Employee was fixing a light fixture in the AF ER Room. Had the wrong breaker turned off (labels are on breaker panel (1) Building Lights (1) ER Room). Used tick tester and it was faulty. Showed no voltage. Employee grabbed connector and received electrical burn.

Mostrando los 6EE was performing preventative maintenance on an MCC bucket. While checking the A phase line side termination EE lifted EE's arc flash hood to align EE's screw driver correctly, as EE lowered EE's hood the screw drive made contact with the frame of the bucket causing an arc flash. EE sustained 2nd degree burns to face.
Electrician had inserted a new breaker bucket to a 480 volt panel and attempted to re-energize the panel. Upon doing so the breaker shorted causing the miner to sustain burns.

EMPLOYEE WAS A SUBCONTRACTOR WHO WAS PULLING THE MOTOR STARTERS CHECKING FOR LOOSE CONNECTORS AND CLEANING THE STARTERS IN THE FINISH MILL AREA. AS HE RETURNED A STARTER TO THE CUBICLE, ONE OF THE WIRES HE HAD FAILED TO TIGHTEN, SHORTED AND CAUSED A FLASH BURNING HIS FOREARMS. HE HAD RETU REMOVED HIS GLOVES BECAUSED THEY HAD MADE HIS HANDS SWEAT.
