower back. Reportable due to RWDs and a number of Dr appointments.
WHILE CHANGING INTO WORK CLOTHES IN THE LOCKER ROOM, HE LIFTED HIS LEG TO PUT HIS PANTS ON AND DEVELOPED SEVERE PAIN IN HIS BACK ON HIS LOWER RIGHT SIDE. IT RADIATES THROUGH HIS RIGHT BUTTOCK AND BEHIND HIS RIGHT LEG. (REPORTABLE DUE TO RESTRICTED WORK DAYS AND PRESCRIPTION DRUGS).
While trying to open a jammed screen gate with a come-along, employee applied force and the gate sprang open and smashed finger between come-along handle and building brae. (Reportable due to draining fluid from under fingernail.)
Employee was sitting at her desk in the office building when she got bite by a spider.

EMPLOYEE WAS REMOVING #3 JOURNAL FROM #5 RAYMONDMILL. THE JOURNAL WAS WEDGED. HE WAS TAKING THE TENSION OFF FROM THE COME-ALONG WHEN THE JOURNALSPRUNG LOOS SHOCKING HIS RIGHT ARM.
EE WAS UNCLAMPING THE TAIL OF A FINISHED CAGE OF WIRE. THE TAIL SNAPPED UP AND GRAZED HIS WRIST.
SHOVEL CAUGHT LIP INSIDE RETORT, STOPPING DEAD & THE SHOVEL HANDLE HIT THE EE IN THE MOUTH, CHIPPING 3 UPPER FRONT TEETH.
WHILE EE WAS SWINGING A SLEDGE HAMMER TO REMOVE A STUCK CONDENSER FROM THE CROWN PRESS, HE SMASHED A FINGER ON HIS LEFT HAND AGAINST THE PRESS.

DOCK LEVELER DIDN'T SPRING INTO THE POSITION IT NEEDED TO. EE PRIED IT UP WITH A CROW BAR AND ASTHE DOCK CAME UP, THE SPRING LOADED LIP SNAPPED INTO POSITION, HITTING THE EE'S RIGHT HAND.
EE GRABBED A REMNANT OF TIN TO PULL IT OUT OF THE CONDENSOR. THE TIN DID NOT MOVE BUT CUT THROUGH THE EE S GLOVE AND LACERATED HIS RIGHT INDEX FINGER.
UNABLE TO DEVISE EXACT CONDITIONS - EE APPARENTLY MOVED OR TWISTED AWKWARDLY - FIRST FELT PAIN WHEN CLIMBING ONTO A RAIL CAR - RESULTED IN UMSCLE STRAIN/LIGAMENT TEAR OF THE GROIN.
EE WAS SWEEPING UP A WORK AREA & CONTACTED STEEL, CUTTING HIS FINGER.

EE WAS COMING DOWN THE FURNACE FLOOR SOUTHWEST STAIRS, [ON THE 1ST FLIGHT, LAST STEP] WHILE TURNING TO GO DOWN THE 2ND FLIGHT, HE LOST HIS FOOTING & TWISTED HIS ANKLE - [RESULTING IN A FRACTUR ED ANKLE/LEFT FOOT] - HE MISSED THE LAST STEP & CAME DOWN ON THE LANDING. - NOTE: LIGHTING WAS GOOD & STAIRS WERE CLEAN.
WHILE LIFTING A BARREL OF GREASE ONTO A PICKUP, THE EE TURNED WITH THE BARREL, STRAINING HIS LOWER BACK.
EMPLOYEE WAS CLEANING BUILD UP BEHIND THE ARM OF THE CORE WIRE MACHINEAND THE HAMMER SLIPPED OFF THE BAR AND HIT THE BACK OF HIS HAND.
EE WAS REMOVING 325LB BARREL FROM THE BACK SIDE OF SKID BY ROCKING SIDE TO SIDE. HE FELT A STRAIN IN HIS LOWER ABDOMEN.
EE WAS SHOVELING FROM FLOOR TO HOPPER (IN SCREENED STONE AREA) TWISTED AND FELT PAIN IN HIS LOWER BACK. MUSCLE STRAIN IS THE DIAGNOSIS.
EE WAS OPERATING CLADDER MACHINE. WAS HANGING STEEL ON MACHINE (PAY OFF END). WAS NOT WEARING GLOVES. COIL MOVED & CUT HIS HAND ON A SHARP STRIP PROTRUDING FROM COIL.
EE WAS ASKED TO WORK IN AREA NOT HIS NORMAL ASSIGNMENT. HE WAS STACKING BAGS OF BRIQUETTES ON A CART AND ALLEGEDLY SPRAINED HIS HAND. EE DID NOT REPORT THE INJURY DURING HIS SHIFT, BUT BONLY T HE NEXT DAY. DESPITE THE FACT HE SAYS IT HAPPENED AT 11 AM.
EMP WAS BAGGING LIMESTONE. HAND LATER BEGAN TO SWELL. EMP DOES NO KNOW CAUSE.
EMP WAS SCREENING CALCIUM. REACHED UNDER SCREEN TO GET BLOCKING PLATE WHICH HAD BEEN IMPROPERLY STORED THERE. PLATE HIT THE COUNTER WEIGHT WHICHWAS UNDER THE SCREEN. PLATE WAS KNOCKED OUT OF E MPS HAND AND THEN HIT EMPS HAND.
EE WAS REMOVING HEADS FROM FURNACE & FELT PAIN IN HIP/BACK.
EE WALKED ACROSS PARKING LOT TO GET PACKING MATERIAL. THE WALKED AROUND THE ICY SPOTS. HE PICKED UP THE PACKING MATERIAL AND THEN WALKED OVER THE ICY SPOTS THRU THERE WAS CHAR PAVEMENT. HE SLI PPED AND FELL AND TWISTED HIS KNEE.
EE FINGER WAS CUT REQUIRING 4 STITCHES. EE REACHED FOR A CORD TO CLOSE FEED HOPPER DOOR. AT SAME TIME NOT CAUSED BY EE MOVEMENT THE COMPACT WHEEL SHAFT MOVED BECAUSE OF SCREWS LOOSE IN THE RET AINING COLLARS WHICH HOLDS THE SHIFT IN PLACE. EE FINGER WAS PINCHED BY THE WHEEL HOUSING.
EE WAS RUNNING FINGER ALONG WIRE TO CHECK IF SEAM WAS OK. GOT SPLINTER OF METAL IN HIS FINGER.

UNCERTAIN - NEITHER EE NOR EMPLOYER ARE SURE IF THIS IS OR IS NOT A WORK-RELATED INJURY. EE CALLED ON 10/18 SAYING HE HAD ARM/ELBOW PAIN & DR. HAD DISABLED HIM FOR 2 WKS.
EE WAS STRIPPING CROWNS & WENT TO PUT A STRIPPED CROWN INTO A BARREL & CAUGHT HIS RIGN FINGER BETWEEN THE CROWN & THE BARREL.
EE HAD A ROACK OF CROWNS ON THE FORK TRUCK. FORKS DROPPED, RACK HIT THE TABLE & ONE CROWN HIT EE IN THE BACK.
EMPLOYEE WAS ADJUSTING FEED HOPPER ON A CORE WIRE MACHINE. HOPPER RAISED.
EMPLOYEE WENT TO REMOVE A HOSE AND WATER/STEAM REMAINING IN IT SPRAYED IN HIS FACE.
EE WAS TRYING TO BEND A PIECE OF STEEL ON FEED HOPPER BACK INTO PLACE A WELD ON THE STEEL GAVE WAY AND THE PRYBAR HE WAS USING HIT HIM IN THE NOSE AND FOREHEAD. EE RECEIVED 50 STITCHES IN HIS NOSE.
