Employee went to the truck to grab a sweater. EE felt dizzy and blacked out and collapsed. Another employee was near by and shook ee to wakeup. Employee was escorted to the doctors where it was determined that ee passed out from heat exhaustion. EE was given fluids and sent home

Employee was replacing clutch on VSI Crusher. Driveline was being removed when it dropped on hand lacerating left pinky finger requiring stitches and prescription medication. No time loss.

Employee was cleaning plant. Working surface was muddy. Employee was gathering tools for mud removal when he slipped in the mud and struck left wrist against plant. No time loss, prescription for pain medication was provided.

Employee was changing out a screen in the rock crusher when he lifted the screen over his head. He developed a back strain.

Ee was changing iron onthe crusher impactor when he sustained a pain to his stomach. Doctor's visit confirmed a hernia

Employee was walking and rolled ankle, when he went to recover he fell and hit knee on a rock.

Employee was trying to remove a stuck rock from the crusher when he caught his left middle finger between the rock and the side of the cursher. He needed 5 stitches, removed fingernail.

Dirt and dust in left eye due to blowing dust.

Employee stated he was hammering a punch and the hammer slipped off the punch hitting his left thumb causing a contusion and laceration
