While scaling work area with 8' scaling bar, employee was injured while backing away from some loose EE scaled out of the back. EE tripped causing EE to fall. EE received a laceration to the left side of neck when EE landed on the ground requiring sutures. Also received a fracture to right foot when part of the loose EE had scaled contacted EE's foot during the event

While installing a bolt from a supported ground position, a rock fell from the un-supported area EE was working towards, landing on its edge then tipping towards employee making contact to right leg/foot. Seen by site medic & referred to the ER, X-Rays were positive for fracture to right tibia in leg, sutures for lacerations to right foot, restrictions issued.

While bolting employee stopped to scale down a small basket of loose forming in the screen just below the spring line on the hanging wall. While scaling a small rock came out of the screen & bounced off the scaling bar, making contact to employee's lower lip causing a laceration, seen by site medic and referred to the ER, employee received sutures making it reportable.

Employee was preparing the face to drill and while measuring and painting the face, a rock fell from the face striking the employee on the back/rib area.

After wetting down and scaling a blasted round, employee was in the process of installing an eye-pin when a rock contacted right forearm resulting in two lacerations. Sutures were required with restrictions issued.

Employee was in the process of loading a round and was struck by a softball sized rock on the edge of hard hat and left side of face. Employee was sent to E.R. to be evaluated and received sutures.

Employee was installing an eye pin to move his slusher block when a small rock form the hanging wall contacted his left arm(just above elbow), causing a laceration which required 2 sutures. Full Release issued, No Days Lost.(This happened on his last shift, then went on days off)

Employee was marking up the face when a rock contacted employee's hard hat. Went to medic on site, then to ER. A Full Release was issued. (This occurred on employee's last shift). While on days off, EE notified us that EE was not feeling any better, went to a doctor on 4/23/2015 & restrictions were issued.

Employee was bolting when a rock contacted EE's right foot. Went to medic on site, then off site for X-rays (X-rays were negative). Doctor issued restrictions due to a foot contusion.

Employee finished drilling a hole & was in the process of putting in 2' driver, when he looked up, a small rock came through the screen & contacted him in the upper lip/nostral, which caused a laceration requiring sutures. No Days Lost. Full Work Release issued.

While bolting with jackleg drill, a small rock came down through the hog wire & contacted EE's left middle finger while it was on the cob, resulting in a laceration, which required 3 sutures. Full Release Issued. No Days Lost.

After employee loaded a round, he was cutting wire when a small rock contacted him in the mouth, chipping 2 teeth. Employee went to dentist on his days off (11/17/14) & teeth were repaired. No Days Lost. Full Release issued.

While bolting, a small rock contacted employee in forearm, causing a laceration. (point of contact-kevlar sleeve was covering arm) Sutures required. Full release issued. No Days Lost.

Employee was bolting when a small rock fell through screen & contacted his finger on right hand.

Employee reported that while bolting, small rock came out of back, contacting hand while on the jackleg throttle causing a laceration to finger & fracture to thumb on L. hand.

Employee stated while scaling, rock fell and employee let go of the scaling bar. The scaling bar struck left foot.

Miner was tying in a round and a rock fell & struck him.

Employee barred down & then was in the process of loading the holes, when a rock contacted his finger, causing a laceration/avulsion to his R. index finger tip. No sutures were required, but restrictions were issued.

While bolting, a rock fell from the back, slid down the FW & struck the back of his leg, causing a laceration.

Employee was standing there with his hands in front of him waiting on the miner to finish, when a small rock fell & slid off his shoulder & struck his wrist, causing a laceration - which needed sutures. Full release was issued. No days lost

While bolting, small rock fell striking #3 finger on left hand causing a laceration. Sutures were required. Full release issued. No Days Lost. (this did occur on the employee's Friday)

while employee was operating jackleg - hanging screen, a small rock came down & struck his wrist between his gloves & sleeves, causing a laceration. went to medic on site, we took to ER. x-rays were negative, but does have a F/U appt with another doctor

While bolting into the screen on the rib, a small rock came through the screen & struck him in the mouth, chipping tooth. Tooth was repaired. Full release back to work issued. No days missed.

Employee was drilling & in the process of pulling the jackleg from the hole, a small rock from above struck his right arm, causing a contusion. He went to medic on site. We took him to ER for x-rays, which were negative. Doctor did issue restrictions.

Employee was bolting & went to pull out 6' steel out of machine, a small rock fell & hit his #3 finger (L.hand) resulted in laceration. no sutures were required, however x-rays resulted in fracture to #3 finger. Full release issued, no days lost

Employee was blowing out lifter hole when a rock fell & landed on Left foot. Went to medic on site. We went to ER for x-rays, which resulted in fracture. Restrictions were issued. 1st day missed was 12/20/2011.

Employee was barring down at the 38W FWL when a rock bounced off the bar striking employees hardhat. The force of the hardhat drove down the hat brim & safety glasses causing a laceration to nose. Went to medic on site. we took to ER room. Derma bond was used to close laceration, however x-rays showed a fracture on his nose. He was issued a full release. no days missed

Employee was collaring holes & was struck by sharp rock on left side of head, shoulder & arm. Sutures were required on head & arm. Restrictions were issued, in which we were able to accommodate.

Employee was drilling in the squares of the screen when a small sharp rock came through the screen striking him in the nose & lip, which required sutures. Restrictions were issued. We were able to accommodate light duty work.

Employee was bolting when a rock fell & employee leaned one way, and rock rolled striking him in the knee, causing it to go the other way, a possible strain/sprain. He reported it & went to medics on site. On 5/20/11, he went to doctor who issued restrictions in which we could accommodate.

Employee was driving a bolt in the back when a small rock fell through from behind the wire he was putting up striking him in hardhat & then forearm, causing a laceration, which required sutures. He was wearing his PPE. He was issued a full release. No days missed.

Employee was driving a mucker, when he was struck on the hardhat with a rock. He was taken to ER room off site, and all was negative. He was issued a full release. On 3/22/11 - he had a follow up appt. and doctor then issued restrictions, which we were able to accommodate.

Employee was scaling into the heading. He set up bolt deck. He scaled a piece of loose that was hung up in the mat. He barred it down, and when it fell to the ground it then rolled onto his foot causing a fracture.

Employee was checking rib for Ore, when he bumped up against a rock & it fell/rolled onto his foot, causing a contusion. He went to medic on site, we sent him in for x-rays, which came back negative. He then went on days off, during this time, his foot still hurt, so he went to doctor on 5/3/2010 which gave him some modified duty within his normal job.

Employee was bolting a high footwall hole, when a rock came through the wire out of the back and bounced off the machine & hit him in the nose, causing a nasal fracture & laceration which required 6 sutures. He was given a full release, and returned to work on his next shift, 1/14/2010.

Employee was bolting & a small sharp rock fell & hit him in the mouth/lip area. He was seen at medic on site, and was sent down to see doctor. 3 sutures were required. He was given a full release to come back to work. No days missed.

Employee just started bolting a breast down round. He was drilling when a rock fell off the hanging wall, striking him on top of the left foot. He reported to the medics and was sent down for x-rays, which confirmed a fracture on his left foot.

While employee was hanging the hog wire in his stope, a small sharp rock fell inbetween the hog wire, hitting him on the left cheek, causing a laceration which required sutures. He was given a full release from the doctor.

Employee was painting the face, getting ready to drill, when a small rock fell, hit his hard hat & the hard hat moved down onto his nose, causing a laceration which requred 1 suture.

Employee was rebolting, when a small rock fell & hit his left hand on the index finger, causing a laceration which required sutures.

While employee was bolting, a part of the rock being bolted came down & struck his left foot, fracturing his big toe.

During the bolting cycle, the miner was removing a bolt that had been inserted into a short hole when a large rock slid down the face and pinned his ankle against another rock causing a Bruise to his left ankle.-deep bruise. Upon examination by Dr. he was taken off work.

Employee secured a screen wire when a rock came down bending the screen into the employee's shoulder causing a laceration that was sutured.

Employee was jackleg bolting when a rock fell striking his right forearm (between his Kevlar sleeve & glove cuff) causing a laceration which required sutures.

Employee had removed his glasses for cleaning, while wiping them and glancing around a small piece of rock fell striking him in his left eye. Upon examination, the Dr. took him off of work.

Employee was rehab bolting an area with screen wire installed on the back. A small piece of rock fell through the 4"x4" opening and bounced off of the jackleg drill muffler, striking his face, causing a laceration on the bridge of his nose which required sutures.

Employee was hit in mouth by rock while bolting, causing a crack in two teeth that were repaired.

On the night shift of 1/27/08, the employee was installing ground support with a jackleg drill. A rock fell through the screen, bounced off the drill and struck the employee on the nose causing a laceration that was sutured.

Employee was collaring a hole with a jackleg drill. A small rock fell, struck the drill's muffler then struck the employee on the right, front tooth. The tooth was chipped.
