Employee was helping to remove a pin from a CAT 990 loader. When the pin released from the bell crank, it hit the frame, upended, and slid out of the supporting sling. It landed on the employees toes of left foot.
Employee was moving the belt guard for the scalping screen. When they turned with the guard, they felt a pop in the lower back. Employee suffered a back strain.

Employee parked truck on the scales and when ee went to exit the truck, ee slipped on the ice and hurt shoulder and back. The employee did not seek medical attention until 01/23/2018. This injury became reportable because ee was given a physical therapy prescription.

Employee was exiting excavator and their foot slipped off the step. This caused employee to lose balance and fall to the ground. Employee went to a physician on 04/24/2017 and was diagnosed with a broken rib.

Employee was walking around the front of a loader and he stepped on some ice and fell onto his back, striking his head on the ground. He went to a physician and was diagnosed with a concussion.

Employee was moving a water discharge hose when he tripped and fell to the ground. He struck his face on the ground which caused a cut above his right eye which required sutures.
Employee was changing slide gates when he lifted one and felt a pain in his left groin. He went to the Emergency Room and diagnosed with a hernia. He returned to work on regular duty the following day and will have surgery to have the hernia repaired at a later date.
Employee was using a flat hoe to clean material from the 2A chute when a piece of stone came up and hit him in the mouth, chipping his front tooth. He went to the dentist and had a cap put on the chipped tooth.

Employee was raising jack stand on the fuel tank. The hand crank slipped off the jack and struck the employee in the nose. Employee will have surgery on 9/16/10 to repair his nose.

EE WAS BENT AT THE WAIST LOADING BLAST HOLES WHEN A ROCK FELL FROM AN ADJACENT HIGHWALL. THE ROCK STRUCK THE EE ON THE LOWER RIGHT SIDE OF HIS BACK. HE WAS TAKEN TO A LOCAL HOSPITAL WHERE HE WAS TREATED AND RELEASED FOR BRUISING AND A CONTUSION TO HIS BACK. RISING TEMPERATURES AND MELTING SNOW & ICE MAY HAVE LOOSENED THE HIGHWALL MATERIAL.

THE INJURED MAN WAS OPERATING THE GARDNER DENVER DRILL WHEN A SMALL STEEL SPLINTER STRUCK HIM IN THE RT THIGH. HE REMOVED THE SPLINTER HIMSELF AND RECIEVED A TETNAS SHOT THE NEXT DAY.
THE MAN WENT TO DR FOR LOWER BACK PAIN WHICH RESULTED FROM A SLIP ON THE 777D CAT ROCK TRUCK. NO SLIPPED WHILE GETTING ON THE TRUCK.
