On 5/6/2024 at approximately 10:30 AM EE was in the shop when EE informed the plant foreman that EE wasn't feeling well. EE stated that EE felt a little dizzy and that EE's left arm was numb. EE was immediately transported to the hospital.

Employee was exiting the crusher control room and as EE proceeded to come down the stairs EE's right foot slipped causing EE fall. When employee fell EE's left foot got twisted and ended up underneath EE causing the ankle to break in three different places.
As an employee entered a haul truck and sat down the employee felt discomfort in left calf. Clinic determined that the strained left knee.

Water pull operator pulled to the side of the road to let semi trucks pass, the shoulder was soft due to recent rains and could not hold the weight of the machine. The shoulder gave way causing the water pull to roll over.

Employee got finger smashed between take up bolt & take up bearing while loosening tail pulley for repairs on the conveyor. Employee smashed left index finger.

1/25/18 employee was walking along the belt line at the crusher and rolled their left ankle. Employee was sent to Concentra to get evaluated. Employee was diagnosed with a muscle strain on the left ankle. Employee was given a prescription and put on light duty. Employee was released to full - regular duty on 2/14/18.
