Changing hoses in mill to set up running a different product. While trying to maneuver one of the hoses, it slipped out of the employee's hand and hit below employee's left eye and cut the left check bone. Employee left work for a short period of time to receive stitches, and returned for the remainder of the shift.

Cleaning out chute and helping move material from the hopper through chute. There is a broken vibrator on chute making it hard to move gypsum. Gypsum has a cement appearance. Left arm felt a sharp pain that went up to left shoulder.

Miner manually lifted wooden pallet and injured back while attempting to place pallet on product loading platform. Conditions: work was being performed indoors at a designated super sack filling workstation. Miner called nurse line for initial consultation (ref# 11949995). Miner went to chiropractor on 05/22/2013.
EEG, director of sales, was giving a plant tour to a customer. Weather had caused entire area to be icy. EE slipped, lost his footing, and hit spine on a 4" tire rut that was frozen. Employee is receiving physical Therapy.
At end of 2nd shift miner was completing task of moving train cars, while walking back to office he slipped & fell. Ground is level & flat but ice had formed over loose gravel during the night. He slipped on ice & fell to ground injuring rt arm & shoulder. He went to dr on 1/15/13 for follow-up diagnostic visit, he was prescribed over the counter pain meds, taken as needed.

EE's were driving from main office to the plant to begin their shift. EE felt something in his eye thought it was a bug. Arrived at the plant another EE helped IEE at the eye wash station. It started to swell shut, could not see what it was. EE's wife took him to the ER Dr. did not find anything. They washed it out and he left ER He has a follow-up visit at the eye doctor.
EE was loading a bulk load of calcium into a trailer. He was closing the gate that feeds the product into the trailer, and his finger became pinched between the valve shut off and the bracket that hold the shut off mechanism.
Employee was removing bolts on the old GD gate. As he attempted to remove an old bolt with a wrench, it slipped off the bolt head, and his wrist was in-between the wrench and the gate.

Hooking up a hose to the portable blower. The hose is re-enforced with a wire & a piece of wire was sticking out of hose and cut his right ring finger.
Operating the Bagging Equipment. Bending forward on the bagging machine to place bags on the spouts and hurt is lower back - burning sensation while doing this job.

Employee was operating the dump truck. He slipped on the platform of the truck as he was exiting the cab.
Employee was doing some maintenance work in the dryer plant, and was operating the Milwaukee 4" electric grinder. A spark flew from the grinder under his safety glasses and struck him in the left eye. He experienced a burning feeling, with watering eye and redness. He did go to the Emergency Room at St. Mary's Hospital.

INJURED HAD JUST FINISHED INSTALLING A KEYSTONE VALVE INTO OUR ST. REGIS BAGGER WHEN HE REACHED A TOOL ON THE FLOOR HE FELT A STABBING PAIN IN HIS LOWER BACK.
A PNEUMATIC TRANSFER LINE 6 INCHES IN DIAMETER BECAME PLUGGED WITH FINELY CRUSHED LIMESTONE. THE INJURED USED A FOUR FOOT PRY BAR TO LIFT THE PIPE A COUPLE INCHES TO REACH SOME BOLTS. THE BOLT S NEEDED TO BE DISCONNECTED TO OPEN THE PIPE TO DISLODGE THE LIMESTONE.
