Relief Canyon Mine Metal/Non-Metal

Employee twisted knee getting out of equipment, was brought to local hospital and released to light duty and is currently scanning mine paperwork.

Employee was off loading a 12" HDPE Tee from the back of a light vehicle when the Tee fell to the side smashing their left ring finger between the Tee and the bed of the light vehicle causing a laceration to the finger that required stiches.

While dismounting a 6200 drill, worker slipped and fell approximately 3' to the ground. The worker began to feel discomfort in their right ankle and was transported to the local medical facility for evaluation.

Operator was tasked with moving a 993K loader from the 5340 bench to the PM bay. As the operator was placing the wheel chocks in the holder operator felt discomfort in their back / shoulder. Supervisor was notified and operator was transferred to local hospital for assessment. Site HSE and management were notified and investigation was initiated.

Employee was flushing valves on the leach pad when they stepped down with there left foot into a soft spot, sinking into the dirt approximately 2/3 up the left leg between the ankle and the knee area. The employee fell forward as they were sinking causing the knee to bend backward slightly.

Employee was at the preg tank to change the filter. EE stepped up onto the curb for the containment area and twisted EE's knee.

Employee was working on the leach pad to repair the water lines for the grasshoppers when some dust possibly containing a small amount of lime got into EE's left eye.

Employee was lifting shop vacuum into the back of the pickup and felt pain in shoulder. This became reportable when employee received prescription for medication to help with pain and sleep on 11/2/20.

Employee was lifting and walking with heavy parts over uneven ground and twisted the right knee.

Employee was cutting some belting on the ground using a razor knife. The knife slipped and caused a small puncture/laceration to the left thigh. Employee was transported to local hospital for treatment.

High 67, sunny, dry, no wind. EE was visually inspecting the placement of synthetic liner by an installation contractor. At the end of normal shift EE indicated right shoulder was experiencing pain. EE did not report a slip, trip, or fall that could've caused injury. We weren't further informed of EE's continuing shoulder pain till after EE personally sought med. attention.

Employee was using clamps to help pull liner into place. Clamps slipped off the liner and employee fell backwards, landing on left hand and injuring wrist.

Employee was working in operating pond east with a water hose for removal of mud and excess water. Employee felt and heard a pop in knee while spraying with the hose and turning a different direction.

As the diamond core drilling crew was adding a 10 foot PQ rod to the drill string, the drill helper had left hand pinched between the drill head and the 10 foot PQ rod.

Individual was moving core boxes from one pallet to another when they caught their foot on one of the pallets and tripped over the pallet twisting their ankle.

Employee was conducting geologic mapping in open country to the south of the mine. While moving down a slope, EE lost EE's footing and slid down, which resulted in injuries to EE's right foot, one finger and a facial cut.

The driller helper was carrying drill rod while ascending down some stairs when he lost his balance and fell to the ground. He strained the muscle in his lower back as a result of the fall.

"EE" was leaving work at the end of his shift. While walking to his car he stumbled, lost his balance, and fell while crossing the parking ditch. "EE" fell forward on his hands and knees and landed face down in the gravel. He felt a sharp pain in his knee when it hit the ground. Hours later his right knee became swollen and he was unable to walk on it.

While working around the core tube breakout tray, the EE slipped and lost his balance in trying to regain his balance, he struck his right hand on the breakout tray. The weather was extremely cold which made the footing slicker than usual. The back of the EE's right hand was bruised.

Employee was on jaw crusher platform doing regular cleaning when he felt his left pant leg get hot. He noticed that his pant leg caught fire from the space heater being used on the platform causing a palm sized 2nd degree burn on his lower left leg.
