While operating the front-end loader, the employee felt ill with chest pains. ambulance was called & employee taken to hospital. He was checked out & his heart was OK. He was diagnosed with COPD - a non-work related illness caused by years of smoking cigarettes.

EE was walking on the dredge deck and stepped on a H-Beam. His foot slipped off, rolling his right ankle laterally, causing pain in ankle.

The ee was walking across the dredge deck to turn on the pump shut off valve, when he slipped on some ice and fell onto the dredge deck, causing a minor strain to his back and neck-he was given a prescription.
THE EE WAS TAKING TRASH OUT TO THE DUMPSTER FROM THE SCALE OFFICE AND WAS STUNG BY TWO WASPS ON HER RIGHT WRIST AND CHIN. SHE HAD AN ALLERGIC REACTION AND WAS TAKEN TO THE HOSPITAL AND GIVEN A SHOT.

THE EMPLOYEE NOTICED THAT THE LOW WATER LIGHT WAS ON FOR THE DIESEL ENGINE ON THE DREDGE. HE WENT TO PUT SOME WATER IN THE RADIATOR BUT FORGOT TO IDLE DOWN THE DIESEL ENGINE. WHEN OPENING THE RADIATOR CAP, HIS LEFT ARM WAS BURNED WITH HOT WATER AND STEAM FROM THE RADIATOR. THESE WERE SECOND DEGREE BURNS WITH BLISTERS.
THE EE WAS WALKING BACK TO THE DREDGE OPERATOR'S CABIN, WHEN HE SLIPPEED ON SOME WET STEPS AND FELL FORWARD HITTING CHIN ON ANOTHER METAL STEP. THIS CAUSED A LACERATION JUST UNDER HIS CHIN THA T REQUIRED STITCHES.

WHILE CHANGING A SHAKER SCREEN THE EMPLOYEE HAD A HOLD OF THE TOP OF THE SCREEN, WHILE ANOTHER EMPLOYEE PUSHED THE SCREEN FROM TEH BOTTOM FO THE SHAKER DECK. WHEN DOING OS, THEEMPLOYEE AT THE TOP CAUGHT HSI RIGHT THIRD FINGER ON A WIRE CAUSING A LACERATION TO HIS RIGHT RING FINGER WHICH REQUIRED 6 STITCHES. THE EMPLOYEE RETURNED THE SAME SHIFT.
EE WAS USING A HAND GRINDER IN THE SHOP @ THE NORFOLK PIT, WHILE GRINDING A PIECE OF METAL. HE WAS USING A FULL FACE SHIELD WHILE GRINDING. SEVERAL DAYS LATER HIS RIGHT EYE BECAME IRRITATED AN D BOTHERED HIM CONSIDERABLY. THE EE'S WIFE SAID SHE COULD SEE SOMETHING UNDER HIS RIGHT EYE LID. THE EE RECEIVED MED. TRATMENT & A SMALL FLAKE OF METAL WAS REMOVED.

38505MPLOYEE WAS REPAIRING A CABLE ON THE PIPELINE, WHEN A STRAND OF FRAYED CABLE WENT INTO HIS RIGHT MIDDLE FINGER. THE EMPLOYEE RECEIVED MEDICAL ATTENTION TO REMOVE THE PIECE OF CABLE AND HA 40255ERAL SUTURES FOR THE LACERATION.
THE EMPLOYEE IS A FRONT-END LOADER OPERATOR AND STATED THAT THE CONSTANT PULLING OF LEVERS INT EH LOADERHAS CAUSED PAIN IN BOTH WRISTS. HE RECEIVED MEDICAL ATTENTION AND A PRESCRIPTION.
