The employee was doing a ground level repair to a water valve. While exiting the repair area, they attempted to step over a water pipe and a connected hose. Their feet did not clear this structure, and they lost balance and fell onto a concrete slab causing injury to their right shoulder. No safety rule violations were attributed to this incident.

The miner was picking up a 4FT wire rope sling. As EE slid EE's gloved hand through the end loop, it pushed EE's shirt sleeve up, exposing EE's arm. When EE lifted upward, the cable rotated on EE's forearm. A broken wire strand on the sling loop penetrated the skin, became lodged and required medical treatment to remove it. The injury was minor.

Employee was climbing down the ladder of a WA500 Komatsu loader and experienced pain in upper back.

Customer was filling water tank on pick up truck. Stepped up on rear bumper of truck and slipped, falling backwards landing on berm, fractured ankle. Bumper was 27" above the ground.

Upon removal of vandal shield from door of excavator, the shield slipped from the hand of the employee. EE proceeded to throw out left foot to stop it from falling to the ground. This action resulted in a pulled groin muscle.

We have no information as the state police could not find any witnesses on accident scene. We also called the local police and they could not give us any information because it was being treated criminal matter. Trespasser on ATV collided with cable gate resulted in a fatality.

Miner was helping change worn out return idler on a conveyor. While positioning self, a small clump of mud fell from above on to miner's face and eye.

Employee experienced shortness of breath. Employee was taken to hospital & diagnosed with pneumonia.

Miner was manually adjusting the tines on a forklift, while letting go of tine with left hand before removing right hand, gravity took over, resulting in the right middle finger being pinched by the tine and tine backboard.

Employee was carrying discharge hose from hole to hole. Suffered a shoulder strain resulting in physical therapy.

Cardio respiratory arrest

Went to put down empty acetylene tank and felt sharp pain leaving work at 11:30 am.

EE WAS PICKING ROCK IN HEAD HOUSE WITH GLOVES ON. A PIECE OF ROCK CUT HIS LEFT INDEX FINGER.
