Employee shut the drill door on his middle and ring fingers on his right hand.
Employee was replacing track on D10T, pulling and lifting caused him to pull the webbing from a hernia injury two years ago, not job related.
Employee was attempting to grind bosses on a hydraulic control valve. He was standing under the machine, reaching through hoses and looking up at the area where he was grinding. Piece of metal dust flew into his eye.
Employee was stepping off his grader when he hopped off the step, he heard a snapping sound & started having severe bak pain & muscle spasms.
Employee had primed water pump (6") and started engine...The primer plate blew off and hit employee in the head requiring stitches.
Employee was stepping down from drill, missed bottom step and fell backward, landing with his weight on his right hand.

EMPLOYEE SLIPPED & FELL OFF THE CATWALK OF THE DOZER, LANDING ON THE PUSHBEAM INJURING HIS PELVIS.
EMPLOYEE WAS HELPING WORK ON HAULER, HOOD HAD BEEN REMOVED LEANING UP AGAINST HAND RAIL. THE WIND BLEW THE HOOD OVER ON EMPLOYEE STRIKING HIM ON THE LEFT SIDE OF HIS HEAD.
EE was operating dozer on 7/28/05 when dozer hit large 5.5. rock stopping the dozer suddenly jarring ee. He felt pain in his back at that time but continued to work during varying levels of pain until 8-17-05. He was helping mechanic work on dozer, reached something to mechanic and felt severe pain in back went for medical help.
EMPLOYEE WAS OPERATING DOZER AND IT SLIPPED OFF A ROCK AND JARRED HIM. HE INDICATED THAT HIS FINGERS WERE GOING NUMB AND HIS SHOULDERS WERE HURTING.
Employee was servcing the 330 excavator and as he was attempting to dismount the machine he slipped. He placed his left foot on the ground and twisted his left ankle.

Employee was dismounting his hauler and twisted his back. Was not reported until 10-1-04.
Employee was helping repair the d-10r dozer when he either wiped his eye and got piece of metal in his eye or the wind blew the metal into his eye.
ROCK ROLLED OUT OF A SHOT AND WENT UNDER HAULER. WHEN LOADER OPERATOR PICKED UP THE HAULER TO SET IT OFF THE ROCK, THE BUCKET SLIPPED, SHAKING THE HAULER AND EMPLOOYEE.
EE WAS STEPPING OUT OF POWDER TRUCK ONTO FROZEN, UNEVEN GROUND AND SPRAINS HIS LEFT ANKLE.
EE WAS HELPING TO LOAD A BLASTING SHOT, STEPPED INTO A DRILL HOLE, TWISTING HIS RIGHT LEG.

REMOVING RADIATOR OUT OF MACHINE STANDING ON LADDER WITH PRY BAR WEDGING RADIATOR OUT OF SOCKET, PRY BAR SLIPPED OFF RADIATOR. STARTED FALLING OFF LADDER, TURNED & JUMPED TO PREVENT FURTHER IN JURIES, LANDED ON LEFT LEG, BRUISING FOOT & KNEE.
EE WAS DISMOUNTING HIS DOZER AND WHEN HE SHIFTED HIS WEIGHT TO HIS RT.LEG ON THE GROUND, IT STRAINED HIS RT. KNEE.
EE WAS WALKING AROUND HIS HAULER DOING AN INSPECTION. HE STEPPED UPON THE BERM. A ROCK ROLLED OUT FROM UNDER HIS FEET, CAUSING HIM TO FALL, HURTING HIS KNEE.
EE WAS DRIVING A 785 B HAULER WHEN HE HIT A ROUGH PLACE IN THE ROAD ADN FELT PAIN IN HIS LOWER BACK FROM THE JOLT.
WHILE STEPPPING OUT OF A FIELD TRUCK, HE CAUGHT HIS FOOT IN A SHIM THAT WAS LYING ON THE GROUND.WHEN HE STARTED PICKING THE SHIM UP TO PUT IT IN ANOTHER LOCATION IT WAS FROZEN TO THE GROUND & WHEN HE PICKED IT UP IT CUT HIS FINGER.

EE WAS STEPPING DOWN FROM A 375 EXCAVATOR TO THE GROUND WHEN HE STEPPED ON A SHARP ROCK WITH THE INSTEP OF HIS RIGHT FOOT. EE CONTINUED TO WORK UNTIL 8-13-02. HE THEN SOUGHT MEDICAL ATTENTION & WAS OFF WORK UNTIL 8-18-02.
EE WAS WALKING AROUND A DOZER AND STEPPED ON A LUMP OF DIRT, TWISTING HIS RIGHT ANKLE.
EE HAD JUST COME FROM UNDERNEATH A MACHINE AND WAS GOING TO REMOVE ENGINE ENCLOSURES WHEN HIS FOOT SLIPPED IN MUD. EE MOVED QUICKLY TO PREVENTHIMSELF FROM FALLING AND PULLED A MUSCLE IN HIS BACK.
EE WAS TAKING DRAIN PLUG OUT OF A 988 WHEEL UNTI USING BREAKER BAR AND SOCKET. THE PLUG WOULD NOT COME OUT SO EE WAS HITTING THE BAR WITH A HAMMER TO HELP LOOSEN IT, STRUCK HIS FINGER WITH THE HAMMER.
EE STEPPED OUT OF A PICKUP & HIS RIGHT FOOT WENT INTO A HOLE. WHEN HE TRIED TO CATCH HIMSELF HIS LEFT ANKLE & KNEE BENT IN AN OPPOSITE DIRECTION THAN HE WAS FALLING.
EE WAS REPLACING HARD BAR BUSHINGS ON DOZER. THE MACHINE WAS BEING MOVED UP AND DOWN BY THE HYDRAULICS WHEN A WEDGE THAT WAS BEING USED WAS STRUCK AND DISLODGE, STRIKING THE EE.
EMPLOYEE WAS CUTTING A 5 GALLON BUCKET WITH A RAZOR KNIFE AND THE RAZOR SLIPPED AND JABBED HIM IN THE RIGHT THIGH. THIS REQUIRED 4 STITCHES.

EE WAS OPERATING A D11R CAT DOZER IN SHOT MATERIAL AND HOOKED A ROCK WITH THE BLADE. WHEN HE TRIED TO BACK UP AND AWAY FROM THE ROCK, THE BLADE SLIPPED AWAY FROM THE ROCK AND JERKED HIS NECK.
