Employee was holding steel piece in left hand grinding it with 4 ½ hand grinder held in right hand. Grinder slipped cutting into index finger of left hand.
Employee was cutting conveyor belt with utility knife with one foot on frame rail flange and other on conveyor frame. Foot on frame rail flange slipped causing employee to cut themselves on the finger.
Fumes from welding pipe caused chemical burns on eyes of employee. Employee should be off for 7 days according to Doctor.

Employee stepped off catwalk beside cone crusher onto the base of cone crusher, a no walk zone, lost EE's footing and fell 5 ft to the ground injuring EE's left shoulder.
Employee was prying a rock loose with a shovel, right hand slipped off shovel handle striking the edge of a steel beam, injuring ring finger knuckle.
Employee lost balance while entering into rear of feeder chain area. EE reached out to catch self with left hand jamming left hand into a piece of channel iron causing middle and ring finger to break.
2 employee's lifting tail pulley guard off conveyor, one employee let go of guard allowing it to fall down on EE's foot (RF).

2 employees were using 2 come along hand winches hooked to outside of bulldozer track hoisting engine belly pan into place. One winch slid to the side crushing tip of little thumb between the winch and track pad on one employee.
Plant man got small finger of L. H. between a 3/8X24"X18" plate while installing a chute with 2 other workers. The plate slipped while being slid into place, pinning the finger between the cone screen chute and the plate. The end of his finger was lacerated and the bone of the finger was cracked.
