Employee was struck by a rock EE was trying to dislodge, fell off the crusher. Suffered cuts and bruises. Update: EE informed company that EE did not fall off the crusher, EE opted to jump off the crusher. As a result of EE jumping off, EE sustained abrasions and contusions.

Employee was trying to free a hose that was frozen to the ground. When the hose gave loose, the jerk was sufficient enough for the employee's hard hat to fall out, allowing the hose to strike him in the head, causing a laceration.
EMPLOYEE WAS HAMMERING A STEEL WEDGE WHILE CHANGING DIES ON THE CRUSHER, A PIECE OF THE STEEL WEDGE BROKE OFF AND STRUCK EMPLOYEE IN THE LEG. THIS RESULTED IN A FOREIGN BODY LEFT LOWER LEG INJURY.
Typical construction conditions, surface was uneven and rough. EE unclipped harness to converse with the drill foreman. He lost his footing on the rough surface and fell approximately 30' off the ledge.
