While checking equipment in back portion of dragline, employee's foot caught on belt guard causing employee to trip and fall fracturing left knee cap.

August 12, 2021 audiogram results showed reportable hearing shift (test results received by management on 8/23/21)

While moving lubrication oil barrel, barrel slipped from one employee's hand and pinched other employee's finger.

When attempting to move a skid steer bucket, on a table, for repairs, employees left hand got caught between the bucket and table, injuring his hand.

As employee approached a ladder to descend from a dragline, he lost his footing and fell onto the dragline "shoe"; he fell on the same level at which he was walking. He was given treatment by EMTs and taken to the emergency room. Employee has sore back muscles and ribs.

Pulling table bushing out of a drill and aggravated his hernia from July, 2011. Went to see a doctor on September 27, 2011.

Pulling on heavy dragline chain to get it in place. Employee did not seek medical advice until approximately a week later and then was diagnosed with a hernia. After a follow up consultation with another doctor on 7/1/2011, employee in the near future is looking to have hernia surgery.

While carrying lunch pail in one hand, was holding on to climb down rail on front end loader with other hand. Missed bottom step with foot, was still holding onto handrail and stretched. Employer was informed by employee on 5/7/2010 that this incident caused a tear in rotator cuff.

While one EE went to get the planks to place over open floor area, inj EE (before planks were placed over open area) removed chain from protected area, cont'd to remove guard over conveyor, stepped down to where the planks would had been placed and fell 8-10' cracking 3 vertebrae.

Employee pulled too hard, on a 7' long 4x4 piece of hardwood that was being used to create a lever to close gate on crusher, receiving sharp pain in groin area.

VICE GRIP PLIERS WAS HOLDING PIECE OF FLAT PLATE STEEL TEMPORARILY. EMPLOYEE WAS LEANING ON THE PIECE OF STEEL WHEN HE REMOVED THE VICE GRIP PLIERS, CAUSING EE TO LOSE HIS BALANCE & FALL.

EMPLOYEE WAS USING HANDTOOLS TO MAKE REPAIRS TO BRADFORD BREAKER. WHEN THE EMPLOYEE WAS STEPPING BACK ONTO CATWALK FROM BEAM, FOOT SLIPPED OR EMPLOYEE MISJUDGED DISTANCE TO CATWALK.

WHILE EE WAS TIGHTENING BOLT ON PUMP WITH WRENCH, WRENCH SLIPPED, EE LOST HIS BALANCE AND FELL BACKWARDS.

HE WAS RUNNING A BULLDOZE.R HE WAS RIPPING ROCK WITH A DOZER. A ROCK GOT UNDERNEATH THE DOZER AND CAUSED THE DOZER TO SLIP.

HE WAS WALKING WITH ANOTHER EMPLOYEE, CARRYING A SMALL WATER PUMP AND HE STEPPED ON THE EDGE OF CONCRETE AND STUMBLED. THE PUMP SWUNG OUT IN FRONT OF HIM AND HE DEVELOPED AN UMBILICAL HERNIA.

16359 EMPLOYEE HAS DESCENDED THE LADDER AND STEPS OFF OF A DRAGLINE.HE WAS STANDING WITH BOTH FEET ON THE GROUND WHEN HE TOOK ONE STEP BACKWARD WITH HIS RIGHT FOOT AND STEPPED INTO A HOLE APPR 67941TELY TWO TO THREE INCHES DEEP.AS A RESULT HE HYPER-EXTENDED HIS KNEE.THE RESULTANT DAMAGE WAS TO STRETCH OR PULL LIGAMENTS AND ALSO TEAR THE CARTILAGE IN THE RIGHT KNEE. PRIOR TO EMPLOYE

HE BENT DOWN AND PICKED UP A 6X6 TIMBER AND AGGRAVATED AN OLD BACK CONDITION.

HE WAS HOOKING BUCKET UP TO FRONTEND LOADER & HE HAD CABLE STRUNG FROM LOADER TO BUCKET & HE TURNED & BRUSHED CHEEK ON CABLE.

EMPL WAS USING NEW B & O HAMMER WHEN 1/4" PIECE OF METAL FLEW OUT ENTERING RIGHT THIGH ON ONE SIDE & EXITING OPPOSITE SIDE OF RIGHT THIGH.

HE WAS INSTALLING A REAR-END DIFFERENTIAL ASSEMBLY IN A TRUCK THERE WERE NO ILL EFFECTS UNTIL 10 DAYS LATER (WHILE EMPLOYED BY JONES & BRAGUE IN 1979 HE DEVELOPED A HERNIA IN A DIFFERENT AREA OF HIS BODY WHICH WAS REPORTED)

HE WAS OPERATING A DOZER WHEN A BOLT SHEARED OFF, HITTING AND BREAKING HIS ANKLE.

HE WAS STEPPING UP FROM GROUND LEVEL TO A BENCH THAT WAS ABOUT 3 FT.HIGH HE PLACED ALL OF HIS WEIGHT ON HIS RT.LEG & TWISTED HIS LEFT KNEE

HE WAS MOVING A3DRUM OF OIL AND DEVELOPED A HERNIA.

THERE WAS NOT AN ACTUAL INJURY DATE. THE MAN HAS A HEREDITARY PROBLEM WITH HIS BACK IN THAT THE VERTEBRAE ANDLIGAMENTS HAVE DETERIORATED OVER A PERIOD OF TIME. MR. THOMAS DID STRAIN HIS BACK W HILE MOVING AN OIL BARREL WHICH HE SHOULD NOT HAVE BEEN DOING. THIS AGGRAVATED THE PROBLEM WHICH HE HAD IN THE PAST.
