Employee grabbed a 1/2" steel plate from a work bench. When EE slid it towards self, EE's thumb got caught between the table and the steel plate. Employees finger was crushed and sustained injuries requiring medical treatment (stitches).

At lewisville Pit EE was using a grinder with a cut off disk to cut the head off of a bolt. The grinder dug in causing the grinder to spin in EE's hands. The cut off disc came into contact with EE's thumb causing a deep laceration. Another EE is taking EE to the clinic and will be updating as we know more. EE has been with the company for 6 years.
Employee was changing a bearing on a screen. While lifting the bearing, EE felt a pop in EE's back. EE went to urgent care and was diagnosed with minor lower back strain.

Employee was lifting a Curtain Liner into place in the H.S.I when it slipped from hands. EE was able to catch it before it hit EE or dropped completely. Two days later on 10/13/2016 EE started to suffer from bad stomach pain then later after the shift EE noticed naval was bulging out of Stomach. The Employee is Temp Employee we hired through the local Temp service.

Miner was working out of a manlift basket when the lift swayed after coming to a stop, miner reached hand out to steady himself on railing, finger over the edge of the railing became pinched between the lift rail and the catwalk rail of the plant. Right index finger pinched, bone cracked,(2)stitches.

EE was pulling welding cables under the prewash screen, after pulling the leads he felt something in his left eye. He then went to look in the mirror to see what it was. We then tried to flush it out with eye wash, the object did not come out, so we proceeded to the doctor's office for removal.
EE had come in to work early and was sitting down and drinking a cup of coffee with two co-workers. He dropped his cup and reached down to retrieve it. When he didn't rise up, his co-workers helped him up. He was in obvious discomfort. 911 was called and the paramedics determined EE was having a stroke. He was transported by amublance to Legacy Hospital.

Injured employee was moving the moveable building around the HSI Crusher. He pushed with his right shoulder and in the process, moved his right elbow in to his rib cage which resulted in a bruised rib cage.

Employee was operating a front end loader at the 3/8 minus stockpile loading trucks. A yellow-jacket bee entered the cab and stung his left ring finger. His finger started swelling. The employee was taken to the medical clinic where the stinger was removed. He was given antibiotics and released to full duty.
Employee was working with a shovel cleaning around the jaw crusher. After completing an 8 hour shift, he felt some discomfort in his lower back. The employee drove home and felt spasms in his lower back. He could not exit his vehicle without help. He then contacted his supervisor and was taken to the hospital.

WHILE REMOVING THE RETAINER PLATE ON THE REAR BRAKES OF A 980F LOADER, THE PLATE CAME LOOSE AND FELL. IT PINCHED THE EE'S FINGER OF HIS LEFT HAND WHICH RESULTED IN THE LACERATION.

WHILE THE ENTIRE PLANT REMAINED RUNNING, THE EE ATTEMPTED TO TIGHTEN A LOOSE SCREEN RAIL BOLT ON #1 MAIN PLANT SCREEN. HE LIFTERD RUBBER ON GUARD AND STUCK HIS HAND UNDER GUARD IN AN ATTEMPT TO TIGHTEN BOLT & WHILE DOING THIS HIS HAND WAS STRUCK SCREEN'S COUNTERWEIGHT.

WELDER WAS WELDING IN VICINITY OF EMPLOYEE AND EMPLOYEE LOOKED OVER AND RECEIVED FLASH BURN. EMPLOYEE WAS PAINTING AT THE TIME WHICH IRRITATED THE BURN.
