EE was climbing down from Gardner/Denver Drill #1064 his finger got stuck on something and he pulled fingers out, his right index and middle fingers were lacerated requiring 7 stitches. Did not know what he caught his fingers on.

EE was fueling a drill, when he climbed down off the drill his foot slipped in the mud and he sprained his left shoulder. (muscle strain)
Employee had gone to put oil in water pump at washplant, had put in oil and when pump was started the discharge hose slung around and hit him in the chest knocking him down. Complained of rib pain.
Employee was fueling equipment when he stepped on a rock and twisted his left ankle.
EE was picking up a tooth for the 1800 hitachi shovel and pulled a muscle in his upper left arm.
EE stated that his tailbone was inflamed due to seat in the dozer.(Unit #1242) he operates is flattened out. This was not reported until 01/12/2010.

EE was on the powder truck and had the bay doors open trying to get the powder to fall (he has a stainless steel rod to knock the powder down). The wind caught the bay door and it fell, hitting his hand (contusion to right hand).
Employee passed out while fueling equipment due to heat. The night oiler found him on the back of the fuel truck. Employee was taken to the air conditioned trailer & given liquids. He did not want to see a physician at the time but later went to the emergency room. Heat exhaustion and mild dehydration.
EE was helping with maintenance while the shovel was down. He went to pull the ladder up that was still latched (it has a sliding ladder) and he pulled a tendon in his left arm.
EE was using a sledge hammer to chip frozen material from a mud bin. EE stated that he had lower back pain at the end of his shift.

Employee was unhooking water pump hose from water truck. He tripped on the hose and hit his left knee on the pump. He had a small cut above the knee, no stiches and said he could hardley walk, went to ER was released to go back to work, went to DR next day and was put on restricted duty.

Employee was installing a brake calliper on a 50 ton end dump. EE said he had bolt in one end of calliper & was trying to bolt in other end. The callifper slipped & caught the tip of index finger on left hand, resulting in about 3/8 of finger being cut off.
Employee was working on loader #1021. He caught his foot on air hose and fell hitting his shoulder and elbow. Employee's shoulder is sore.
Employee was helping change a course coal basket in the washplant. Employee was using a sledge hammer hitting on the course coal basket puller. His left shoulder was hurting and a knot came up on the back of his left shoulder. Employee went to doctor on 08-15-2007.
Employee was getting out of the mechanic truck and hit his right elbow against the exhaust causing a second degree burn 3 cm by 5 cm. Went to doctor on 8-4-07.
EE was changing oil in dozer, stepping down, slipped off side step on blade arm. Did not think much happened until next day. Told foreman that night he hurt his back but would probably be ok. Co was notified on 5/23/07 he was having back pain and muscle spasms.
EE was walking across coal yard and stepped on frozen ground, causing him to fall and twist his left knee. He said his knee hurt when climbing steps.

Employee's end dump was stuck in mud. Employee got out of truck to hook pull cable on front of end dump. Employee sprained his shoulder when hooking cable and getting back in truck.

Employee was climbing down the ladder from loader (980) when he got down he stepped in a frozen tire rut. Twisting his knee. On 12-20-05 he was standing on a walkway using a sledge hammer. Knee gave out and he fell to the ground. On 12-13-05 EE missed no work on 12-20-05 he went to E.R.
EMPLOYEE HAD BEEN RUNNING ROAD GRADER, WENT TO EXIT CAB AND THE STEP WAS MISSING, IT HAD BEEN KNOCKED OFF. HE FELL TO GROUND ABOUT SIX FEET, MULTIPLE CONTUSIONS
EE was climbing up the ladder on #1047 loader, he felt something, pop in his chest. said he was in a lot of pain and difficult brenbing.
After welding and grinding ee thought he might have a flash burn on 5-18-05 his eye was still bothering him. he notified ee and went to eye dr. piece of rust in eye.
EE was attempting to break a large piece of coal that had blocked the feeder chute that feeds the wash plant. He climbed onto the belt. When the coal broke loose he fell on the belt on his right knee. Did not report as injured.
EMPLOYEE WAS REMOVING THE RADIATOR FROM THE 910 LOADER, HE USED THE CRANE FROM HIS MECHANIC TRUCK TO SET IT ON THE GROUND. HE WAS REMOVING THE RADIATOR FROM THE RADIATOR HOUSING, HE WAS GOING TO LAY THE RADIATOR ON THE GROUND WHEN HE FELT PAIN IN HIS BACK.

Ee was climbing off the back of the fuel truck (unit #1334). His left arm gave out. He lost his grip from the railing and fell to the ground about 5 feet. Left shoulder separated.
EE WAS WORKING ON THE DRILL. HE WAS STANDING ON THE DECK OF THE DRILL. HE WAS STEPPING DOWN OFF THE DRILL AND STEPPED ON SOME DIRT AND TWISTED HIS LEFT ANKLE.
EE WAS CLIMBING DOWN THE LADDER ON THE CAT END DUMP WHEN HE SLIPPED ON STEP AND FELL TO THE GROUND. FELL 15 FEET. FRACTURED NOSE.
EE WAS WORKING ON D9L #1222 DOZER. HE WAS GOING TO PUT A DIPSTICK TUBE UNDER THE FLOOR OF THE DOZER. HE TOOK THE FLOOR BOARD OUT OF THE CAB, CLOSED THE DOOR, GOT DOWN OFF THE DOZER TO GET WRENCH AND NEW PART, GOT BACK UP ON THE DOZER. HE OPENED THE DOOR AND STEPPED IN FALLING THROUGH THE HOLE IN THE FLOOR. BRUISED LEFT LUNG AND HAIR LINE FRACTURE OF RIB.
EE WAS UNDER THE HEADACHE ROCK OF (#1174) A 50 TON END DUMP. HE WAS BENDING OVER THE BATTERY BOX AND GOING TO JUMP START THIS TRUCK WHEN HE STOOD UP HIS HARD HAT FELL OFF AND HE HIT HIS HEAD. HE HAD A KNOT ON HIS HEAD.

AT ABOUT 4:40AM EE SAID HE WAS PREPARING TO FUELAN END LOADER IN THE PARKING AREA. HE WAS PULLING OUT THE HOSE ON THE FUEL TRUCK AND STUMBLED ACROSS A PALLET LYING NEXT TOTHE LOADER, CAUSING HIM TO FALL & FRACTURE HIS RIGHT LEG BELOW THE KNEE.
EE WAS DOING MAINTENANCE IN THE WASH PLANT SOME TIME THAT NIGHT HE GOT SOMETHING IN HIS EYE. DID NOT BOTHER HIM TILL THE NEXT DAY. WENT TO EMERGENCY ROOM NEXT MORNING.
EE WAS USING A HITCH PIN TO CLOSE THE EARS ON A 4" CAM LOCK FITTING WHEN INSTEAD OF HITTING THE FITTING HE STRUCK HIS LEFT HAND WITH HIS RIGHT HAND HOLDING THE PIN.
EE WAS CLIMBING DOWN OFF OF THE GRADER WHEN HE SLIPPED. HE WAS HOLDING ONTO THE HANDRAIL WITH HIS LEFT HAND. EE DID NOT GO TO THE DR WHEN THIS HAPPENED BUT THE INJURY GOT WORSE. HE WENT TO LIG HT DUTY 8-27-03.
EMPLOYEE WAS CLEANING A DOZER TRACK, SLIPPED IN MUD. HE TRIED TO CATCH HIMSELF WHILE FALLING AND HURT HIS SHOULDER.
EE WAS WALKING IN THE PARKING LOT IT WAS WINDY SOMETHING GOT IN HIS EYE.
