EE finished making a weld on the excavator bucket, EE lifted welding helmet to chip and clean up what had just been welded and a slag fragment got in eye.

Employee was walking toward the shop when the employee slipped. Snow slick surface.

Walking along pipeline when right knee dislocated.

Shoveling under conveyors & over-exerted self.

Buffing crusher parts before reinstalling back together.

Employee was over around crushing plant greasing when stepped on rock twisting left knee.

Installing discharge chute on conveyor screen when it slipped pinching the right thumb while aligning the bolt holes.

Operator was helping change wear plate on loader bucket weighing less than 50 lbs

While doing maintenance on conveyor #2123, slipped in mud causing contact with pipe support for conveyor.

When holding metal up to be welded in place is when the arc flash occurred.

Setting screens in place when one screen slipped, catching EE's left middle finger between the two screens, EE was wearig gloves.

EE was shoveling wet & heavy materials from around conveyors when he dislocated disc in his lower back. EE did not receive medical help until 10/13/10.

Pulling on a ratchet tightening screen bolts. Note: Injury was not reported until employee went to Dr. on 3/17/09.

Pouring oil in the used oil tank, twisted lower left arm. (April 16, 2009 went to dr for check up on left elbow no follow up necessary and no restriction). by 320091130025

Employee was using a utility knife to cut open a stick of blaster dynamite while loading a rock blast not realizing the position of his fingers.

Stepped on a small rock, rock moved, lost balance and fell against a large rock. Struck his left shoulder, causing a dislocation of his left shoulder.

Greasing crushing equipment, he stepped on a rock and twisted left knee, tearing ligaments in the knee.

CLEANING OF OIL SOTRAGE BUILDING

EMPLOYEE WENT TO LIFT TONGUE ON COMPRESSOR TO HOOK UP.

WHILE WELDING AND GRINDING ON R9OCK CRUSHER A PIECE OF METAL SUBSTANCE ENTERED LEFT EYE CAUSING THE INJURY. SAFETY EYE PROTECTION WAS BEING WORN AT THE TIME OF THE INJURY.

CLEANING ROCKCRUSHER HOPPER OUT, AND WHILE DOINGSO A ROCK CAME SLIDING DOWN STRIKING OPERATORS LEFT FOOT, CAUSING A HAIRLINE FRACTURE.

WHILE PULLING ON CABLE TO ATTACH TO BREAKING BALL, PULLED TOO HARD, TORE MUSCLE AT TOP OF PELVIS.

STEPPING FROM THE LOADER PLATFORM ONTO THE TRUCKTIRE. HE DID NOT USE LADDER PROVIDED. HE LOST HIS BALANCE & FELL ON HIS BACK SIDE.
