Employee slipped off the last step while dismounting the front end loader. He initially told the foreman that he was OK and worked without incident for the rest of the week. On the morning of 3/8/09 he awoke to find his right leg swollen and black and blue. The ER @ Dubois Hospital diagnosed an infection under the unbroken skin on his leg. His first day off work was 03/09/2009.

Employee states that he was dismounting from his Cat D10R Dozer when his feet went out from under him and he fell to the ground onto his right shoulder. He believes that he may have slipped on snow and ice on the tracks/structure.

Employee was standing next to his dozer. As he turned to talk to a co-worker, he felt a pain in his lower back. He completed his shift and went to the doctor the next day. He was advised by the doctor to take some time off work.

Employee was climbing down from a rock truck, when he missed the handrail and lost his balance, causing additional strain on his hand and arm still holding onto the handrail.

Employee was climbing down the ladder of the 777B Rock Truck to fuel up at the end of his shift, when he felt a pain in his shoulder. Employee shift started on 01/21/2008. Employee continued to work until he had surgery on the shoulder on June 20, 2008.

Injured was attempting to lift part of a broken tree from across the top surface of his D-11 Bulldozer blade when he dislocated his left shoulder.

Employee was using an air wrench when the 1" airhose disconnected from the wrench and struck the employee on his left eyebrow causing a laceration requiring 3 stitches.

Employee, prior to the start of her shift, slipped while walking across the parking lot causing her to twist and fracture her right ankle.

Employee was changing fuel filters on a Cat 777 C haul truck when he bumped his head on a mounting bracket.

OPERATOR DOES NOT REMEMBER FALLING, THERE WERE NO MARKS ON HIS HEAD, BUT HE LOST MEMORY AND HAD SLIGHT CONCUSSION - HE ACTED UNUSUAL.

OPERATOR CLAIMS HE STRAINED A MUSCLE IN HIS BACK WHILE LIFTING AN OIL CAN.

WHEN REMOVING A TRANSMISSION PUMP FROM A 992 LOADER, HIS BOOTS HAD OIL AND SNOW ON THEM, HE SLIPPED AND LANDED HARD ON HIS RIGHT SIDE.

WHILE REMOVING A BOLT FROM A DOZER BLADE CUTTING EDGE. HE WAS HOLDING WITH HIS LEFT HAND. THE BOLT TURNED LACERATING HIS FINGER.

DISMOUNTING FROM 992C LOADER, HAD MUD ON HIS FEET & SLIPPED ON LADDER & FELL APPROX 5 FT TO THE GROUND & LANDED ON HIS SHOULDER. FRACTURED HIS UPPER LEFT ARM

HE WAS CHANGING THE SHAFT ON A DAVEY M8B DRILL WHEN THE TURNTABLE ON THE DRILL CAME BACK SMASHING HIS LEFT HAND AGAINST THE DRILL FRAME BREAKING HIS LEFT INDEX FINGER BELOW THE KNUCKLE.
