While individual was moving the telephone cable a 636 battery scoop ran onto individuals foot twisting ankle. Doctor applied brace for support.
While drilling holes to anchor the section tail piece, the air line came loose from the drill and sprayed air and debris into individuals eyes, the foreign material in his eyes needed to be removed at the hospital and the Optometrist office.
While re-installing the relay bar pin to the pan line. A crib block was used to apply pressure to the relay bar, assisting in aligning the holes. The crib block flipped out and the shield rocked forward striking the individual's hand, causing a fracture to his right little finger.
While walking down the longwall set-up face, individual struck head against a roof bolt hanger. This jammed his neck. On 3/27/06 individual visited a doctor, the evaluation found his neck to be in spasm and causing a mis-alignment. Individual was kept off work and received physical therapy.
While releasing the roof screen mesh from the top of the ATRS individual had right elbow pinched between mesh support plate and ATRS main frame. This caused a severe bruise to right elbow area, resulting in missing work to allow swelling to subside.

While installing a water hose to the section water manifold, the manifold broke free from the water line and struck individual in the left shoulder and side of face. This chipped his bottom left wisdom tooth and caused a hair line fracture to jaw.
While pulling a wire mesh roof screen over his head and onto the ATRS of the roof bolter, EE felt pull in lower abdomen. Doctor visit on 7/12/05 diagnosed with hernia. Surgery secheduled for 7/19/05.
While installing the resin roof bolt into the mine roof by pushing the bolt in place with the chuck wrench, the wrench slipped off the head of the roof bolt and struck individual in the nose. This caused a small laceration that required 3 stitches to close.
While carrying a bottle of compressed oxygen into the beltline, the employee slipped and fell and the bottle landed on right hand and wrist. This caused a contusion to his wrist and he was not allowed to return to work for two days.

EE WAS CAUGHT BETWEEN THE MACHINE AND THE RIB. OPERATOR DID NOT SEE HIM THERE.
While installing a roof bolt, a chuck wrench was used to help push the bolt into the hole. As push pressure was applied to the bolt and rotation started, the chuck wrench flipped free and struck individual in chin. This caused a 1/2 inch laceration to chin which needed stitches.
While installing a roof bolt, the roof bolt plate became stuck against the drill mast. The individual moved the roof bolt plate to get it unstuck. As he moved the plate it popped free spinning and stricking the individual in the left ring finger. This caused a laceration to the back of the finger, which required stitches.
WHILE INSTALLING AN "I" BEAM TUNNEL LINER SUPPORT, THE "I" BEAM ROLLED AND PINCHED HIS RIGHT RING FINGER CAUSING A SMALL LACERATION THAT NEEDED STITCHES.

INDIVIDUAL HAD JUST GOT OUT OF UNDERGROUND TRUCK AND STARTED TO WALK TOWARDS FACE AREA, SLIPPED ON SOME ICE ON THE FROZEN MINE FLOOR AND TWISTED RT KNEE. INDIVIDUAL DID NOT SEEK TREATMENT UNTI L 12/10/03 WHEN KNEE BECAME VERY SWOLLEN. DOCTOR TREATED SWELLING WITH MEDICATION AND KEPT OFF WORK 5 DAYS.
