The employee was seen by a Banner Health clinic on 03/18/2025. Prescription eye drops were prescribed by the physician. The employee did not have any lost time or restrictions.

The injured employee was performing maintenance on a bearing for the P-1 conveyor when right foot slipped into a gap between the conveyor frame and the concrete retaining wall. The injured employee's body turned to one side causing a twisting injury to the right knee.

On Friday, October 19, 2018, a welder/mechanic was walking through a shop when they tripped on a welding cart outside of the welding bay, resulting in a fall. The EE fell onto a piece of angle iron that was on the weld cart, causing a laceration to their hand. The EE was taken to the emergency room where they received 6-8 stitches.
