EE WAS WORKING ON THE ROD OF THE RIDE CYNDER OF THE CATERPILLAR 769C HAUL TRUCK USING AN ACETYLENE TORCH TO HEAT THE BROKEN NITROGEN VALVE. AN EXPLOSION OCCURRED CAUSING THE 113 LB ROD TO BE E XPELLED FROM THE CYLINDER STRIKING THE EE IN THESTOMACH CAUSING FATAL INTERNAL INJURIES.

Employee was receiving required CBC blood draw on the PHS mobile testing van when he became light-headed and lost consciousness. He was only out for a few seconds and was easily aroused by PHS employee talking to him.

EE was repeatedly swing sledge hammers to help unplug feed chute. He was sore that night, but woke up next morning feeling worse & reported injury when he came to work 12/22. Initially he felt it was a slight pull & he worked that day. 2 doc visits 12/23 diagnosis: strain-rotator cuff, release no restrictions, 12/27 prescribed Naproxin, 2 wks PT, again no restrictions.

EE was loading explosives into drill holes.Holes on the edge of the bench were on sloping ground so the ee was using a full body harness and lanyard for fall protection. EE lost his footing and slid app. 1' down the side of the bench, his slide was stopped by the fall arrest system. He slid, he reached up to grab the lanyard, which extended his left shoulder causing the injury.

Employee was tightening bolt on rock breaker with impact on top of bolt & a wrench on the bottom nut. As the wrench was activated, bottom nut & wrench rotated. The wrench pinched his finger between the wrench & the base of the rock breaker. This resulted in fracture to employee's right distal on his middle finger. *Fracture determined to be OLD injury-Not Reportable-Deg 10*
