Employee sitting in a enclosed cab as EE turned EE's head a foreign body fell into EE's left eye, seen by site medic, flushed & removed foreign body. EE's eye was still bothering EE on EE's next shift 5/4/17 seen site medic again removed another foreign body & referred to a ophthalmologist where EE was evaluated & a rust ring was removed making this a medical reportable.

Employee was attempting to reposition the leg of employee's jackleg machine by lifting on the D handle with employee's left arm when employee felt a pop in employee's upper forearm area near the elbow followed by pain. Was seen by site medic and referred to the ER where restrictions were issued.

Employee was installing a termination kit on an electrical cable. EE removed EE's gloves and started slicing the outer jacket of the cable with a razor knife using EE's right hand when the knife slipped contacting the right side of EE's left thumb tip causing a 1cmx1cm avulsion to EE's left thumb. Seen by site medic, referred to ER where EE received restrictions

Employee attempting to loosen the chuck on a cordless drill that had a hole saw bit in the drill & as EE was twisting the chuck their left hand slipped causing #2 & 3 fingers to contact the teeth of the bit resulting in lacerations. Seen by site medic, referred to the ER, 2 sutures were required, Full work release was issued making this a medical reportable.

While scaling work area with 8' scaling bar, employee was injured while backing away from some loose EE scaled out of the back. EE tripped causing EE to fall. EE received a laceration to the left side of neck when EE landed on the ground requiring sutures. Also received a fracture to right foot when part of the loose EE had scaled contacted EE's foot during the event

While drilling a backstope with a long hole drill employee noticed a basket in the wire mesh. While re-positioning the drill the wire separated at adjoining seams allowing some small material (rock) in the basket to fall in front of the drill, startling the employee, causing the employee to retreat quickly and in doing so sprained left ankle in the process.

Employee reported that EE put a split set bolt through wire mesh and lifted it over head and put the end of the split set into a drilled hole and as EE was attempting to maneuver the jackleg to drive the bolt, EE slipped and let go of the bolt and it came down and the bolt struck EE in upper lip causing a laceration.

2/26 a ground fall occurred at 23E 300 A3E. 5' bolts of ground support system came out in the fall, 8' bolts of support system were still intact. A rehab plan was developed of 12' bolts & shotcrete. Rehab was started, ground started unraveling, releasing the 8' bolts, 12' bolts held. 2/27 management inspected area & called to hotline even though conditions didn't meet criteria.

After Jacking up a rail flat car to re-rail. 2 employees began to push car back on the track. 1 employee slipped causing both employees to fall & in the process the employee who slipped, boot contacted the other employee in the upper lip causing a laceration. Seen site medic & referred to ER, sutures were required, full duty release, no days lost, making it a medical reportable.

On 2/19 employee was helping partner pull a jackleg out of the backfilled floor when EE felt a burning sensation in the right shoulder. When EE reported for the next shift on 2/20 EE reported the incident to supervisor and was then seen by the site medic & referred to the ER. X-rays were negative, restrictions were issued for muscle strain to right shoulder area.

While installing a bolt from a supported ground position, a rock fell from the un-supported area EE was working towards, landing on its edge then tipping towards employee making contact to right leg/foot. Seen by site medic & referred to the ER, X-Rays were positive for fracture to right tibia in leg, sutures for lacerations to right foot, restrictions issued.

While preparing to off load a dumpster at trash yard employee slipped & fell on snow/ice in a non-designated travelway while attempting to chock vehicle. Employee was seen by site medic complaining of low back pain & was referred to the ER, X-rays were positive for a tail bone fracture & restrictions were issued.

While bolting employee stopped to scale down a small basket of loose forming in the screen just below the spring line on the hanging wall. While scaling a small rock came out of the screen & bounced off the scaling bar, making contact to employee's lower lip causing a laceration, seen by site medic and referred to the ER, employee received sutures making it reportable.

After installing a 3" victaulic valve assembly below the pump on an 8" waterline employee turned the valve on & the valve assembly began to spin, the valve handle caught the glove of the employee's right hand twisting their hand & arm around the valve assembly & pipe causing injuries to right hand & arm. Seen by the site medic and referred to ER, Restrictions were issued.

Employee said they stepped to grab a 6-foot jackleg drill steel and experienced some knee discomfort. Employee worked the rest of the shift. Employee went to the doctor the following day and an MRI was taken. Employee had an appointment on December 19 to review the MRI and was diagnosed with a small meniscus tear.

Employee was conducting Shelby tube sampling on tailings using hydraulic ram set. While taping cap to sample tube, the sampler slipped from chucks lacerating and fracturing the tip of the right number 4 finger. Full release, no days lost. Employee is from Canada and has valid TN-1 status. (no social security number)

Two employees were hanging 80' of 2" HDP pipe in the Main behind the bolter. The two employees secured the pipe and blew the pipe out prior to hanging it. They locked out the compressor then hung the pipe. They unlocked the compressor, one employee entered the cab of the bolter, the other charged the pipe. Material from the pipe struck the bolter window and employee.

Employee was attempting to enter the back of a vehicle when EE felt a pop in EE's left calf muscle on EE's last shift. EE was seen by the site medic. The following day the employee went to a doctor and was diagnosed with a muscle tear and restrictions were issued.

Employee finished bolting & while wiping face with a rag, EE felt something in left eye & EE stated EE got it out. At 4:00 am it started bothering EE again, EE reported it, was seen by site medic, referred to the ER, then onto a opthamologist where a rust ring was removed & restrictions were given.

Employee was bolting with a jackleg drill and while was bent down picking up the drill a 6-foot drill steel fell and struck the employee across the back of the head. The employee called off on last shift of the rotation. Upon further investigation when the employee returned on the next rotation (4 on 4 off rotation) it was determined that EE called off for a headache.

Employee was rehab bolting with a jackleg and while attempting to drive a 5-foot split set in the upper hanging wall rib the jackleg pushed back pinching arm between the footwall rib and the jackleg feed control handle resulting in a fracture to the right ulna.

Employee stated was walking back from starting an auxiliary fan, stepped on a rock in a water puddle & it rolled out from under foot causing employee to twist left knee & ankle. Employee finished shift & reported but refused a medical evaluation. Prior to next shift the employee reported injury had worsened & requested to be seen by physician. Restrictions were issued.

At 0840 Company became aware of fall of ground beyond support, reported to MSHA hotline & field office at 0847. A verbal 103K order was issued at 1447. 10/20/16 inspector arrived at site, starting investigation. 10/21/16 remediation plan was developed & approved by Rocky Mtn. District & field office. 103K was terminated, company will update MSHA on the progress/completion of plan.

On 9/29/16 employee reported to the company EE's right hand was bothering EE & EE was going to seek medical treatment. EE stated there was no event, impact or injury, that it had started acting up over the course of time the last few months. EE was seen 10/5/16 by a physician & diagnosed with tendonitis in the right hand & restrictions were issued. 1st shift missed was 10/7/16

Employee was carrying a Gardner Denver drill to the face and while carrying the cradled drill EE felt pain in the center of EE's back. EE was diagnosed with a back strain.

Employee was operating a jumbo drill and when EE went to dismount the drill using the handrails and steps EE twisted left ankle as EE stepped to the ground. EE continued to work for about 2 hours but pain was persistent so EE contacted supervisor, seen by medic, referred to ER where restrictions were issued for swelling.

Employee was carrying a BO soot Trap Filter back to the service vehicle when EE stated, that EE stepped wrong and felt a pop followed by discomfort to right knee. EE reported to the supervisor and was seen by the site medic and referred to the ER where EE was issued restrictions.

Employees low back/right leg had been sore for a period of time, and EE could not specify an event causing the pain. Exiting a mantrip at the start of shift EE re-aggravated the condition, and reported later in shift to the supervisor. EE was seen by the medic and referred to the ER and restricted until further care from a specialist.

A Non Employee (MSHA Inspector) was startled by an approved on-shift blast causing them to lose footing during their retreat and fell injuring right middle finger. They were seen by the site medic and referred to the ER where they were diagnosed with a dislocation to finger and required to wear a splint.

Employee was pulling a jack-leg machine & driver off a newly installed bolt when EE slipped causing EE to fall with the machine. EE's right ribs made contact with D-Handle. Employee reported to supervisor at the end of the shift and was seen by the medic who referred EE to the ER. X-Rays were positive for a non displaced fracture to a rib and restrictions were issued.

Employee was preparing the face to drill and while measuring and painting the face, a rock fell from the face striking the employee on the back/rib area.

Employee reported to supervisor ee was not feeling well and was seen by the company medic who referred EE to the ER where EE was diagnosed for heat exhaustion and restrictions were issued.

On 7/23/16 a miner reported a possible fire on a 2 yard loader at 44W 14300 mining block. The mine was evacuated & it was called into MSHA. Working w/MSHA local field office, Mine Rescue teams entered the area & found no fire had occurred. An investigation team found the source to be coolant leaking on the exhaust & therefore this was not an immediately reportable incident.

Employee was cutting broken angle iron framework on the trash screen and the cutting tool slipped off of the angle iron and contacted left forearm resulting in a laceration.

Employee got a glass sliver in right index finger cleaning up a broken window 7/12/16. Sliver was removed, seen by medic & released back to work. 7/21/16 went to physician as felt something was still in finger, examined, given full work release. 9/11/16 seen again as it seemed to be infected, injury location was opened & cleaned requiring 1 suture to close becoming reportable.

On 6/11/2016, a subcontractor (on days off) to the general contractor found the high wall at the Benbow Development/Construction project had a slough. The root cause analysis and follow up mitigation plans are currently being developed and will be forwarded to the MSHA Field Office upon completion.

While stepping out of a 2.5 yard loader, employee lost balance causing EE to step backwards and the foot bent awkwardly. The following day, the employee was experiencing discomfort and EE was evaluated and diagnosed with a hairline fracture to the 5th metatarsal and issued restrictions.

An employee and a partner were lifting a valve onto a flatbed pickup, when the valve was on the vehicle they went to lay it down flat, the employee pinched their left little finger between the flatbed and the valve causing a laceration. Employee was seen by the medic and referred to the ER, sutures were required, no restrictions were issued, no days lost.

Employee was pulling out of a drill hole with the jackleg when the leg kicked out causing the jackleg to drop un-expectantly pinching the employees left little finger between the machine and a feed frame of a diamond drill. Employee was seen by the medic and referred to the ER, no sutures were required and x-rays were negative, but restrictions were issued.

Employee was performing overhead welding when a piece of slag fell inside of the employee's boot resulting in two burns to the employee's foot, one on top of the foot and the other on the inside of ankle.

Employee went to close the man gate of a John Deere tractor, upon doing so pinched the tip of right pinky finger between the gate and man basket frame. Employee was seen by the site medic and referred to the ER, X-rays were positive for a hairline vertical non-displaced fracture of the pinky finger tuft, full release was issued and no days lost

Employee was tightening a bit in the chuck on a magnetic drill press, the chuck key slipped out of the teeth of the chuck causing EE's third knuckle of right middle finger to come into contact with the edge of metal EE was drilling causing a laceration. EE was seen by the medic and referred to the ER where EE received 2 sutures, no restrictions were issued.

After wetting down and scaling a blasted round, employee was in the process of installing an eye-pin when a rock contacted right forearm resulting in two lacerations. Sutures were required with restrictions issued.

On 3/30/16, employee reported that on 3/24/16 was injured when lost control of the jackleg while bolting, and while getting out of the way something contacted left shin, causing a small abrasion to shin. On days off, pain and swelling persisted. Employee was seen at the ER, referred to a specialist, a small piece of metal was removed, restrictions issued.

Employee was walking around a parked loader and tripped and fell striking head on the ground. Employee was seen at the ER and received sutures for two lacerations on forehead. Full work release was issued, no days lost.

Employee was seen by doctor on 3/3/16 for low back strain due to wear and tear over years of mining, no specific event. Employee was issued restrictions on 3/14/16 as an occupational illness taking the employee off work.

Employee was performing a sand pipe swap and while attempting to line the 4" pipe up to connect it felt a pull in right bicep area. The employee was evaluated on March 3rd and received restrictions making this incident reportable.

After employee finished drilling a round with a jumbo drill, was changing the bit on the jumbo and was struck on the ring finger resulting in a finger fracture and laceration.

Employee stated lifted a core barrel tube and twisted and felt pain in lower back. Employee ended up receiving medical treatment and received a full work release to return to work on 2/22/2016.

Employee was installing a water line at a tailings impoundment when wind gusts blew dust in behind safety glasses and into eyes. Went to the medic and was referred to the E.R. then to an eye specialist to remove imbedded dust particles and rust rings that were starting to form. Full Release issued - no days lost.
