Uasing a rake/pry bar to pull a small piece of wood out from the rock box on the dredge. The piece of wood broke off, his arm slipped & he bumped elbow on rock box.
Employee was using post hole digger without the safety bar between the motor and the post hole digger.
Adjusting valve on 20" cyclones. Trying to close the valve that is stuck open, using a pipe to close the valve.
Injured party was traveling by foot from one area of the property to another when he slipped and fell because of the ice. *The Tri-State area had an ice storm on Tuesday 2/13/08 and everything had ice on it for the next two days. This was a major contributor in this accident.
