Employee was bolting in #6 entry, putting up the last row of bolts in the face. The hole was drilled and the employee was putting the glue in the hole when a piece of rock fell glancing him on the back of the head resulting in a laceration.

The employee was running a centerline in the # 1 heading when a small piece of draw rock fell from the roof hitting the employee on the left wrist. This resulted in a cut to the left wrist requiring (4) stitches.

While removing a pogo stick to pull down curtain a rock approx. 14 inches long and 5 inches wide was dislodged cutting the left hand below the thumb. Resulted in three (3) stitches.

Rock fell from the top and struck the employee on his right foot while he was bolting top. Resulted in a cut to his foot requiring stitches.

Employee moved a pogo stick on corner in #5 entry when a piece of drawrock fell and cut his right wrist.

Employee was struck by a piece of drawrock on left arm and elbow. He continued working until 1/16/09 when the doctor removed him from work.

Employee was installing a roof strap when a piece of rock fell striking him in the upper left side of his lip causing a laceration. The rock was 10 inches by 17 inches wide and 1/2 inch to 3 inches thick.

EE was bolting top when a piece of rock fell from the roof striking the canopy and hitting the EE on the left side of body, causing 2 cracked ribs.

Employee was bolting top when a piece of rock fell between last row of bolts and the atrs on the bolter stirking the victim on the head after breaking on the canopy.

A piece of rock fell from between the bolts and struck employee on left shin causing a laceration.

EMPLOYEE STATED THAT HE WAS HOOKING UP HYD HOSES TO CHAIN SAW AND A PIECE OF DRAW ROCK FELL, STRIKING HIM ON HIS LEFT HAND CAUSING A LACERATION.
