Roof rock fell out from between bolts striking employee on the head.
Two bolter operators had finished bolting the mine roof, backed the roof bolter up and started to hang a section of exhaust tubing. A large piece of rock fell out from the rib striking one employee on the leg causing a deep tissue contusion. IE was unable to return to EE's normal work duties. EE was placed on restricted duty restrictions due to weight bearing on the calf muscle.
IE was setting a timber and a piece of rock fell from the roof striking employee on the back of their neck causing a laceration resulting in sutures.
IE had right hand placed on top of the miner, when a piece of cap rock came down from the top and stuck the IE on the right hand, causing a laceration and fractured bone.
IE had kneeled to tie IE's boot lace, when IE was struck by a section of coal that came loose sliding down the coal rib on the Unit 3 working face in Entry #7 at XC 43 on the 3L submain. The IE suffered several small fractures including the left fibula and a portion of the lumbar vertebrae
While installing the outside roof bolt, a piece of coal measuring 3.5' x 1.5' x 1' broke loose from the roof, near the rib that was not cut down by the continuous miner. This piece of coal struck the roof bolter on the right ankle resulting in a fracture to that ankle.
IE was walking leaving Unit 2 RM 1 and was struck by rock that fell from the roof. Small pieces fell first, then a piece measuring (3'.5" L x 2'.2" W x 4' T) struck EE in the hip area. The ER X-rays were negative, and IE was released to work as tolerated. A follow up MRI showed a closed stress fracture of left inferior pubic ramus of the pelvis.
A contract employee was spreading rock dusting by hand. As the employee reached to pick up another bag of rock dust from the scoop, a rock fell from between the roof bolts, striking employee on the left forearm and wrist. X-rays show two fractures, one in the forearm and the other in the hand.
The IE was kneeling on one knee marking up EE's section cut map. The IE heard a pop in the roof right above EE and immediately moved to get out of the way. As the IE stood up and stepped away, a rock fell from between the bolts striking the left leg at mid-calf. X-rays showed no broken bones, IE missed work on 12/19/22 making this reportable.
Employee was backing up shearer and a rock slid over the top of the shearer striking the employee in the arm and leg.
Employee was conducting pre-shift down towards tail gate and a rock fell between the shields striking employee on their hand.
Piece of rock fell out of the roof striking employee on right arm causing a small laceration.
IE was in Unit 1 entry #3, removing a wooden pallet from the scoop bucket. Loose material fell from between roof bolts and struck IE in the right upper portion of their body. First aid was administered, IE was transported out of the mine and taken to the emergency room.
Employee was riding to bottom and was struck by a piece of rock on the right arm causing an abrasion. Employee received stitches for the cut.
IE walked over to check the CM that was down in E2 at 14+18. As the IE approached the CM, a rock measuring (2.5-4 W x 4 L x 5-1 T) fell out from between the roof bolts. The rock struck the IE's head and right forearm pushing IE forward. To prevent from falling forward, the IE planted right leg in front of self. The rock hit the IE's right knee as it fell to the floor.
IE was bolting in RM3 XCR at the 120' station. After IE installed 2nd bolt in the 5th row of the cut. The IE lowered the canopy to swing the boom towards the ATRS to install the last bolt. A rock measuring (4'8" L x 1" to 4" W x 9" T) broke loose from a brow in the roof sliding across the top of the canopy cantilevering over the edge striking the IE on top of the left foot.
IE was backing continuous miner out of right side of cut in room 2 at 2+10 on right side of unit 1. Section of rock fell from between bolts striking IE on top of head/right hand. IE was seen by physical therapist on 12/28/2020 due to type of therapy administered during the visit this is considered medical treatment which makes injury MSHA reportable. IE has not missed any work.
Employee was kneeling down inspecting a roll of belt. A rock measuring 4.5"x3'8"x7" fell from approximately 6', striking the employee in the head, shoulder and back.
IE was operating the head gate shearer drum as the shearer was cutting back toward the tailgate. The IE was standing approximately 10 shields away from the head gate drum when a small rock fell from the roof hitting the pan line before hitting the IE in the safety glasses over IE's right eye.
Employee was hanging curtain when a rock fell out between rows of bolts and struck employee in the head and shin.
Injured employee was operating a continuous miner when a 15"x10"x4" piece of cap rock fell out striking the right foot just behind the steel toe causing a fracture to the pinky toe.
EE was standing under EE's roof bolter canopy when bar slate fell out behind EE. The slate struck the roof bolter then broke up and struck EE on the backside of EE's lower legs. EE received abrasions and contusion to EE's lower legs.
IE was cleaning up some coal/rock material that had fallen from the upper rib area along the beltline. As the IE was shoveling it up, a piece of rock approximately 8" x 7" x up to 2" thick struck the IE on the right side of the face (cheek/Jaw) area. IE complained of pain to the jaw, cheek, neck, and shoulder area and was brought out and taken to the hospital for treatment.
A rock fell from the roof, striking the employee on the middle and upper back causing them to be pushed into the roof bolter canopy. While waiting on the other operator the injured employee inserted their drill steel into the drill head to check for suction. As soon as the steel was removed and was being placed onto the tray a rock fell striking the employee.
IE was struck by a rock that fell out between roof bolts. EE received a laceration to EE's left ear and a fracture to the second metatarsal of EE's right foot.
IE had backed right side continuous miner into intersection to inspect machine, while standing on right side of miner a piece of roof broke loose striking IE on right side of head and shoulder. IE continued to work in EE's normal duties until 2/22/19 when EE received an injection and missed EE's scheduled shift that day.
IE was roof bolting when a piece of rock fell loose and struck IE in the mouth causing a front tooth to break off just below the gum line.
As the bolter was being moved around the corner the ATRS pad flipped up. IE used the palm of left hand to flip the ATRS pad into the normal position. While they flipped the pad a rock fell out of the roof between the hog panel and the rib, striking them on the middle, ring and little fingers causing fractures to the tip & avulsions to ring and little fingers.
A coal hauler operator and the miner operator were standing on the intake side of a line curtain, hanging miner cable in entry # 7. As they were in the process of hanging the cable a rock fell out behind striking both employees on the back of their legs. Coal Hauler operator was transported to medical facility and suffered a fracture to second toe on left foot.
IE (Temporary Employee) was cleaning roof rock off of a parked coal hauler. A piece of rock approximately 3.5 ft. x 1-2 ft. x 2 in. fell out between the roof bolts striking IE on top of the left hand. IE received a fracture to the middle finger of the left hand.
I.E. was operating 239CM on the Right Side of Unit #2. I.E. set the CM over to start on the 1st cut out of the Right Side. After doing so, I.E. knelt down in the corner, which had been cut when the slider was turned for Entry 8-7. A rock measuring approx. 6' x 4'4" x 3" fell from a height of 6.5', traveled approx. 39", and struck I.E. in the Right Shoulder.
IE had a piece of roof rock approximately 2ft. x 2ft. x 0-6in. fall out between roof bolts. The rock either hit the rib or the ground then made contact with EE's right foot causing EE to twist right knee. EE has a right knee strain.
Mine Operator was cutting in 9R and recognized the roof was falling out. EE radioed the section supervisor to come take a look at the roof. EE saw the IE walking up behind and continued to operate the machine. When ee turned around, ee saw the IE on the ground. A rock measuring 18"x40"x3" fell from the roof from approximately 11 6 striking the employee on the head and neck.
IE stated while squatting to look at a continuous miner a rock measuring 16.5" X 5" fell from the roof and struck IE on the hard hat. When this occurred IE lost balance, fell forward, and reached out to brace self. On 12/27/2017 IE was diagnosed with an elbow strain and given work restrictions.
IE stated that the scoop operator had cleaned entry # 7 and part of the line curtain had been torn down. IE went up to help the scoop operator hang curtain back up. While IE was hanging the curtain a rock fell out between the bolts striking the IE on the hard hat.
IE was changing a bit on new drill steel at the start of shift. As IE was standing next to the bolter a rock fell from between roof bolts measuring approximately 4'X3'X4' - striking a glancing blow to left shoulder. IE was taken to the local ER for examination and was taken off work pending a follow up visit on 10/16/2017.
IE was installing roof bolts as primary support in #6 entry. As IE was preparing to place screen wire on the ATRS a rock fell from the roof striking the screen wire on the free end. The screen wire pulled IE to the left and forward then a second rock fell striking IE at waist level. The rock measured approx. 5'x3'x1-6'.
IE (Temp. Employee) was examining the roof on Unit #2 when a rock approximately 2' X 4.5' X 1-8" fell out between roof bolts striking IE on the left upper arm. The laceration required 7 sutures to close.
IE (Temp. Employee) was backing roof bolter out from the face using a remote when the IE was struck by top coal/rock on the top of the head and lower left leg. The top coal/rock was approximately 3ft. X 8ft. X 0-3.5in. IE sustained a fractured tibia and fibula of the left leg.
3-13-2017 after shift- temp emp told a supervisor the temp emp was struck by a rock shoveling beltline. TE said was OK but was reporting per procedure. Supervisor 3 times asked if TE was OK or needed examined etc. TE said was OK. Alleged location was investigated - no rock found. TE began missing work. Mine was informed 6-2-2017 by temp agency work comp was accepting claim.
A Contract employee was roof bolting in entry 3 when a rock fell out in front of the bolter ATRS. A piece of rock broke off of that rock that measured 34"x 4"x 1 to 2" thick, and came back and struck ee on the ankle/leg area. The contract employee was transported to the medical facility and diagnosed with a hairline fracture of the fibula. EE started missing work on 1/31/17.
Temporary employee was hanging curtain and a rock fell from between bolts striking the employee on the top of the foot. The rock was approx. 5'X 2' and about 1-4" thick. This individual was diagnosed with a broken metatarsal bone and started missing work on 11-11-2016
IE was partially struck by a rock that fell out between roof bolts. The rock measured approximately 7'x1.5'-3'x14". EE received a laceration under right eye requiring 6 sutures and a sprained right ankle.
IE was struck by a piece of roof rock. The rock approximately 6" x 8" x 0-2" thick slid off the end of wire roof mesh and struck EE on the top side of EE's left forearm. This resulted in a laceration requiring 7 sutures to close.
A mechanic was driving a maintenance vehicle on a travelroad. Evidence shows a rock fell and broke after striking the canopy in front of the driver. It then continued through the windshield opening striking the employee on the right side of EE's face. Employee began missing work on 3/23/2016.
EE (Temporary Employee) was roof bolting when a piece of roof rock approximately 1ft. x 1ft. x 1-4in. fell and struck him on the right hand. He received a laceration and a fracture to his right ring finger. 4 sutures to close laceration and finger splinted.
Employee was grabbing a roof bolt plate off his roof bolter tray when a rock fell from the roof and struck him on the left hand fracturing his ringer finger.
The IE was traveling the 6th North travelroad when a rock fell from the roof measuring (3'L x 2.5'W x 2.5'T) entering the cab of his Tomcar, striking him on the right hand, chin and upper chest. The IE was transported to hospital where he was treated for his injuries and was diagnosed with a break in the little finger at the last digit
IE was roof bolting on Unit 1 when EE was struck on left shoulder by a piece of roof rock. The rock was approximately 3ft.x3ft.x1-3in. It fell out between bolts. EE's injury resulted in a displaced left distal clavicle fracture.
IE was roof bolting when a small piece of roof rock fell and struck him on top of his upper lip. The rock was 3in. x 3in. x 7in. He received 3 sutures to close the laceration.
