An employee was in the process of removing a metal dumpster from the bottom floor of our truck unloading building using a hoist. When they removed the dumpster from the building the hand rail was not put back into position and the employee fell into the lifting bay of the hoist. Fall was approximately 37 feet to the bottom level.

An employee fell off of "A" crusher while transitioning from the steps to the platform, which both are built on the crusher. EE landed on feet, but landed on a patch of ice. This caused EE's feet to kick out from underneath EE. EE landed on left shoulder and neck area. This aggravated a previous injury EE sustained at home on 1/7/25.

EE removed guard to clean the belt drum and realized the coal tripper needed move to the N end of the building. EE requested the CRO to start the unloading conveyor and traveling tripper. After the conveyor and tripper started EE laid the spud bar at EE's feet and proceeded to move phone from shirt pocket to pants pocket so it didn't fall out of shirt when EE was struck by bar

Employee (Mechanic) was pulling on the handle of a come-a-long when the equipment mechanism slipped, they lost grip and fell backwards. Their right elbow struck a large bolt on the crusher machine. Bruise to right arm.

Replacing screens and breaker plates on PA Crusher. The hoisted cradle assembly fell free from the swivel pin bolt Holding it in a raised position. The hex nut came off of the bolt threads. The assembly fell onto the arms of the employee.

Employee was using an air lance to clear the inside of a plugged hopper. Air from the lance end blew debris back into the face and a foreign object entered his left eye. He was wearing safety glasses with side-shields.

Initial injury occurred on 1/7/14. The employee was using a spud bar to clear a plugged chute when he felt a strain on his right arm. Initially, no first aid was sought. Later,(1/22/14), the employee went for follow-up tests to see why his arm continued to hurt. Prognosis- partial tear of the common extensor tendon.

EE WAS RE-INSTALLING GUARDRAIL AFTER MAJOR EQUIPMENT MAINTENANCE WHEN HE SLIPPED FROM SAME LEVEL & FELL ONTO HIS LEFT WRIST. DUE TO HEAVY RAINS,HIGH WATER MADE CONTACT WITH FUEL MATERIAL NEAR EQUIPMENT CAUSING MUDDY CONDITIONS.

EE WAS REMOVING THE SCALPING SCREEN DRIVE SIDE SUPPORT AND BEARING. THE SUPPORT HOUSING DISLODGED AND HIT EE ON THE RIGHT ARM AND LARGE TOE ON LEFT FOOT. EE MOVED QUICKLY TO GET OUT OF THE WAY AND STRAINED MUSCLES IN LOWER BACK AREA.
REMOVING OLD ABANDONED DUCTWORK, EE FELL FROM 2ND RUNG OF 6' STEPLADDER WHEN A 5' SECTION OF DUCTWORK FELL FROM TAGLINE AND STRUCK LADDER CAUSING EE TO FALL TO FLOOR. EE RECEIVED NO MEDICAL TR EATMENT BUT WAS XRAYED AND FRACTURED TAILBONE WAS DIAGNOSED.

EE CLIMBED ONTO RECLAIMER BELT TO CLOSE COVER. HE SADI HE WAS NOT PAYING ATTENTION TO THE COVER SUPPORT AND HIT HIS LEFT KNEEL. EE PUT HAND ON DOOR LATCH RELEASE LEVER ON LOADER, SWUNG DOOR IN AND DID NOT MOVE HAND. HAND CAUGHT BETWEEN BAR AND RELEASE LATCH. PUNCTURE WOUND TO HAND.
EE WAS LIFTING NORTH END OF RECLAIMER FLIGHT ATTEMPTING TO FREE IT/ REMOVING IT. WHEN IT BROKE FREE, IT SLIPPED OFF CHAIN AND PINCHED LEFT LITTLE FIGNER TIP BETWEEN FLIGTH AND RECLAIMER TRUSS.
