The injured employee was performing maintenance on a bearing for the P-1 conveyor when right foot slipped into a gap between the conveyor frame and the concrete retaining wall. The injured employee's body turned to one side causing a twisting injury to the right knee.

On Friday, October 19, 2018, a welder/mechanic was walking through a shop when they tripped on a welding cart outside of the welding bay, resulting in a fall. The EE fell onto a piece of angle iron that was on the weld cart, causing a laceration to their hand. The EE was taken to the emergency room where they received 6-8 stitches.

An employee was attempting to connect a light plant trailer to the hitch on a pickup when EE used a bar vise to strike the tongue of the trailer hitch, missing and pinching EE's finger.

Employee was removing rock from stopped jaw crusher when he had a little discomfort in his lower abdomen. He thought it was just a strained muscle.

This employee and another were welding in the welding shop. Employee went home and started experiencing pain in both eyes. He went to the doctor at 12:00 am and was told he had flash burns to both eyes.

Employee stated she was having pain in her right shoulder and left wrist. She states the pain is from years of repeated motion due to operating a wheel loader.

Folding D-10 dozer track for shipment. While folding tracks master link folded under to far. Employee straightend master link with right hand. At this point his left hand was between track pads. The tracks flatened cutting and breaking three fingers on his left hand.

Employee was changing sheer bolt in feeder through access door when door came unlatched and fell striking employee on his right hand.

STEPPED OFF CHIPS SCREEN ONTO CATWALK AND ROLLED ANKLE.

WHEN MOVING DRILL OFF SHOT DOWN DRILL ROAD, DRILL TRAVELED OVER A HUMP & A ROCK HIT BOTTOM OF TRAMPAD PINCHING FOOT BETWEEN PAD & DRILL.

EMPLOYEES WERE SHOOTING ELECTRIC BLASTING CAPS BY SUBMERGING CAPS IN 6 FT WATER HOLE. EMPLOYEE'S CAP DID NOT GO IN WATER. FLYING METAL OFF CAP CUT INJURED'S LEFT CHEEK. REQUIRED STITCHES.

EMPLOYEE WAS INSTALLING ROLLER ON NW80D BALL MACHINE. WHILE INSTALLING ROLLER, IT SLIPPED OUT OF HIS HAND SMASHING HIS RIGHT MIDDLE FINGER TIP BETWEEN ROLLER AND THE OUTER TRAVEL SHAFT.

TAKING OFF CATWALK HANDRAILS AND GUARDS CLIMBNG UP SIDE OF CONE TO HOOK TABLE GRABBED LOCKING RAILS IT TURNED AND LOST BALANCE FELL BACK STRIKING GUARD RAIL ON WAY INJ R HIP

LUBRICATING FRONT END LOADER HE SLIPPED WHILE TRYING TO STEP FROM CAB PLATFORM TO FRONT END LOADER

EMPLOYEE WAS DRILLING. AFTER CLEANING A PLUGGED BIT I.NAME RECOUPLED THE STEEL. IN DOING SO, HE GRABBED THE STEEL WITH HIS RIGHT HAND. THE MITTEN ON HIS RIGHT HAND BECAME ENTANGLED IN THE STEE L. BRUISED WRIST AND FOREARM.

A NEW CONVEYOR BELT HAD BEEN INSTALLED, BUT TENSION WAS NEEDED ON THE BELT TO INSTALL FLEXCO FASTENERS. A PAIR OF PLIERS WAS CLAMPED ON TO THE BELT TO HOLD THE BELT. A CABLE COME ALONG WAS T IED TO THE PLIERS. THE PLIERS CAME LOOSE, TRAVELED UP THE BELT AND HIT EMPLOYEE ON THE NOSE.

THE EMPLOYEE WA8 ASSISTING WITH THE ASSEMBLY OF THE HEAD ON THE 4 FOOT CONE.HE CAUGHT HIS RIHGT THUMB BETWEEN THE HEAD AND THE ADJUSTING BRACKETALONG SIDE OF WHERE THE HEAD FITS.HIS RIGHT THUM B WAS CUT REQUI9ING STITCHES.

DRILL STEEL WAS BEING LOADED ONTO A TRUCK BY ONE EMPLOYEE STEEL SLIPPED FROM HIS HANDS CAUSING INJURY TO LEFT MIDDLE FINGER
