EE got out of telehandler and stepped on a metal pan which was on the ground in the path of EE. EE's foot slipped on metal pan. EE fell to ground causing fracture to left wrist.

Mostrando los 43The employee was walking to a piece of equipment in the early morning hours before full sunlight when they stepped on a rock, rolling their ankle which caused them to fall to the ground.
EE was replacing battery while standing on operator platform of equipment when they rotated with battery in hand causing their foot to slip off the platform and going between the track and engine compartment body. EE was initially released to full duty and later placed off work by the medical provider.
EE was drilling holes to install guard when the drill caught metal and twisted in their hand causing injury. The EE strained their right wrist. The EE did not report their incident until 08.18.2025 after seeking medical attention on 08.17.2025. Injury was inadvertently entered as an OSHA incident and is now being corrected.
EE was driving a T-post to anchor the dredge along the berm. When the T-post driver was lifted higher than post, upon downward strike the EE's hands were caught between the post and the driver. EE stated they were not wearing gloves at the time of the incident.
EE was using a handheld grinder w/cone wire wheel to clean a weld. The grinder caught on the pipe and kicked up hitting the EE's welding hood and knocking it up. The grinder hit EE's chin and lip.
EE was using excavator to dig out a silt containment area when the bank gave way causing excavator to roll into hole. Excavator came to a stop with cab side down and operator was stuck inside. EMS arrived on scene and was able to extract EE and transport to area hospital.
EE was changing screens inside screen box and felt a back strain. EE self-cared until 10.13.2023 when they sought medical attention. EE was placed on restricted duty Eff 10.13.2023 and is currently treating and working within restrictions.
The EE was breaking up frozen sand from around the sand screen with a frost bar and felt a tear in their right shoulder. The EE has had an MRI and is working restricted duty.
EE was trying to free a clogged pipe using a compressor when the plastic section broke under pressure. Sand and small rocks flew up and hit EE in both eyes. EE was wearing safety glasses. EE was seen and treated by optometrist & given an Rx to apply to both eyes and able to return to work today with no restrictions. F/U appt on 11.10.2022.
EE was using a straight edge to cut a conveyor belt w/box cutter knife. The blade slipped & cut EE's left wrist/forearm. EE was transported to ER by a co-worker. EE had a surgical procedure to repair a torn nerve, tendon, and artery and discharged from ER after surgery. EE is off work for 2 weeks w/a RTW restriction of no use of left hand/arm for 3 months upon return to work.
EE tripped over wheel chock while placing a water house on their water truck. EE fell forward onto their right elbow resulting in a contusion. EE went to Urgent Care for x-rays (no fractures noted) and denied a Toradol injection. EE returned to Urgent Care on 07.19.2022 for a Toradol injection. Ortho Referral is scheduled for 07.28.2022
EE was assisting co-worker to remove a shaft from a tail wheel when a piece of metal broke off and punctured EE's left arm. EE was transported to Urgent Care to remove the foreign material. EE's arm was cleaned, x-rayed, and given prescribed antibiotics. EE obtained a Return to Work Release with no restrictions.
The employee was moving a water line under the wash plant when they slipped and fell into a support beam. The impact caused a laceration above the employee's left eye. The employee was seen at a local Urgent Care and received stitches.
EE was washing down conveyor and the end of the water hose separated from nozzle striking EE's right hand by their thumb.
While EE was taking apart the dredge, the wrench slipped and strained right shoulder.
Employee was replacing tail pulley in conveyor while sliding shaft into bearing when it became wedged and wouldn't move. Employee tried to adjust the bearing to finish sliding the shaft in when the bearing shifted causing the safety to slip and fall thus pinching employee's right hand between the shaft and conveyor structure.
EE was changing screens and slipped hitting shin on a piece of metal causing a laceration requiring 12 staples.
EE was tightening a belt on the plant screen and the wrench slipped off the nut and EE slipped, and while trying to catch themselves, hit arm on roller on plant and broke the left arm
EE was washing down plant and hose slipped and struck right hand causing injury.
EE was cutting and grinding on conveyor bins. When done building guards, employee rubbed right eye and felt foreign body enter their right eye.
EE was in the process of hooking up lift cylinders to a truck with a come along chain, when the cylinder rolled over and pinched the EE's right thumb between the cylinder and the come along chain, resulting in a laceration that required stitches.
EE was attempting to open up gate on rock bin and the wheel on the gate was not moving freely so EE used a pipe wrench to pry it open and wheel snapped off and wrench hit left hand.
Conveyor was moving heavy rock and became obstructed and stopped. EE was shoveling off rock to free the obstruction and strained abdomen. Employee went to the Dr. on 11/28/18 and was not restricted. EE will have another medical procedure on 12/12/2018 to diagnose the medical condition. The medical notes do not note that incident is related to work at this time.
10/24/18 employee fell when walking up stairs to start the crusher & hurt left wrist. Employer did not know about this incident until 10/26/2018 when employee was told ee was terminated for job performance & then the employee told manager about the incident and that ee had gone to the doctor. Employee was not terminated for this incident.
Employee was pulling a hose filled with water up a ladder and as ee got to the top of the ladder and was pulling the rest of the hose up, ee turned to the right and felt a pain in abdomen.
Employee was bending over to pick up a piece of scrap metal and felt pain in the abdomen. Employee went to the doctor and was diagnosed with an umbilical hernia. Surgery is scheduled for 04/20/2018.
Employee was lifting an oxygen tank and strained abdomen. Employee did not go to the doctor until 7/9/18 and was diagnosed with a hernia.
Employee was manually moving rock at site and strained back.
Climbed out of loader and ran to shut off belt and stepped in a hole and twisted right knee.
Diagnosis of heat exhaustion and dehydration from hot weather. Employee was terminated on 08/24/2017 but not due to the illness. Condition was not reported to employer until 08/24/2017 which is the day employee went to the doctor.
Employee was cleaning off the cross-over belt on the crusher with a spud bar trying to break up a large piece of aggregate. While ee was trying to break down the large piece of material, ee strained right elbow.
Was welding a chain on dredge and a piece of steel flew into eye.
Employee was removing a pin on the surge bin and slipped on ladder while hammering the pin and hit mouth with hammer damaging front tooth.
Employee was trimming rubber skirting on a conveyor belt with a knife and slipped and cut right forearm needing stitches.
Employee seen at Black Hills Urgent Care and there were changes in annual hearing examination which is required by MSHA. Past two years employee was driving a haul truck and operated a loader for three years prior. The findings on the test was moderate to severe bilateral sensorineural hearing loss in comparison with the baseline done on 02/13/2012.

Croell employee was working on the impactor on the mine site and felt discomfort in his chest. Possible heart attack. 911 was called. He is in the hospital in Rapid City for observation.
