Employee moved leads on upper end of conveyor, walked down middle of conveyor to end. End had debris and employee due to momentum jumped down approx. 2 feet from the conveyor end and twisted EE's knee.

Mostrando los 6Employee was striking material build-up from beneath the Fab Tec screen at the wash plant. The material broke free and the clump pinned fingers of left hand between the clump and the brace employee was holding on to.
EE was hooking up chains to pull the Jam die out, he held onto the top of the die with his right hand allowing his middle finger to get between the die & the Jam, the die slipped to one side approx 1/2" & smashed the end of his middle finger.
Freezing temperatures - Employee was standing on a metal plate while welding equipment for repair. His feet started to hurt so he took a break to warm up. He then worked the rest of the day doing the same thing not realizing that he was getting frostbite to his toes.
The employee was digging with a shovel trying to remove a wooden block that was frozen in the ground. When he went to lift it out, he strained his back. He went to the Immediate care center and returned the next day with work restrictions. As he was laid off and filled in when needed, there wasn't any work available for him when he returned.
